Human Services Grant Preliminary Recommendations Page 2 City of San Luis Obispo Organization Project/Program Description of Project/Plan Amount Requested Amount Recommending 5 Cities Homeless Coalition Rapid Re-Housing & Homeless Prevention Assistance Collaborating with key agencies throughout the county to provide housing stabilization service, deposit and rental assistance to those moving into permanent housing $10,000 $10,000 Big Brothers Big Sisters Community and Site Based Mentoring for Vulnerable Youth Support 85 at-risk children and youth and 50 high school Bigs receiving leadership skills. $15,000 $5,000 City Farm 2024 Youth Empowerment Program In the Summer of 2024, YEP will provide 1,500 individual instruction hours to 38 teens. 5,000 pounds of produce will be delivered to the SLO Food Bank $15,000 $15,000 Community Action Partnership CAPSLO Homeless Prevention Services CAPSLO provides emergency financial assistance to residents in households in the City of SLO that are at risk of being evicted and need help with a rental payment $10,000 $10,000 Community Action Partnership CAPSLO SAFE Grant funds will be used to support a portion of a full -time SAFE Family Advocate, who will provide services to SLCUSD homeless students’ families and unaccompanied youth in the City of SLO when school is not in session $10,000 $5,000 Court Appointed Special Advocates General CASA Program Support Will partially cover rental fees for CASA office $7,500 $0 Dignified Hope Care Community Services Senior Caregiving Funds will be used to pay for the implementation of outpatient caregiving that will work in cooperation with existing hospitals and home health agencies $20,000 $0 Diversity Coalition Diversity Education 2 aspects to project - 1) Local Storytelling, a partnership with SLO Tribune; 2) School Speaker Program, bringing speakers to schools $5,000 $0 Human Services Grant Preliminary Recommendations Page 3 Organization Project/Program Description of Project/Plan Amount Requested Amount Recommending Family Care Network, Inc. General Operating Support Funds would be used for the clinical social workers and case managers and the youth/families who utilize services, along with costs related to transportation, housing, food, clothing, education, and etc. $10,000 $5,000 Food Bank No-Cook Bags for Homeless Residents Funds will- be used to purchase food for No-cook bags that will help the needs of unsheltered individuals $12,000 $6,000 Hospice of SLO County Caregiver Support Funds will be used for Caregiver Support program, which offers a variety of services to help keep home caregivers healthy and well $20,000 $10,000 Literacy for Life Literacy Program Funds will be used to cover costs of volunteer tutors, materials for clients, and other literacy activities for children, parents and families $5,000 $5,000 Long Term Care Ombudsman Services of SLO County Ombudsman Services Funds will be used to cover costs of providing homeless prevention for seniors and disabled adults residing in long term care facilities $4,000 $2,500 Lumina Alliance Emergency Shelter Operating Support Funds to support general operating costs with emphasis on emergency shelter and transitional housing program, and necessary repair costs associated with shelter and housing units $15,000 $5,000 Mission Headstrong Mission Headstrong Seed Money and Autism Personal Training Program 2 aspects to project - 1) Seed Money; and 2) Autism Personal Training certification for coaches and certify two staff for further training to future staff and other local gyms $52,500 $0 Human Services Grant Preliminary Recommendations Page 4 Organization Project/Program Description of Project/Plan Amount Requested Amount Recommending One Cool Earth Garden-based Nutrition Education Evidence-based practices to create school gardens, improve student health, academic performance and general well-being $7,000 $3,500 People's Self Help Housing Supportive Housing Program for Low - Income Households Funding for the SHP clinical social services and case management, developing wrap-around services for residents $25,000 $0 Restorative Partners Opportunity to Fund Emergecy funds for aid with rent, mandated courses, I-9 documents, and other living expenses.
