Additional Narrative (#1 of 5 ): BC2 C3 with first alarm to reports of possible structure fire, BC2 arrived at scene and established Prado IC and found a large commercial structure with a vegetation burning on the charlie side of the structure, fire was approximately 1/4 in size burning in brush, trees, and refuse with moderate rate of spread, area appeared to be a hoarding site and was filled with large amounts of trash, refuse, and disabled/abandonded vehicles, small driveway limited apparatus access, working incident declared, truck 1 and 2 staged on street with personnel assigned to walk in, E4 brought in to scene to pump fire, E21 assigned warter supply, E4 was assigned Div A w/ T2 assigned to assist, T1 was assigned Div Z, and E3 was assigned patrol for ember cast on the Alpha flank, PD assigned evacuations of area and homeless persons camping throughout the area, T2 reported hazard of firefighter falling approximately 10 feet into ditch, non-injured, hazard reported to all crews with radio acknowledgement received, all crews required to wear SCBA due to smoke and fuel types, fire knocked down and overhauled, light and air unit requested to scene, Fire Marshall requested for investigation, DC given city coverage, E3 was released for Code 11, Cal Fire Code 11 engine cancelled, scene turned over to E4 and FM, BC2 avail. Fox BC2 A Page 9 of 10 Additional Narrative (#2 of 5 ): Fire Origin and Cause Report Rodger Maggio, IAAI-CFI June 4, 2021 Property: Approximate 2 acre parcel with 2 Single Family dwellings used for storage of combustible waste material. Conditions: 54 degrees F, 90% rH, winds S at 3 mph (at time of dispatch). On Friday, June 4, 2021 at approximately 02:18 AM, I received a phone call from San Luis Obispo Fire Dispatch requesting that I respond to a 2nd alarm fire at 46 Prado (parcel includes 46, 48, 50, and 52 Prado) in San Luis Obispo. I arrived approximately 10 minutes later and made access to the fire at the end of a narrow paved driveway. At the end of the pavement the driveway transitioned to dirt and led to an area with piles of stored combustible waste and storage, where I investigated a previous fire in December 2020. At the end of the driveway I encountered the firefighters overhauling an approximate 50 x 50 foot area of vegetation and debris. I checked in with Battalion Chief Sammy FOX before initiating my scene examination. The burn area was bordered on the south by a trash enclosure, to the west by the driveway, to the north by a Honda mini van and eucalyptus tree, and to the east by a creek and ravine. On the south end I observed a free standing barbeque grill that had fallen over and was next to a breached chain link fence that separated the property from the creek bank to the east. Against the fence were several large tractor tires that were scorched, but not consumed by the fire. There were numerous cut, charred logs in the burn area. On the north side the eucalyptus tree had burn scars that extended 30-40 feet up the bole with large branches that over hung the burn area exhibiting significant charring. Just south of the tree there appeared to be the remains of a 6 x 5 foot fiberglass camper shell that was completely consumed. To the west of the fiberglass debris was an 2003 Honda Odyssey mini-van . The van was aligned west-east with the front facing west. The vehicle burn damage was more severe from rear to front with complete passenger compartment involvement. It appears that a fire came from the south east based on the burn indicators. During my investigation, the horn sounded, so most likely the vehicle was operational prior to the fire. The engine compartment was unaffected. Approximately 3 feet south of the van I observed the remains of an upholstered chair which consisted of the metal springs and the wood arm rests. Moving further south were other items that were more completely consumed such as a vehicle wheel that only had a remnant of the tire on a protected part of the rim. It is my opinion based on my observation and training that this fire originated in an area approximately 8 to 10 feet south of the van and moved symetrically out from that area spreading through heavy tree litter and debris. As there were no accidental ignition sources in the origin area and the temperature was too low and humidity to high for cigarette ignition, it is my opinion that the fire was human caused. Possibly causes are recklessly kindled campfire, overturned barbeque, or intentional act. The name of a person of interest related to the van was forwarded to the police department for follow up interview. Until such time as more information becomes available, the cause of this fire is undetermined. Additional Narrative (#3 of 5 ): T1 responded to above location with fist alarm. T1 arrived at scene and staged in the street due to no access for the truck. T1 personnel geared up and walked to the fire area. Initially T1 crew assisted with securing a water supply from E4 with E21. T1 was assigned div. Z. T1 crew then made are way back to E4 and pulled a cross lay. FF1 and Capt. 1 began extinguishing fire while operator 1 and ladder 1 used the circular saw to cut through fencing to make access to other parts of the fire. E21's crew was assigned to T1 to assist with cutting line around the zebra side of the fire towards the creek. Once that area of the fire was extinguished and a line was cut from the driveway to the creek area. T1 crew assisted with overhaul of the fire area. T1 was then released by IC. T1 crew gathered used SCBA bottles form E4 and T2 and replaced them with fresh bottles then cleared. Additional Narrative (#4 of 5 ): E-3 was assigned to Division Alpha to search for embers cast from the fire to the other exposures on the property, after completing that assignment, E-3 was reassigned to assist E-4 and T-1 personnel with mop up and overhaul of the fire area. E-3 was released by IC and assigned to code 11 at station 1. Captain Mike King Additional Narrative (#5 of 5 ): T2 arrived and assigned to pull line off E4. T2 pulled 1 3/4 line crosslay to the Alpha side of the fire and engaged in fire attack. T2 knocked fire down, while wearing full PPE. T2 FF had a fall down embankment after ground sluffed off and FF2 fell approx 10 feet into creek bottom. FF2 reported no injuries and priority traffic made to IC to notify all units of hazard on Charlie side. T2 capt stood by to ensure no one went near edge until hazard tape could be used to mark area. T2 rehabbed equipment and assisted E4 getting back into service. Page 10 of 10
