Dear Sir or Madam, This is a formal letter requesting CERTIFIED PAYROLL RECORDS AND PUBLIC WORKS FRINGE BENEFIT STATEMENT from the above noted contractor. We are requesting the records from, 4120/20 to 11/2/20 for all concrete & related work. Please notify this office when you receive the records. This request is made pursuant to Labor Code Section 1776. As stipulated in Section 1776 (d) certified copies must be submitted within ten (10) days after receipt of written request. In the event that the contractor fails to comply within the ten day time frame, the contractor shall forfeit $100.00 for each calendar day, or portion thereof for each worker, until strict compliance is effectuated. The Cement Masons Southern California Work Preservation Labor Management Cooperation Committee ("Cement Masons LMCC') is entitled to receive the above -requested certified payroll records ("CPRs") pursuant to Labor Code Section 1776. The Cement Masons LMCC is a Joint Labor Management Committee established pursuant to the Federal Labor Management Cooperation Act of 1978,29 U.S.C., Section 175a. Pursuant to Labor Code Section 1776(e), the only information which may be obliterated in the CPRs are the social security numbers of the individual employees listed therein. Therefore, the employee names, addresses and classifications must remain on the CPRs and cannot be redacted/obliterated.
