T1 established Promega IC and described a one story industrial building with no smoke showing. E4 was assigned to investigate. This facility is known to have multiple different hazardous materials. I assumed IC and established the command post across from Promega on side A. T1 reported that the fire was confirmed to be out and was bringing two employees to the command post. The employees, are scientists at Promega and they were in the room when the fire started. They stated that they were pouring approximately 60 liters of Methanol into a container when the methanol ignited. They reported that approximately 30 liters were spilled on the floor which also ignited. Employees used extinguishers to try to put out the fire and the foam sprinkler system activated. An employee pulled the fire alarm and our fire department responded. This facility has a back up system for air handling that was still in operation and left in operation for the other chemicals/mixtures in the room. This facility also has a secondary containment system that collected all the run off from the sprinkler system so no materials/chemicals affected the environment. stated that the fire was out, the electricity to the building was turned off, but there was a mixture of Acetonitile and Trifloro Aceta Anhydride that was in another ventilated area, adjacent to the fire area, that needed to be covered to... They wanted to re-enter the building to secure the mixture. The Cal OES Warning Center was notified of the spill and assigned Contact # 20-00111. SLO City Dispatch was contacted to make notifications to County Environmental Health, City Streets, City Biologist, and the Fire Chief. Captain Larson was made Haz Mat Group Supervisor. He and I discussed the options and deemed the re-entry of employees too high of a risk. The County Haz Mat Team was requested to assess the situation and assist with re-enty of employees to secure the mixture. When the County Haz Mat Team members arrived, Nate Bass was Haz Mat IC, Jordan Streeter was assigned Haz Mat Safety, FF Barnhart was assigned Entry Leader, Eric Carrieno was assigned Decon Unit Leader, and Kerry Boyle worked with and as Technical Specialist. County Environmental Health Officer, Mary Bliss, arrived on scene and was assigned Environmental Health. A safety briefing was led by Haz Mat Group Supervisor Larson and the plan was put in place to make entry. Prior to entry the County Haz Mat Team was able to view air monitoring systems inside the building of origin and all air system checks remained normal. The Haz Mat Team made entry with the goal of investigating the conditions, monitoring air and explosive limits, and ensuring the conditions were safe to allow the employees inside to secure the mixture of Acetonitile and Trifloro Aceta Anhydride. The conditions were deemed safe by the entry team and were escorted in to building, secured the mixture, and exited the building without incident. The building was turned over to the owner with Haz Mat Specialist Kerry Boyle assisting with safety of the clean up. Fire Marshal Maggio was placed in contact with the building owner and he conducted his fire investigation. A debriefing was held and all units were released. Additional Narrative (#1 of 4 ): E-3 arrived on scene and staged. E-3 was brought to the Command Post location and Captain Larson was given the assignment of Hazardous Materials Group Supervisor. E-3 established appropriate ICS positions as outlined in the FOG Manual. E-3 consulted with HazMat team and administrative personnel and made the decision to do a recon entry to determine any smoke, fire or chemical hazards that may be present. HazMat entry was made utilizing two personnel in turnout PPE with Decon 1 established as our primary Decontamination Unit. No abnormal atmospheric air readings were reported from the remote air monitoring station or the chemical/gas detection devices took in by the Entry Team. Once the determination was made that the fire was out and that the chemical in question were contained, Promega Response personnel were allowed to enter utilizing their proper PPE and respiratory protection to secure the remaining chemical that were continuously contained. All objectives were competed within our allotted timeframe and using one entry only. A Page 14 of 1 Additional Narrative (#2 of 4 ): Fire Origin and Cause: January 7, 2020 On Tuesday, January 7, 2020 at approximately 2:15 PM, I was contacted by the San Luis Obispo Fire Dispatch to respond to investigate a fire at 277 Granada, San Luis Obispo (Promega Bioscience). I arrived on scene and met with Battallion Chief Neal BERRYMAN and Haz-Mat Coordinator Kerry BOYLE. I was tod there was an extinguished fire in the ventilation hood area of a process room in the 28,000 sf building and the the environment was safe to enter. I entered the building with Promega representative . led me in to a ventilation hood area where I observed a fractured glass reaction vessel adjacent to a plastic container that was melted to the liquid line. Above and to the right of the melted plastic vessel I observed a burned flexible ventilation hose. Also observed was one fire sprinkler head in the hood that appeared to have activated. Examining the electrical equipment in the hood area, it all appeared to be Class 1, Division 1, typical where flammable vapors are present. Speaking with he explained that an employees was transferring methanol from the glass vessel into the plastic filtration container when the material ignited and he dropped the glass vessel and evacuated the building. Conclusion: I observed no competent ignition sources in the area of origin, the ventilation was active during the transfer operation. The only competent ignition source, in my opinion would be a static electrical discharge. This opinion is founded based on the use of a plastic container, which should not be used for the transfer of flammable liquids, the lack of bonding between the two vessels, and the lack of grounding. Flammable liquids, free falling through air have a potential to accumulate a static charge. Cause: Accidental. Additional Narrative (#3 of 4 ): Upon arrival E4 personnel met with Promega personnel and was led to the affected building room. E4 personnel observed the affected room and saw no fire or smoke, the sprinkler system was actively flowing into the affected room. E4 personnel pulled back and met up with the IC at the CP for information sharing on our observations and findings. E4 was assigned to Decon 1 and was instructed to provide a booster line for Decon operations. Additional Narrative (#4 of 4 ): Truck 1 arrived on scene established command and advised that there was nothing showing but we would be investigating. Truck 1 met with E4 on the B side of the structure. We were met there by two employees who had been working in the area of where the fire started. They told us the fire was out. I advised the BC who had assumed command on his arrival that the fire was out and I would be bringing two employees to the ICP to advise him on what had occurred. Truck 1 brought the employees over to the ICP and then was made available at scene by the IC. T1 stayed at scene until the incident was mitigated and then was released. Page 1 of 1
