Upon arrival E3 found heavy black smoke showing from division alpha front door and window on the first floor on a (2) story multi-unit garden style apartment complex. E3 performed size-up and hazard analysis. E3 officer gave orders to crew to stretch an attack line (cross-lay) to the front door alpha side for fire attack and secure a water source. E3 questioned bystanders for occupant status and was unable to confirm occupant(s) were clear of the structure and identified high victim profile with the objective of fire attack/knock-down and primary search. E3 officer and FF made entry for fire attack and primary search, E3 encountered heavy fire load conditions and zero visibility. E3 utilizing TIC camera was able to push interior and locate the seat of the fire located in the kitchen area and completed knock-down of the fire. E3 then transitioned to complete a primary search on division 1 and 2 with no victim found. E3 then transitioned to personnel rehab and returned for overhaul assignment. Additional Narrative (#1 of 3 ): T2 arrived and was initially assigned 360 and evacuation and exposure check on bravo side. T2 face to face with IC and was reassigned vertical vent. T2 threw 24' ladder to alpha side and Ladder 1 assisted with second ladder on opposite side of fire, Alpha side. T2 attempted to communicate with interior crews prior to opening up vent holes but unable to reach, T2 vented and had large volumes of smoke coming from two heat holes. T2 continued with two additional heat holes to assist interior crews with visibility. T2 exited roof and reassigned by Div Alpha to secondary search, T2 gave all clear on secondary search. T2 released without incident. A Page 11 of 13 Additional Narrative (#2 of 3 ): Battalion 1 responded from a previous call on Prado Rd with MCU so was delayed to arrive on scene. In route B1 requested Truck 1 to assume IC until BC1 arrival. T-1 (Pratt) assumed Southwood IC and assigned his crew to 2 out, water supply and secure utilities. Upon arrival of E21, IC assigned 21 to 2 out and re-assigned T-1 to perform and primary search of the occupants on Division 2. Upon BC 1 arrival IC was transitioned to BC with command post located at the corner of Laurel and Southwood. Truck 1 Captain was assigned to Div A with E3, E21, and T1 initially assigned to him. Engine 3 working under Div A performed fire attack with a primary search. Engine 3 encountered heavy smoke and fire with zero visibility but was able to locate the fire using TIC. Truck 1 crew was assigned a primary search on the second floor of the structure and found heavy smoke conditions on the second floor as well as high fire load. Truck 2 was assigned Vent Group and performed vertical ventilation. Engine 4 was assigned to check both exposures and perform evacuations. Crews were able to contain the fire to the apartment of origin with moderate to heavy smoke damage the Bravo Exposure. Once fire was contained crews began a secondary search and then transitioned into overhaul. Fire investigator was requested and investigation team began their investigation to determine cause and origin. Building department was notified and arrived on scene and worked with IC and Fire Investigators to red tag both 1235 and 1237 Southwood. Crews completed overhaul and both tenants and property manager were on scene and incident was secured and turned over to property manager and tenants. Engine 3 and Battalion 1 performed patrols throughout the night and the following day to check for any flare ups. Hais Additional Narrative (#3 of 3 ): SAN LUIS OBISPO FIRE DEPARTMENT FIRE INVESTIGATION REPORT Incident Number: 23-03758 Location: 1235 Southwood Dr Date: June 23, 2023 Investigator: Julianna Hutchinson, Fire Inspector II, San Luis Obispo Fire Department Assisting: Rodger Maggio, Fire Marshal, San Luis Obispo Fire and Mattie Bliss, Hazmat Coordinator, San Luis Obispo Fire Department 1) VIOLATION N/A 2) SUMMARY On Friday, June 23, 2023, at approximately 1152 hours, units from the San Luis Obispo City Fire Department responded to a report of a fire in a condo at 1235 Southwood Dr. The first in crew found heavy black smoke showing through the front door and window on the first and second stories of a 2 story multi-unit condo complex. I responded from my location off Woodside Dr near the area of the fire after hearing many different sirens and receiving the notification of a structure fire on my work cell. Upon arrival on scene, I saw suppression crews actively suppressing a fire inside a condo unit with black smoke coming out of all openings on the Southwood St side. Deputy Chief Sammy FOX signaled me over and informed me of the current fire conditions with fire damage only on the first floor and heavy smoke damage throughout the entire unit. The fire was knocked down quickly by suppression crews and vented for the investigation team. 3) SUSPECT N/A 4) WITNESS(ES) 5) EVIDENCE See Photographs 6) CONDITIONS Temperature: 68 degrees F Page 12 of 13 Humidity: 56% Wind: 3mph Conditions: Fair (https://www.wunderground.com/history/daily/us/ca/san-luis-obispo) 7) EQUIPMENT N/A 8) PROPERTY Approximately 1200 square feet, 2 story townhouse of type VB construction, no fire sprinklers 9) NARRATIVE On Friday, June 23, 2023, at approximately 1152 hours, units from the San Luis Obispo City Fire Department responded to a report of a fire in a condo at 1235 Southwood Dr. The first in crew found heavy black smoke showing through the front door and window on the first and second stories of a 2 story multi-unit condo complex. I responded from my location off Woodside Dr near the area of the fire after hearing many different sirens and receiving the notification of a structure fire on my work cell. Upon arrival on scene, I saw suppression crews actively suppressing a fire inside a condo unit with black smoke coming out of all openings on the Southwood St side. The neighboring units of each side of the fire did not appear to contribute to the fire and remained mostly undamaged. Not long after arrival I heard a call for an investigator over the radio and I notified dispatch that I was on scene. I contacted Inspector Mattie BLISS and Fire Marshal Rodger MAGGIO for investigation assistance and equipment. Deputy Chief Sammy FOX signaled me over and informed me of the current fire conditions with fire damage only on the first floor and heavy smoke damage throughout the entire unit. I tied in with Captain Jason PRATT who confirmed this information and informed of the original fire location in the kitchen and that he would notify me when the structure was safe to investigate. MAGGIO arrived to assist with the investigation and interviewed the neighbors while I took photos of the exterior. After donning my personal protective equipment, I entered the unit and took pictures of the first floor and kitchen area. Entering the unit, I noticed many items piled up with a pathway through them from the stairs to the kitchen. In the Kitchen I noticed a majority of the damage was above the right side of the cooktop and the cabinets above were mostly consumed. A V-pattern on the wall came down to the same point above the cooktop. MAGGIO and BLISS assisted in examining the area and came to the same conclusion that the right side of the cook top was the area of origin. I went to clean my boots and came back to find MAGGIO with an officer who had the tenant of the burned unit on the phone. and after notifying her of the fire she arrived approximately 20 minutes later. While waiting we went back in for another look at the area of origin because informed MAGGIO that she hadn’t used the oven for about three days and that she had a coffee pot plugged in in that area. On the counter next to the cooktop, we observed protected areas where items sat during the fire, including the coffee makers supposed location, but the V-pattern came down to the area above the stove on the back left side and did not have any indicators starting at the coffee maker. The interior of the oven was empty, and the front glass was broken while the glass top remained intact with predominantly melted plastic items on top. We were unable to determine if the stove was on or its model number or exactly what breakers may have tripped due to fire damage. When the tenant arrived MAGGIO, BLISS, and I met with her to discuss what items may have been in the kitchen at the time of the fire. informed us that the stove was purchased a few years ago and was a Sears Kenmore Induction Range with front turn knobs. She also kept medicines and a bread tray with fruit on top of the range. There was a cat crate in the backyard and when we asked about a pet,informed us that she has a cat who was kept outside while she’s at work and wouldn’t have been inside during the fire. She had left around 1100 hours for work and didn’t notice anything out of the ordinary. The neighbors MAGGIO interviewed originally wer and who were visiting from Sacramento and staying in unit Southwood. They mentioned the smell of burning food on plastic in between 0600 and 0630 hours and predominantly smelled smoke at the back of the burned unit around 1140 hours. Opinions and Conclusions With the fire patterns in the kitchen indicating the area at the range, I researched the brand on the U.S. Consumer Product Safety Commissions for any recalls. There were recalls on Kenmore electric ranges, but I was unable to confirm if this range was one of them due to the lack of a model number. I am unable to rule out electrical behind the range and was unable to tell which breakers blew as there was no labeling in the panel after the fire.
