Fox, BC2 Additional Narrative (#1 of 2 ): T2 arrived and was assigned fire attack and primary search by Chorro IC, FF2 Pulled 200' preconnect, FE2 established water supply, T2 and FF2 made access to structure on the alpha side of the structure, south door, to find heavy smoke and a working fire in kitchen area, Bravo/Charlie corner of the structure, upon further investigation T2 confirmed deep fryer/cooking oil fire, dry chemical fire extinguisher was used for fire confinement and extinguishment. Hood system and fire sprinkler system had already activated, and fire sprinkler system was flowing water, upon fire knock down, T2 checked for extension, T2 made contact with E3, E3 completed primary search and relayed all clear to Chorro IC, T2 assisted with salvage and overhaul to the affected area, T2 reported all findings and actions to PRV20 as well as restaurant operations director and chef, T2 released from incident by Chorro IC. sms A Page 11 of 12 Additional Narrative (#2 of 2 ): Fire Origin and Cause Report Rodger Maggio, IAAI-CFI March16, 2021 On Tuesday, March 16, 2021 at approximately 11:07 AM I responded from my office to a report of a structure fire at Luna Red restaurant, 1023 Chorro Street. Upon arrival I observed fire department personnel extinguishing a kitchen fire with extension through the roof vents and I checked in with Battalion Chief Sammy Fox, and initiated my exterior examination and photographs. Other then the smoke coming from the roof my exterior examination was unremarkable. Upon entering the building through the front door I made my way back to the kitchen where I observed that the Ansul system had been activated and discharged and below the pull station was a spent Type 'K' fire extinguisher. In the kitchen area, several fire sprinklers had also activated. Along the back wall was the cookline, on the left was the griddle. On the right of the griddle was a charbroiler and to the right of that was the Pitco deepfat fryer. The fryer and charbroiler were separated by a stainless steel partition in lieu of the required 16 inches of physical distance. Above the charbroiler and extending over part of the fryer was a Salamander type broiler. The indicators lead me to believe the fryer was the origin with extension into the plenum and the hood duct system. There were 2 ducts in the hood. The fire extended up both ducts with heat and flame impingement damage present on both grease extractors on the roof. I also observed that both duct outlets were within 2 feet of the property line and an evaporative cooler intake, where the California Mechanical Code requires at least 10 feet of separation. The burn damage to the ducts along with grease dripping down the walls from the hood would leave me to believe that excessive grease accumulation contributed to the spread of this fire and difficulty in extinguishment. After my scene examination, I interviewed the restaurant manager. He stated that earlier that morning he had replaced the old oil in the fryer with new oil and set the temperature at 350 degrees F, then went to the next room for a staff meeting when he noticed flames in the kitchen. The fire suppression system was activated and he attempted to use the Type 'K' extinguisher without affect. He approximated the age of the fryer at 3 years. There are 3 primary causes of fire in deep fat fryers: Failure of the thermostat/high temperature limit switch causing over heating and autoignition of the oil, excessive grease and debris in the chimney, and piloted ignition of aerosol oil from open flame. Piloted ignition was eliminated as neither the fryer or the charbroiler were actively cooking and were separated by the metal panel. The burn patterns did not indicate an origin at the bottom of the unit that would have been consistent with the debris accumulation being the cause. Based on my observations and training, it is my opinion with reasonable degree of certainty that the cause of this fire was equipment failure. The switches and thermostat were not disturbed and left for any potential examination from the insurance company. Page 12 of 12
