RE: Disinfection Byproduct Reduction and Water Treatment Plant Pipe Gallery
Improvements Project, Specification No. 1000044
A pre-construction conference was held December 5, 2019 at 1 PM, at 919 Palm Street,
San Luis Obispo, CA. A fully executed copy of the contract is attached. The following
information is required for use in the administration of the project and prior to the beginning
of work and shall be submitted as soon as possible.
1. Emergency Standby List (complete attached form). List at least two emergency
personnel who can be contacted after normal working hours and on weekends.
Include name, address, work and home phone numbers, and their position relative
to this project.
2. A statement designating your authorized representative, who shall have the
authority to act on our behalf at the site of the work.
3. A list giving the names and titles for the company official authorized to sign contract
change orders and extra work bill.
4. An equipment list and rental rate for equipment to be used on the project.
5. A water pollution control plan (complete attached form).
6. Evidence that construction yard/disposal site are correctly permitted, if applicable.
The City will review the material and issue a no-fee Encroachment Permit for the
Disinfection Byproduct Reduction and Water Treatment Plant Pipe Gallery Improvements
Project, Specification No. 1000044. Contract Time shall commence on December 9,
2019 as previously agreed by both Cushman Contracting Corporation and the City of San
Luis Obispo.
