Truck 2 requested additional units and Engine 3 and Battalion 1 were dispatched. Upon arrival I was briefed by Truck 2 crew Captain Gutierrez and I agree with his narrative in this report. I assumed command from Truck 2 and continued to isolate and deny entry into the hazard area. The cafeteria of the hospital was evacuated and the gas co worked on mitigating the hazard. The incident was turned back over to Captain Gutierrez on Truck 2 after it was deemed safe to do so. Truck 2 requested additional units and Engine 3 and Battalion 1 were dispatched. Upon arrival I was briefed by Truck 2 crew Captain Gutierrez and I agree with his narrative in this report. I assumed command from Truck 2 and continued to isolate and deny entry into the hazard area. The cafeteria of the hospital was evacuated and the gas co worked on mitigating the hazard. The incident was turned back over to Captain Gutierrez on Truck 2 after it was deemed safe to do so. BC Hais Additional Narrative (#1 of 1 ): T2 arrived to find Gas Co and Building rep from SVH outside cafeteria area stating they were getting readings of 60% gas leaked from ground. T2 request 1 additional unit and BC1. T2 ordered an evacuation of all cafeteria staff, kitchen and cafeteria area. T2 make contact with SVH COO Eleze to nofity her of the potential of needing to evacuate the hospital if the situation escalated. T2 coordinated with Gas Co Rep, Tyler to ensure we secured an area that was appropriate for the current status of the gas leak. T2 initiate isolation of the immediate area and assigned E3 to assist block the secondary access point. Upon arrival of BC1, command was transferred and he assumed Murray IC. T2 assigned Div Alpha and isolated and denied area to hot zone. Command given back to T2 after hazard was mitigated; T2 stood by until readings were below LEL and Gas % levels for the evacuated areas to be populated, per Gas Co rep Rhy's determination per their policy. COO Eleze notified of the lifted evacuation order. T2 turned scene back over to SVH and Gas Co. A Page 6 of 6
