Attach The Business License Supplement
In addition to the Business License Application, fill out
the Business License Supplement form and attach it to
your completed application. The supplement is used to
insure that your business meets city planning require-
ments and that necessary permits have been applied for.
Pay The Fees
The completed Business License Application and the
attached Business License Supplement should be return-
ed to the City's Finance Department. The cashier's win-
dow is open weekdays from 8:00 am to 5 00 pm.
Business license fees are determined by the type and
location of your business. If you submit your application
in person, your fees will be paid at that time. Your
business license certificate will be mailed to you.
If necessary, you may mail us your completed applica-
tion and supplement. However, it may take longer to get
your business license due to the additional paperwork and
mailing time.
If other permits are required (such as a use permit,
sign permit or health permit), you must deal directly with
the appropriate department or agency. The Finance
Department only collects fees and Issues your business
license tax certificate.
What Happens Next?
Your business license is valid as long as there are no
significant changes to your business. You must notify us
if there are any changes to your business mailing
address, location, ownership, or types of goods or ser-
vices you offer.
Your business license must be renewed each year.
Renewal forms are mailed annually.

1. Hazardous connections @ terminals in service disconnect behind Unit #2.
2. Fuses oversized @ Unit #3 (unless there is a second panel inside the unit
(possibly a general violation).
3. Direct burial wire exposed at grade (improperly installed). (Unit #6)
4. NMC run exposed to damage outside Unit MI6.
5. Uncovered openings in dead fronts of panels @ individual units.
6. New panels have been mounted on wood boxes which conceal the old panel
boxes making them unaccessible.
7. "Homemade" wood panel box in laundry room.
8. There is no grounding @ individual buildings.
9. The electrical drops are outside the minimum clearances permitted in NEC
Section 225-19, exceptions (1)[3' if roof not less than 4:12] and (2)
[18": if voltage is less than 300, cross no more than 4', and terminate
in an approved manner.]
NOTE: This was a cursory inspection. No attempt was made to gain access to
the units or to list all probable violations.
Generally, it appears that a certain amount of electrical "house clean-
ing" is necessary. items 1, 2, 3 and 7 should be given priority
attention. If the Fire Department wants to pursue this, I would
recommend they make arrangements with this Division and the owner to
make a more thorough inspection, including inside the units. Perhaps
then it would be wise to ask the owner to submit a schedule for repairs
and corrections.

GENERAL PROVISIONS
1. It is understood and agreed by the Permittee that the issuance of the permit shall
constitute the acceptance of the provisions hereon.
2. This permit shall be posted on the job site in a conspicuous place and easily ac-
cessible.
3. All work shall be done in a thorough and workmanship -like manner. Any work not
so done shall be corrected by the Permittee at his expense at the request of the
Engineer.
4. The Permittee shall be responsible for preserving all stakes unless authorized to
remove such stakes by the Engineer.
b. This permit shall become null and void if the work is not commenced within sixty
(60) days from date of issue, or within sixty (60) days of notification that grades
are ready and completed within thirty (30) days thereafter.
6. The Permittee shall remove at his own expense all obstructions such as trees,
stones, debris, roots, etc., that are encountered in prosecution of the work.
7. The Permittee shall safeguard the public at all times during the course of the
work by the erection of barricades, lights and by-pass walkways subject to direc-
tions of the Engineer. The Permittee is responsible for all liability for personal
injury or property damage which may arise out of work herein permitted or which
may arise out of failure on the Permittee's part to perform his obligations under
this permit in respect to construction and maintenance. In the event any claim of
such liability is made against the City of San Luis Obispo, or any department,
officer, or employee thereof, Permittee shall defend, indemnify and hold them and
each of them harmless from such claim.
8. Any underground utilities damaged during the course of the work shall be the re-
sponsibility of the Permittee.
9. In case it should be necessary to move the property of any owner of a public utility
or franchise, such owner will, upon proper application by the Permittee, be notified
by the City Engineer to move such property within a specified reasonable time, and
the Permittee shall not interfere with said property until after the expiration of
the time specified.
10. The right is reserved to the owners of public utilities and franchises to enter upon
the street for the purpose of malting repairs or changes of their property that may
become necessary by the work. The City shall also have the privilege of entering
upon the street for the purpose of repairing sewers, or making house drain con-
nections therewith, or repairing culverts or storm drains.
11. Before pouring concrete the forms are to be inspected and approved. Request for
inspection must be made four (4) hours in advance.
12. The Permittee shall remove all surplus material and rubbish from the work and
fill to grade with approved material all holes and depressions in adjacent areas.
13. For further provisions, attention is called to the Standard Specifications for Con-
crete Driveway, Sidewalk and Combination Curb and Gutter.

GENERAL PROVISIONS
1. It is understood and agreed by the Permittee that the doing of any work un-
der this permit shall constitute an acceptance of the provisions.
2. Before starting work the Permittee shall notify the City Engineer, or his au-
thorized agents. Such notice shall be given at least one day in advance of the
date worx is to begin.
3. 'Phis permit shall be kept at the site of the work and must be shown to any
representative of the City Engineering Department or any law enforeemeat of-
ficer on demand.
4. All work shall be executed in every respect in a thorough and workmanlike
manner. The contractor shall preserve al. stakes set for the lines, levels or meas-
urements of the work in their proper places until authorized to remove them by
the Engineer, and any expense incurred in replacing said stakes which the con-
tractor or his subordinates may have failed to preserve shall be borne by the
contractor.
b. The contractor shall give forty-eight (48) hours notice in writing, when he
will require the services of the Engineer for laying out any portion of the work.
He shall dig all stake holes necessary to give lines and levels.
G. The contractor shall not disturb any monuments or stakes found on the lines
of the improvement until ordered by the Engineer, and lie shall bear the expense
of re -setting any monuments or stakes which may be disturbed without orders.
7. The contractor shall remove at his own expense all obstructions such as trees,
st,,nes, debris, roots, etc., that may be in the way of making the proposed im-
provements.
8. No material shall be stored to co%er more than one-third the width of the
street. The contractor shall observe all the regulations of the City of San Luis
Obispo, in relation to the obstruction of the streets, keeping open passageways,
and protecting the same when they are exposed and would be dangerous to pub-
lic travel. lie shall also erect and keep erected, by day and night, a fence or prop-
er barrier along the line of work and across the ends of the same, in order to
gi:ard the public effectively from hanger of falling into trenches, or from up-
setting their vehicles against the cr.rth thrown up during the progress of the
work, and he shall post all proper notices and signals to the public of the state
of the street while the work is in progress. No more than one cross street shall
be closei at one time.
9. The Permittee is responsible for all liability for personal injury or property
damage which may arise out of work herein permitted, or which may arise out
of failure on the Permitt:ee's part to perform his obligations under this permit in
respect to maintenance. In the event any claim of such liability is made against
the City of San LAi Obispo, or any department, officer, or employee thereof,
permittee shall defend, �naemnify and hold them and each of them harmless from
such claim.
10. Any sewer connection or water pipes that may be broken by the contractor
yr his agents shall be replaced by said contractor at his own expense.
11. A red light must be maintained at each end of barriers from sunset until
sunrise, and a watchman employed as additional security whenever the same may
be needed.
12. In case it should be necessary to move the property o!: any owner of a pub-
lic utility or franchise, such owner will, upon proper application by the contract-
or, be notified by the City Engineer to move such property within a specified
m asonable time, ani the contractor shall not interfere with said property until
after the expiration of the time specified.
13. The right is reserved to the owners of public utilities and franchises to enter
upon the street for the purpose of making repairs or changes of their property
that may betonse necessary by the work. The City shall also have the privilege
of entering upon the street for the purpose of repairing sewers, or making house
drain connections therewith, or repairing culverts or storm drains.
14. The contractor shall remove all surplus material and rubbish from the work
after its ccmpletion, and before he makes application for the acceptance of the
work.
15. The contractor shall notify the City Engineer when he desires a final inspec-
tion of the work, when the latter will, as soon as possible make the necessary
examination, and if the work is found in compliance with the above soecif cations,
the City Engineer will furnish the contractor with a certificate to that effect.
16. For further provisions attention is called to the Standard Specifications for
Portland Cement Concrete Sidewalk and Combination Curb and Gutter.
