Dispatched to: end of Granada Drive behind Farm Supply off Tank Farm for flames seen.
Next report indicated possibility of an airplane down into the building at Promega (277
Granada). Cal Fire was sending their response for an aircraft down (alert 3, category 1).
Based on these reports I requesting a working incident response and asked for additional
PD units and a Watch Commander (Chad Pfarr) to respond to facilitate evacuation. Other
initial requests included C-1, C-2, and Haz Mat Inspector Kerry Boyle.
Conditions found: E-21 arrived on scene first to find a small aircraft down on the B side
of 277 Granada with fire involving the aircraft, a Fed-Ex delivery truck, and the roof of
the building. The plane had flown thru power lines, clipped two trees, and then crashed
into the rear B side of 277 Granada. E-21 advised one fatality (the pilot of the plane) and
two people unaccounted for. Both of the two unaccounted were later located (one had left
work for lunch without his cell phone). The driver of the Fed-Ex truck (Serafin Armenta,
805-354-4609) was inside Promega making a delivery when the crash occurred, he was
uninjured.
Actions taken: E-21 gave a condition report and established command. I arrived and
assumed command as Granada IC. E-21 was assigned Fire Attack, E-3 was assigned
Water Supply, E-2 was assigned RIC and Utilities, and E-4 was assigned to E-21 (Fire
Attack) to assist with FA and structure protection. T-1 was assigned to E-21 (FA) and to
set up the aerial and to ladder the roof of 277 Granada to check for extension. E-21
indicated they had a water supply available for themselves and T-1.
Due to a liquid spill in the interior corner B/C portion of the 277 Granada facility the
CHIRT (Haz Mat Team) was requested and responded. Cal Fire BC Paul Lee arrived and
was assigned Haz Mat Group Sup. Also assigned to the HM Group were Nate Bass, Tom
Bass, Kerry Boyle, and Reynolds from County Environmental Health. C-1 arrived and
was assigned PIO. C-2 arrived and was assigned Safety/Accountability. This function
was transferred to Cal Fire Chief 3402 (Eric Cleveland) and an E-21 FF. Fire Marshall

Maggio arrived and was assigned Staging Area Manager. Crash-21, Rescue-21, WT-21,
and SLAS Adam 3 and two SLAS paramedic units were also assigned to Staging. Haz
Mat-1 and Decon-1 were requested and arrived. City Water Dept rep Noah Evans arrived
at the Command Post, as well as the PD Watch Commander Pfarr, CHP, PG & E, Red
Cross (Cheryl), SLO County Sheriff/Coroner (Sheri Kessler), and Matthew Hill, Aviation
Safety Inspector from the FAA (408-291-7681). Dan Goulet from City Utilities arrived
later, as well as a person from County Environmental Health (Reynolds).
Follow up: An NTSB investigator arrived later in the
evening and took over the investigation of the cause of the crash. All units were
eventually released with the exception of SLOPD and SLO County Sheriff's Department
personnel. PG & E was finishing up repair work to the power lines.
The plane was identified by its tail id series (N337LJ) as a Cessna 337, recently
purchased and owned by through his
company.
The Fed-Ex truck was unloaded into another Fed-Ex truck, which arrived on scene later
in the incident.
Haz Mat Inspector Boyle initiated a Cal EMA Spill Report (# 13-3844) by phone for the
fuel spill from the aircraft./
