David Setterlund Recreation Supervisor Parks & Recreation Community Services & Events 1341 Nipomo Street, San Luis Obispo, CA 93401-3934 E dsetterlund@slocity.org T 805.781.7067 slocity.org 1 The Special Event Application for Shinning Our Light: Re-Imagining SLO to be held at Mitchel Park on Sunday, August 16, 2020 is approved subject to the following conditions: GENERAL INFORMATION 1. Facility Use Permit #R21890 has been issued for the use of Mitchel Park from 6:30 PM to 9:00 PM. Please remit the balance due (Fee waived) prior to your event – without the balance paid in full the event is null and void. 2. Please sign and return the above mentioned facility permit once received after Parks Maintenance meeting within seven (7) days. 3. Amplified sound is approved for this event. Per City of San Luis Obispo Ordinance 9.12.040-9.12.070, sound levels may not exceed 60 decibels. If noise levels are excessive, and complaints are received from nearby residents, the sponsor may be requested to reduce or eliminate amplified sound. 4. A Certificate of Insurance showing Property Damage and General Liability in the amount of in an amount $1,000,000 per occurrence, $2,000,000 general aggregate, for bodily injury, personal injury, and property damage, including without limitati on, blanket contractual liability and an Endorsement naming the City of San Luis Obispo as an additional insured is required and has been purchased through the City of San Luis Obispo. If alcohol is sold during the permitted activity, coverage must include full liquor liability The Certificate is enclosed. The sponsor is required to obtain insurance for individual vendors as appropriate. 5. Sponsor must adhere to all standard restrictions for special events as listed on the special event application. SUSTAINABILITY In accordance with Municipal Code Section 8.07 of the Health and Safety plan, events cannot sell/distribute single-use plastic bottles or cups. Plastic straws may only be served upon request. In accordance with Integrated Waste Management Authority Ordinance 2008-3, events must provide sufficient garbage and recycling services. Garbage/Recycling: At least three weeks prior to the event, complete Special Event Checklist with Parks Maintenance Division Supervisor. You will cover: Sufficient Service: The number and size of cans needed for your event will be determined by the Parks Maintenance Division. Smaller events may be compliant with existing infrastructure. Additional Service: If additional cans and service are required per the Parks Maintenance Supervisor, contact San Luis Garbage at 805-543-0875. Organic Waste: If food will be served and additional service is required, food waste service must also be arranged for. Waste should not be hauled off-site. Bins should be serviced on-site by San Luis Garbage or the City of San Luis Obispo. It is the event holder’s responsibility to monitor solid waste practices at this event to prevent contamination of bins. Special Event Requirements EVENT SPECIFIC | Conditions Regarding Your Event 1. No city streets will be closed for this event. 2. Sponsor is responsible for removing all trash and debris remaining from this event. 3. Dogs must be kept on leashes at all times. 4. Alcohol is not permitted to be served or sold at this event. 5. The event is not permitted for Vendors to participate in selling food, goods or services. PARKING AND TRANSIT INFORMATION 1. The parking lot behind the Senior Center is not available for attendee use. Please encourage attendees to utilize the parking garage on Marsh Street. Best wishes with the event!
