SUBJECT : FIRE DEPARTMENT REPLACEMENT OF CARDIAC MONITOR S
RECOMMENDATIO N
1.Award a sole source contract to Zoll Medical Corporation for purchase of four X Serie s
Cardiac Monitors and accessories and authorize the City Manager to execute the contrac t
totaling $128,300 .
2.Approve additional $33,700 from unallocated General Fund reserve for Cardiac Monito r
Replacement CIP .
DISCUSSIO N
Backgroun d
Cardiac Monitors/Defibrillators are indispensable and vital to the San Luis Obispo City Fir e
Department Emergency Medical Services (EMS) program . The Fire Department Paramedics utiliz e
• this important tool in conjunction with State and County protocols to deliver advanced cardiac lif e
support (ACLS) emergency medical patient care to the citizens of our community . Cardia c
Monitors/Defibrillators are utilized when a Paramedic deems it necessary to gather an
electrocardiograph assessment of a patient in order to determine the most appropriate course o f
treatment . This may entail utilizing the Cardiac Monitor/Defibrillator as an initial diagnostic tool o r
as a source of continuous information to evaluate a patient's response to interventions and therapies .
The Cardiac Monitors/Defibrillators are also essential in cardiac arrest calls for service and fo r
patients experiencing other potentially lethal cardiac rhythms which require an electrica l
intervention in order to improve their medical condition .
The request to replace four cardiac monitors was originally submitted and approved as a CIP fo r
$94,600 for FY 2012-13 in the 2011-13 Financial Plan (page 3-43 of the CIP appendix). Sinc e
submission of this CIP, the most current and advanced generation of cardiac monitors has been pu t
on the market . This model offers greater patient care capabilities and has therefore become th e
regional standard for emergency medical services . The City's medical transportation provider, Sa n
Luis Ambulance Services, now uses this model . The new X-series Cardiac Monitor has increase d
the cost of the cardiac monitors under the scope of the original CIP from $94,300 to $128,204 .
The Zoll X series models are equipped with 12 Lead and capnography (measures the patient's
oxygen level-which is the best indication of the effectiveness of the resuscitation efforts )
capabilities that are superior to the current model of cardiac monitor/defibrillator . The X-series ha s
far more advanced software capabilities that will allow first responders to detect and treat cardia c
events consistent with current policies and protocols, thus improving patient survivability . The X-
series is also equipped for data collection and transmission as per County policies and protocols .
They also have additional life saving features such as carbon dioxide detection, non-invasive bloo d

pressure monitoring, and Real Help CPR software . Real Help CPR software shows the underlyin g
cardiac rhythm during CPR, no longer needing to stop CPR to "read" the rhythm . Real Help CPR i s
not consistent throughout current equipment . The batteries of the new X-series model are fa r
superior and have a much longer life span than the battery system in current cardiac monitors .
Reasons to Sole Source to Zoll Medical Corporation :
Product Standardization-San Luis Obispo City Fire Department currently utilizes Zoll Cardia c
Monitors/Defibrillators exclusively on all Emergency Apparatus . San Luis Ambulance Services
Inc ., which is the private advanced life support ambulance company in San Luis Obispo, als o
utilizes Zoll Cardiac Monitors/Defibrillators exclusively on its emergency response apparatus .
Product standardization facilitates better interoperability and integration of patient care personne l
and equipment. Product stabilization also facilitates a more timely and consistent transition i n
patient care . San Luis Ambulance Services Inc . and CalFire Paramedics utilize "Zoll-X series "
brand cardiac monitor/defibrillators as well .
•Product Compatibility-Zoll Cardiac Monitors/Defibrillators are compatible with product s
currently being utilized by the San Luis Obispo City Fire Depa tment EMS Program . San Lui s
Obispo paramedic-firefighters are already trained to use Zoll cardiac monitor/defibrillators . A s
such, the learning curve for a new model with consistent design is much shortened . This aspec t
of compatibility decreases training needs and expenses, while simultaneously increasing
proficiency .
•Product Superiority-Following extensive usage in the EMS field, Firefighters and Emergenc y
Medical Personnel have found Zoll Cardiac Monitors/Defibrillators to be a highly reliabl e
product when caring for the most critically ill patients . Zoll produces Cardia c
Monitors/Defibrillators which are reliable, durable, user friendly and require minima l
maintenance . The Zoll X Series cardiac monitor's new design weighs less than 12 pounds ,
which is about half the size and weight of its competitors . Since cardiac monitor/defibrillators
are carried by firefighters to the patient's side, along with several other boxes of emergenc y
medical equipment, including other diagnostic equipment, oxygen bottles and tubing, and a bo x
of drugs, decreasing the size and weight of this equipment is important . The Welch Allyn
NIBP with Sure BP and Smart Cuff utilized on the Zoll X-series is the gold standard for non-
invasive blood pressure (NIBP) monitoring . This style is the most accurate design, even whe n
used in the most challenging conditions . Having the capability of NIBP allows fire departmen t
medical personnel to apply the blood pressure cuff and rely on the X-series monitor to obtai n
blood pressure readings automatically throughout patient care . This allows the medica l
personnel who are otherwise committed to monitoring blood pressure changes to perform other
critical patient care tasks and procedures . The Zoll X Series is also the first cardiac monito r
with Wi-Fi capabilities . This feature allows for greater options in data transmission of 12 lea d
electro-cardiogram (EKG) tracings and vital sign trends to the receiving hospital prior to an d
during patient transport . Early 12 lead EKG transmission to hospitals is shown to decrease th e
time between a cardiac emergency and definitive care at a cardiac care center hospital .
Decreasing this time interval is shown to significantly impact cardiac patient outcomes . The X
series model provides more options and greater reliability for this important goal .

•Less Maintenance-Zoll Cardiac Monitors/Defibrillators include a battery support system tha t
is self-supporting and requires less maintenance . The Sure Power 6 .6Ah Li-ion battery is a
lithium ion battery which does not maintain a memory, thus requiring less maintenance an d
shorter recharging periods .
FISCAL IMPACT
This is an existing Measure Y CIP for $94,600 that is approved as part of the 2011-13 Financia l
Plan . The request for additional funding from the unallocated General Fund reserve is $33,700 . If
approved by the City Council, the expenditure will be tracked as a use of over-realized Measure Y
revenue from the 2011-12 fiscal year . Total cost of the Cardiac Monitors CIP is $128,300 .
ALTERNATIVE S
Replace three (3) Cardiac Monitors in FY 2012-13 and one (1) Cardiac Monitor in FY 2013-2014 .
This alternative is not desirable in that it delays the replacement rotation of the current older mode l
for one frontline apparatus and does not maintain the goal of standardized cardiac monitors wit h
San Luis Ambulance Services or within the Fire Department . In addition, 25% of our citizens woul d
be served by equipment that offers decreased capabilities ; this would include those patient s
receiving care from the one remaining fire engine that will utilize the "old" cardiac monitor for a n
additional year .
