Thank you very much, Jan Maitzen, Vice President and Grants Manager Transitional Food and Shelter, Inc. 1 4507 Del Rio Ave. #1 - Atascadero, CA 93422 - (805) 466-5404 www.nowheretogo.com Mailing Address: 7343 El Camino Real #346 - Atascadero, CA 93422 HOUSING- HELPING- HEALING June 3, 2020 City of San Luis Obispo Human Relations Commission Agenda Item # 3: Consideration of the 2020-21 Grants-In-Aid (GIA) Preliminary Funding Recommendations Transitional Food and Shelter is both honored and grateful to be recommended for funding through Grants-In-Aid for our Medically Fragile Homeless program. We ask for your approval of the recommendations. TFS has provided individual studio apartments, case management and supportive services for the homeless who are medically fragile since 2004. While the apartments are in Atascadero, it has always been a county-wide program. These funds will add significant capacity for TFS to serve residents of the City of San Luis Obispo. Those who stay with us are referred by Sierra Vista as well as Twin Cities. Referrals are received from agencies such as DSS, Family Care Network (HSP) and other service providers when there is a medically verified need for non-congregate care for individuals and/or families experiencing homelessness and in need of safe and fully operational housing to ensure healing and greater health. Participants of the program are referred for 7-90 days. In addition to housing and assisting with basic needs, TFS works to connect those it serves to income, health services and more permanent housing through case management and transportation to ensure access and follow through. To accomplish the goals of case plans, TFS works closely with all service providers, partners and county-wide systems. This grant will assist in paying for the cost of one of the units and the case management so necessary to achieve the outcomes of restoration of health, increased income and more permanent housing for th e medically fragile homeless community members of the City of San Luis Obispo.
