Record Description As to the 10/27/2019 fire at PetSmart (1530 Froom Ranch) and San Luis Obispo NFIRS-1 (Fire Station 4) No. 19-05532, please provide: 1. Photos taken on 10/27/2019 depicting the back room where the fire originated - i.e., the aquatic unit that was identified iat page 11 of the Fire Report as the origin of the fire and located in the A/D corner of the building, containing: plastic cart, supplies on top of chiller comprised of an electric motors, pumps, radiator for heat exchange and transformer). 2. Confirmation as to information obtained by the Fire Department (4) from personnel who responded to the scene on10/27/2019 that while a duplex system was observed in the subject aquatic system, no GFCI was observed. 3. Confirmation from Fire personnel who responded to the scene on 10/27/2019 that given the proximity to a water source, a GFCI should have been used for the system, apart from any component part manufacturer requirement for a GFCI. 1 4. Confirmation from Fire personnel who responded to the scene on10/27/2019 that excess insulation (that is, supplies placed on top of the aquatic unit were too close to the aquatic unit), creating stress on the subject aquatic system that likely caused component part(s) failure and/or the subject fire.
