Box 3810 San Luis Obispo, CA 93403 RE: Railroad Safety Trail Taft to Pepper, Specification No. 91375 A pre-construction conference has been scheduled for __TUESDAY, SEPTEMBER 15, 2020 at 10:00 AM. To follow newly instituted health protocols, all the participants to this project will be able to join us remotely via phone call and/or videoconference. A fully executed copy of the contract is enclosed. The contract time shall commence on Monday, September 28, 2020. The following information is required for use in the administration of the project and prior to the beginning of work and shall be submitted at the pre-construction conference or sooner. 1. Emergency Standby List (complete attached form). List at least 2 emergency personnel who can be contacted after normal working hours and on weekends. Include name, address, work and home phone numbers, and their position relative to this project. 2. A statement designating your authorized representative, who shall have the authority to act on our behalf at the site of the work. 3. A list giving the names and titles for the company official authorized to sign contract change orders and extra work bill. 4. An equipment list and rental rate for equipment to be used on the project. 5. A progress schedule showing the order in which the contractor proposes to carry out the work, and the contemplated date of completion. 6. A traffic control application 7. A traffic control plan (see email dated 8/25 to Steve Souza re: Traffic Control for Spec 91375). 8. A water pollution control plan (complete attached form). 9. Location of construction yard. 10. Location of disposal site. 11. Evidence that construction yard site is correctly permitted, if construction yard in not your business address. 12. Evidence that disposal yard is correctly permitted. 13. A door hanger for notification of adjacent properties.
