Attached, find copies of the complaint and summons served on the City 2/21/23 regarding a tripping on the sidewalk at Sinsheimer Park.   Caller stated the sidewalk near his driveway at 236 Clover Drive is raised three inches, causing a tripping hazard. He also said there is an area of brick that has sunken, causing the concrete next to it to be raised, causing a tripping hazard. It's becoming more & more dangerous, especially for those that can't walk or see well. And better lighting along the walkway would be beneficial as well. Thanks!   Potential liability hazard causing tripping/slipping located on sidewalk below open space across from 847 Patricia Drive. I have recently seen an adult slip and fall in the area, and have seen a young child on a small scooter crash in the area. In the past, debris clean up has involved scooping it up and throwing it back up the hill above the sidewalk, only to come down onto the sidewalk again. Perhaps the debris should be scooped up and hauled away or deposited where it won't come back onto the sidewalk again. Thank you.   Hi The sidewalk is lifting/sinking with a 2" height on one side. Not sure what the standard for this is but FYI. Attached image has some red arrows locating the issue. Thanks SS   Vegetation has overgrown onto the sidewalk. There are elderly residents living in an assisted living home just down the street. We walk the residents with their walkers and the vegetation doesn't allow us to fully use the sidewalk. It's a trip hazard when the resident is using their walker & a caregiver is walking next to them holding their gait belt. On Friday, one of our residents caught his walker wheel in the vegetation and almost fell. She is still currently receiving treatment for her injuries which include back, knees, ankles, hands, and wrists.   I received a call from a community member, Stacy, who was walking at Sinsheimer Park and slipped on mud / water that was coating the sidewalk to the right of the bike rack near the baseball field. She was not hurt, but she wanted to report the issue so it can be addressed before anyone else slips. She believes the water / mud is starting to dry up, so it may not be an issue now, but she wanted to report it regardless. I let her know I would submit the request, and no further follow up is needed with her.   noticed a gap/tripping hazard on the corner of Morro and higuerra.   Caller stated there is a 4-5 inch hole located in the sidewalk in front of his business at 1220 Marsh Street. Claims it is a tripping hazard. She claims that she was unable to pass with her wheelchair due to unmaintained pavement and cracks on the sidewalk surface.   Email from resident: Hi there. A neighbor gave me your department to report some bad sidewalks. They are located on both houses to the side of the house located at 867 mission st. Thank you for your attention. We have tripped many a time here.   on the intersection of chorro and higuera there is cla cover missing for a gas hole. definitely a tripping hazard and other potential risk.   Caller stated the sidewalk in front of 492 Mitchell Drive is raised about one inch, due to the street tree lifting the sidewalk. Caller is Catherine   The sidewalk and curb have been lifted/damaged by street tree roots. This is a tripping hazard. Temporary asphalt has been placed by the Streets Division. Please remove and replace, making a permanent repair with concrete. Trip Hazard - Resident reported that the sidewalk has a rectangular area that has risen and it is now a tripping hazard. She said the city has attempted to repave and fixed it in the past few years. The neighbor tripped yesterday and she has also tripped a few times already. You can feel free to call her to determine where the issue is located and to inform her the status of her request.   There has been construction of a second ADU at 425 Luneta where there's a huge uplift in the sidewalk (about one foot of uplift). I thought when the city issued any substantial permit that it required repair of such things in the public right of way, yet the construction appears near completion and no sign of dealing with this issue (though they poured some new sidewalk for a hole they made in the old one -- visible at top of photo). So how do we get this major right of way hazard fixed?   Trip Hazard - Missing utility box lid on utility box which is adjacent to sidewalk. trash bin was placed over open hole around 10:30 to prevent fall injury. Note - box is on Galleon side of address.   When are you going to replace the grate that was around the tree in front of 781 Higuera Street? Multiple people have fallen and/or twisted their ankle because there is no metal grate. You replaced it with DG that was creating a mess all over the sidewalk that we had to keep cleaning and now it's been taken out and is now a tripping hazard. please replace the metal grate asap before any more people get hurt. Someone may end up suing the city over it. Thank you.   the sidewalk/bike path has cracked significantly during the storm, above/next to the bridge on Madonna Road, adjacent to the lake.   There is about a 4-5 inch drop in the sidewalk with no warning. This is a huge trip and fall hazard!   This big missing chunk in the sidewalk/curb poses a serious trip and fall risk.   There are several tree basins that drop down 6-8 inches from missing mulch or other infill. This poses a serious safety risk in the public right of way!   Multiple brick pavers have sunken down and pose a trip hazard.   grate missing, causing significant trip hazard.   trip hazard where child crashed bike   540 Mitchell Drive needs repair to 2 spots 1) sidewalk - see photo and 2) concrete at sidewalk level that is above storm drain. I don't have a pic of #2 but once you locate the storm drain you can't miss it.   Sidewalk and driveway approach is lifted/damaged due to a street tree. Please remove and replace the sidewalk and driveway approach. There is a very bad sidewalk issue in front of this building on Mill Street in San Luis Obispo. The seam between sidewalk segments is lifted up quite badly. There is also a pot hole in the middle of two segments. I tripped on this on 1/29/23 and hurt both my knee and wrists. I am fortunate that I did not break one or both wrists or fracture my knee.   There is a broken fiber optic cover on the sidewalk on the northern side of Orcutt Road between McMillan Ave and Duncan Road. No mention of trip hazard at this time.   Email from resident: the blue metal bus stop bench on Ramona near Tassajara has again disappeared, and its four bolts now stick up in the sidewalk constituting a dangerous trip and fall. Some thoughtful citizen has placed rocks next to the bolts to make their danger more apparent. You may recall this has happened before and last time the city replaced the bench. I am asking that the city remove the bolts permanently and not place another bench ON the sidewalk at that location. Why? Because the bench + legs & feet of a user narrows the sidewalk to less than the minimum required width for passage per the various ADA design guidelines. That narrowing would cause any pedestrian to have to step off the curb and into the street, which is a total non-starter for someone with wheels. I remain amazed that this sort of thing isn't understood by the city and incorporated into day-to-day practice but must be brought to the city's attention case by case by persons like me. If the city deems there is need for a bench at this bus stop, I respectfully request it be placed behind the sidewalk so it does not narrow the sidewalk.   The issues below have been needing help for the past few years (at least) 1: Not sure about the Men's but the Women's restroom at the stadium needs repair. The handicap stall on the left side is out of order and the first 2 or 3 stalls on the right side usually have an inch or two of water that also is in the area in front of the stalls. 2: The walkway to the stadium really needs to be repaved and better lit. It's especially dangerous at night if one has any kind of disability. 3. Pest control in the grass above the seats and along the walkway/grass border directly behind the top row of seats in the stadium.
