Since 2004, the behaviorally based Medically Fragile Homeless program has provided individual, studio apartments, case management and supportive services for those who are homeless and in need of non-congregate shelter to ensure physical recovery from surgery, illness or injury. Those who stay with us are referred by hospitals, county agencies and other service providers when there is a medically verified need. Participants are referred for 7-90 days. In addition to housing and assisting with basic needs, TFS works to connect those it serves to sources of income, health services and sheltered, more permanent housing through case management and transportation to ensure access and follow through. Although the studio apartments are located in Atascadero, the MFH program has provided its services county-wide since it was established. This grant funding will assist in paying for the cost of one of the units and the case management so necessary to achieve the outcomes of restoration of health, increased income and more permanent housing for the medically fragile homeless community members of the City of San Luis Obispo. The funding we requested and for which we are recommended is a significant amount, as is the need and we are grateful for your support and approval of the recommendation.
