Record Description
Sometime during the late night of Sunday, May 10 or early hours of morning Monday, May 11 a call was placed to 911 from
my mom asking for assistance as my dad had fallen and could not get up. My parents are Carol and Olin Gordon Wilcher Jr.
and live at 1512 Balboa Street, San Luis Obispo, CA 93405. The call may have been placed either on the land line 805‐234‐
8558 or on my mom's cell phone 650‐865‐0254.
Three firemen entered the house in corona virus protocol/ protective gear and were able to get my dad up and back into
bed. I think that Firehouse 4 is the closest firehouse to their home, but do not know who had responded.
I am their youngest daughter and live in San Luis Obispo. I am requesting ALL records from this incident including call
transcript, any reports filled out, and any waivers/ reports signed by either of my parents declining immediate medical
assistance, and any follow up made by department personnel.
At this point, I am trying to gather as much information as possible about what happened.
At some future, point I may request access to fire department policy, procedures, and training when assisting victims of a
fall. Please let me know any other form you may need me to fill out. Thank you.

agency or his or her designee to the person making the request, setting forth the reasons for the extension and the date
on which a determination is expected to be dispatched. No notice shall specify a date that would result in an extension
for more than 14 days. Pursuant California Government Code Section 6254 (c) Personnel, medical, or similar files, the
disclosure of which would constitute an unwarranted invasion of personal privacy [are exempt from disclosure].
