Remarks:
BC3 responded to a reported structure fire. While enroute, dispatch notified BC3 of power lines down in front of the
dispatched address. BC3 communicated with all responding units enroute, with all units acknowledging power lines down.
Upon arrival, BC3 established Olive IC, to find a single-story, single-family dwelling with heavy fire and smoke showing from
the Alpha and Bravo sides of the structure, with a confirmed power line blocking the street at the alpha/bravo corner of
involved structure, two cars in driveway, with no occupants standing outside.
IC declared an offensive strategy and assigned E2 to complete a 360.
IC advised all other units to approach from Lemon and Olive due to access to Alpha side of house being blocked with power
lines down.
IC declared a working incident and requested PG&E to expedite response due to power line down and blocking access.
E2 assigned rescue and primary search due to high victim profile.
E3 assigned to initiate fire attack and assist with water supply.
E3471 assigned to assist E3 with fire attack.
T1 assigned roof division for vertical ventilation.
E4 assigned Div A, with E3, E3471 working for Div A.
Due to rapid progress of fire and deteriorating roof conditions, Olive IC cancelled T1 from roof/ventilation assignment and
reassigned T1 to Div A to assist with primary search and attack.

Primary Narrative:

Due to rapid progress of fire and deteriorating roof conditions, Olive IC cancelled T1 from roof/ventilation assignment and

Battalion 6 was assigned safety.
Primary search all clear and fire knocked down were reported with Div A assigned to have secondary search completed.
Once secondary search was completed, crews transitioned into salvage, overhaul, and rehab of crews.
Fire investigator (P20) arrived on scene and investigation team began their investigation to determine cause and origin.
Crews completed overhaul and gross decon, recovered equipment and were released from incident.
Crews were able to contain fire damage to address of origin with minor radiant heat damage to neighboring structure at 1134
Olive, damaging/breaking an exterior window.

No damage occurred to 1130 Olive, rear house, however, two occupants were displaced due to not having electrical or gas
service due to damage from fire.
IC worked with displaced occupant and dispatch to attempt notification of property management and property owner.
Save card issued to displaced occupant at 1132 Olive.
IC was also unable to have building department representative respond due to early morning time of incident. P20 was
notified of inability of building dept response and red tagged 1132 Olive at request of IC.
Scene was turned over to occupant and Olive IC was terminated.
BC3 was contacted by property manager after termination of incident and advised her of displaced occupants and following
steps to occur to secure 1132 Olive. sms
Additional Narrative (#1 of 5 ):
T1 responded code 3 to reported well involved residential structure fire in FZ2. While enroute reports of power lines down, T1
acknowledged power lines down from Batt3. Upon arrival T1 was assigned vent group for vertical ventilation. T1 laddered the
roof with a 24' ext. ladder and determined roof was unsafe to access due to heavy fire thru the roof. T1 was reassigned to Div
A to assist with interior operations, T1 performed primary search/ secondary search and opening up with fire attack teams. T1
then performed gross decon of personnel and equipment and assisted with hose take up. No further actions taken, T1
released from incident and returned to quarters.

Additional Narrative (#2 of 5 ):
E2 responded code 3 from FS2 for a report of a fully involved structure fire. E2 arrived at the scene with BC3. E2 was unable to
drive in front of the residence due to arcing power lines down on the street in front of the residence. E2 drove into the
parking lot adjacent to the house. E2 noted heavy fire coming from the entire Bravo division. E2 assigned 360 condition
report. E2 firefighter pulled cross lay for transitional attack from side bravo. E2 operator secured water supply from the
hydrant. E2 performed 360. E2 found a second residence behind the involved structure and opened the door, and woke
residents advising of fire in front. E2 with condition report, then immediately assigned rescue. E4 pulled crosslay to the front
door for E2 entry. E2 assigned to work for Div. Alpha. E2 with FF2 made access into the front door and began fire attack for
rescue. E2 moved throughout the right side of the structure in zero visibility conditions for search. E2 completed knockdown
and primary search. E2 remained on scene throughout overhaul until released by command.
Additional Narrative (#3 of 5 ):
E4 responded to structure fire. Due to power lines down and acknowledging over the radio, E4 responded in from Lemon
street side. Initial actions were to grab a water supply and pull the crosslays to A side for E2 crew due to the powerline
situation. BC3 assigned E4 Captain Division A. E4 pumped two crosslays and E4 FF assisted with interior operations with T1 and
E2. After the fire had been knocked down, E4 crew members assisted with salvage, overhaul, and gross decon before being
released from the incident. No further action taken.
Additional Narrative (#4 of 5 ):
E3 responded code 3 to a report of a structure fire on Olive St, fire district 2. Upon arrival E3 was assigned to assist E2 water
supply, operator 3 assisted with that initial assignment. Power lines down in street was acknowledged. E3 and FF3 was also
given fire attack assignment from BC3 (Olive IC) via face to face.
FF3 pulled crossly from E2 which was located on side Bravo of the property (Jack/Box parking lot.) E3 initially made our way to
Bravo side driveway (from parking lot) for exterior fire attack, then proceeded to side Alpha for fire attack.
Cal Fire E23 crew met with me on side Bravo, I assigned them to assist with 2nd hose line on side Bravo/Charlie area for
exterior fire attack.
Captain 3 was then given assignment by division Alpha for a secondary 360, gas and power were shut off on my 360. Face to
face with division Alpha informing him of 360 and utilities completed, no other hazards found.
E3 crew rehabbed for 10 minutes.
Last assignment was to assist E2 with hydrant shut down and reload 4" and 1 3/4".
E3 available, first out at scene per Olive IC.

On Thursday, June 29, 2023, at approximately 3:39 AM, three fire engines, one truck, a battalion chief from the San Luis Obispo
City Fire Department along with one engine from San Luis Obispo County Fire Department were dispatched to a reported
structure fire at 1132 Olive in the City of San Luis Obispo. The first unit the first unit on scene reported an approximate 870
square foot, single-story, single-family dwelling with heavy fire and smoke showing from the Alpha and Bravo sides of the
structure, with a confirmed power line blocking the street at the alpha/bravo corner of involved structure, cars in driveway,
with no occupants standing outside. The fire was controlled in approximately 30 minutes resulting in extensive loss of the
structure, contents and two vehicles in the driveway.

I responded from my residence at approximately 4:15 AM on June 29, 2023 after receiving a text of a structure fire from San
Luis Obispo Dispatch for an investigator to respond to the scene of a structure fire at 1132 Olive. I arrived on scene at
approximately 04:20 AM and checked in with Battalion Chief Shane SKOVE who directed me to the fire, still undergoing
overhaul activities in an approximate 100 -year- old one-story wood frame single family house. I notified Bureau staff for
assistance and was able to contact Inspector Julianna HUTCHINSON who responded to assist in the investigation. While
waiting for conditions to improve so that I could enter the structure for my scene examination, I interviewed ,
the tenant of 1132 Olive. told me that he went downtown with the tenants of 1130 Olive and returned home around
12:30 AM. He went in the house to leave his jacket, then spent the night on the couch of 1130 Olive, which is the second
dwelling on the property. He did not notice any signs of fire at that time. He was awakened later that morning by firefighters
pounding on the door to evacuate them due to the fire in the front house. Captain FLATOS, who responded with Engine 2 also
stated that his crew awakened the residents of 1130 Olive to evacuate. FLATOS also stated that he was puzzled by the
presence of bricks in the kitchen, which he thought was unusual.

Fire Scene Examination
I initiated my scene examination by visualizing all sides of the fire building, starting on the street side (Side A) and moving
clockwise around the structure. Looking at the front of the home I observed fire and smoke damage at the upper portions of
the living room window and front door with charring on the front porch upper supports and roof. There was sooting at the
upper gable of the roof. On side ‘B’, the south side of the building there was a driveway with two vehicles. The vehicle closest
to the street was a late model Ram pickup that exhibit heat damage to the right front of the truck. The truck further to the
rear of the driveway was a 1992 Mazda pickup which had extensive fire damage to the cargo and passenger compartments.
The living room window on the A/B corner exhibited heat and smoke damage on the upper portion, while the wall adjacent to
the Mazda pickup showed extensive burn damage, most likely for the flame impingement of the burning vehicle. The gable
ends of the attic were burned through. There were two electrical meters on side ‘B’ with the one closest to the Mazda
destroyed and the one closest to the street having heat damage on the side closest to the Mazda. Approaching the B/C
corner there appeared to be a “shed” type porch attachment to the main structure with an entry door on side ‘C’ at the B/C
corner. Any siding that was on side ‘C’ had been consumed by the fire from the B/C corner to approximately 10 feet from the
C/D corner. The roof of the covered porch area was consumed in this area. The C/D corner had a bathroom, there was
charring above the window on side ‘C’. I observed heat damage and charring at the upper portions of a window on the ‘D’ side
of the C/D corner.
Entering the building through the front door that faced the street I observed a living room with even level of damage that
appeared to be from downward radiated heat with a line of demarcation approximately 3-4 feet above the floor. On the back
wall of the living room was a wall furnace, I did not see any obvious patterns that would lead me to believe the fire initiated
from the wall furnace. The ceiling of the living room had been pulled down by suppression activity exposing the wood studs. I
observed charring on the top side of the ceiling joist while the underside was unburned. To the right was a bedroom in the
D/C corner of the house. This room showed primarily smoke sooting. Also to the right of the living room was a bathroom and
laundry room. This also had primarily smoke damage. Behind the living room was an entry to the kitchen and another entry to
a second bedroom. Entering the bedroom, I observed a bed with the mattress completely consumed an electric radiant
heater. The dry wall was heavily damaged however the exposed wood studs had more damage from the top down. There was
an opening onto what may have been a porch that was enclosed and made into living space rendering the rescue opening
irrelevant as it no longer opened to a yard that led to a public way. In addition, the enclosure had opening that failed offing
plenty of ventilation for the fire. The ceiling of the second bedroom was for the most part non-existent. The remaining
ceiling joist were heavily charred with more damage on the top side. The damage was more prevalent toward the kitchen
wall. Above the ceiling joist I observed knob and tube wiring. I also observed evidence of cellulose insulation. In the kitchen
the damage was more severe on the side where the kitchen shared a wall with bedroom 2. The heat damage was more sever
on the wall studs from the top downward with the ceiling and roof burn-through above the wall. The refrigerator exhibited
severe heat damage and the range-top hood had collapsed down onto the stove. The range itself showed limited damage
and there were no burn indicators that would lead me to believe the fire originated from the kitchen range. I also observed a
significant number of loose bricks that had fallen on top of the stove and floor, presumedly from an abandoned chimney in
the attic space. The kitchen had an opening onto the back porch where there was and entry way. There was a water heater
closet immediately to my left as I exited the kitchen. The burn indicators and protected spaces around the water heater
indicated that the fire burned into the water heater enclosure and as such was eliminated as a credible ignition source. The
remainder of the enclosed back area showed heavy fire damage with more dame towards the top. The roof structure was
completely consumed. The outside edges of the exterior wall studs showed protected areas and were less damaged. It was
evident that the fire had vented through the roof above the bedroom/kitchen and out the back porch.
After my interior examination I inspected the electric panel boxes on side ‘B’. The 100 AMP service was installed with a City
permit in 1999. An additional service was added in 2003 to serve the residence at the rear of the property (1130 Olive). The
panel serving 1132 Olive showed 2 breakers that had tripped.

On Monday, July 3, 2023, I received a phone call from a neighbor of the subject residence who told me there have been rat
infestations due to a neighboring construction project and that the residents of 1130 and 1132 Olive told him that they could
hear rats running around in their home. That same day I contact 1130 Olive resident, told me that they
had a rat infestation in both 1130 and 1132 Olive in April of 2023 and could hear them running in the walls. When I asked him if
they had experienced any electrical issues told me that if they tried to run more than one appliance at the same
time that a breaker would trip. He stated that this occurred in both homes on the property.

Opinions and Conclusions
It is my opinion based on burn patterns, indicators, my experience and training that this fire was most likely caused by
electrical wiring/resistance heating. The Knob and Tube wiring, while cable of carrying the electrical load of 100 years ago,
typically has trouble with the loads of modern appliances. Evidence of cellulose insulation in the attic, which is prohibited by
the Electrical Code for use in areas with Knob and Tube wiring, contributes to both the build- up of resistant heating and
smoldering ignition. Rodent activity has also been known to have contributed to fires in Knob and Tube wiring due to chewing
of the insulation material. It is also my opinion that the fire originated in the general area of the attic area above the
kitchen/bedroom wall. The cause determination of this fire is accidental.
