Upon seeing ignition approx 12'-20' away,tossed a wood pallet on fire of dried grass in an attempt to "smother" the fire. After attempt failed, he summoned 911. SGonzalez BC4 Additional Narrative (#1 of 2 ): T-2 arrived after E-21 and was attempting to establish command when BC-4 arrived and took command. T-2 was assigned 360 and assist E-21 with water and man power. T-2 completed a 360 and reported slow rate of spread, approx 1/4 acre. T-2 then coordinated with the business owner to use his equipment to move the power poles for complete extinguishment. T-2 completed the assignment and turned over to the business owner. Additional Narrative (#2 of 2 ): E3 arrived at scene and directed by IC to have engineer start water shuttle operation to support T2 and E21. E3 crew used hand tools to cut perimeter line, and work on overhaul of burning power poles. E3 remained at scene working for IC until command was terminated. T1 established Promega IC and described a one story industrial building with no smoke showing. E4 was assigned to investigate. This facility is known to have multiple different hazardous materials. I assumed IC and established the command post across from Promega on side A. T1 reported that the fire was confirmed to be out and was bringing two employees to the command post. The employees, are scientists at Promega and they were in the room when the fire started. They stated that they were pouring approximately 60 liters of Methanol into a container when the methanol ignited. They reported that approximately 30 liters were spilled on the floor which also ignited. Employees used extinguishers to try to put out the fire and the foam sprinkler system activated. An employee pulled the fire alarm and our fire department responded. This facility has a back up system for air handling that was still in operation and left in operation for the other chemicals/mixtures in the room. T1 established Promega IC and described a one story industrial building with no smoke showing. E4 was assigned to investigate. This facility is known to have multiple different hazardous materials. I assumed IC and established the command post across from Promega on side A. T1 reported that the fire was confirmed to be out and was bringing two employees to the command post. The employees , are scientists at Promega and they were in the room when the fire started. They stated that they were pouring approximately 60 liters of Methanol into a container when the methanol ignited. They reported that approximately 30 liters were spilled on the floor which also ignited. Employees used extinguishers to try to put out the fire and the foam sprinkler system activated. An employee pulled the fire alarm and our fire department responded. This facility has a back up system for air handling that was still in operation and left in operation for the other chemicals/mixtures in the room. This facility also has a secondary containment system that collected all the run off from the sprinkler system so no materials/chemicals affected the environment. stated that the fire was out, the electricity to the building was turned off, but there was a mixture of Acetonitile and Trifloro Aceta Anhydride that was in another ventilated area, adjacent to the fire area, that needed to be covered to ensure it was safe. They wanted to re-enter the building to secure the mixture. The Cal OES Warning Center was notified of the spill and assigned Contact # 20-00111. SLO City Dispatch was contacted to make notifications to County Environmental Health, City Streets, City Biologist, and the Fire Chief. Captain Larson was made Haz Mat Group Supervisor. He and I discussed the options and deemed the re-entry of employees too high of a risk. The County Haz Mat Team was requested to assess the situation and assist with re-enty of employees to secure the mixture. When the County Haz Mat Team members arrived, Nate Bass was Haz Mat IC, Jordan Streeter was assigned Haz Mat Safety, FF Barnhart was assigned Entry Leader, Eric Carrieno was assigned Decon Unit Leader, and Kerry Boyle worked with and as Technical Specialist. County Environmental Health Officer, Mary Bliss, arrived on scene and was assigned Environmental Health. A safety briefing was led by Haz Mat Group Supervisor Larson and the plan was put in place to make entry. Prior to entry the County Haz Mat Team was able to view air monitoring systems inside the building of origin and all air system checks remained normal. The Haz Mat Team made entry with the goal of investigating the conditions, monitoring air and explosive limits, and ensuring the conditions were safe to allow the employees inside to secure the mixture of Acetonitile and Trifloro Aceta Anhydride. The conditions were deemed safe by the entry team and were escorted in to building, secured the mixture, and exited the building without incident. The building was turned over to the owner with Haz Mat Specialist Kerry Boyle assisting with safety of the clean up. Fire Marshal Maggio was placed in contact with the building owner and he conducted his fire investigation. A debriefing was held and all units were released. Additional Narrative (#1 of 4 ): E-3 arrived on scene and staged. E-3 was brought to the Command Post location and Captain Larson was given the assignment of Hazardous Materials Group Supervisor. E-3 established appropriate ICS positions as outlined in the FOG Manual. E-3 consulted with HazMat team and administrative personnel and made the decision to do a recon entry to determine any smoke, fire or chemical hazards that may be present. HazMat entry was made utilizing two personnel in turnout PPE with Decon 1 established as our primary Decontamination Unit. No abnormal atmospheric air readings were reported from the remote air monitoring station or the chemical/gas detection devices took in by the Entry Team. Once the determination was made that the fire was out and that the chemical in question were contained, Promega Response personnel were allowed to enter utilizing their proper PPE and respiratory protection to secure the remaining chemical that were continuously contained. All objectives were competed within our allotted timeframe and using one entry only. A Page 14 of 15 Additional Narrative (#2 of 4 ): Fire Origin and Cause: January 7, 2020 On Tuesday, January 7, 2020 at approximately 2:15 PM, I was contacted by the San Luis Obispo Fire Dispatch to respond to investigate a fire at 277 Granada, San Luis Obispo (Promega Bioscience). I arrived on scene and met with Battallion Chief Neal BERRYMAN and Haz-Mat Coordinator Kerry BOYLE. I was tod there was an extinguished fire in the ventilation hood area of a process room in the 28,000 sf building and the the environment was safe to enter. I entered the building with Promega representative . led me in to a ventilation hood area where I observed a fractured glass reaction vessel adjacent to a plastic container that was melted to the liquid line. Above and to the right of the melted plastic vessel I observed a burned flexible ventilation hose. Also observed was one fire sprinkler head in the hood that appeared to have activated. Examining the electrical equipment in the hood area, it all appeared to be Class 1, Division 1, typical where flammable vapors are present. Speaking with he explained that an employees was transferring methanol from the glass vessel into the plastic filtration container when the material ignited and he dropped the glass vessel and evacuated the building. Conclusion: I observed no competent ignition sources in the area of origin, the ventilation was active during the transfer operation. The only competent ignition source, in my opinion would be a static electrical discharge. This opinion is founded based on the use of a plastic container, which should not be used for the transfer of flammable liquids, the lack of bonding between the two vessels, and the lack of grounding. Flammable liquids, free falling through air have a potential to accumulate a static charge. Cause: Accidental. Additional Narrative (#3 of 4 ): Upon arrival E4 personnel met with Promega personnel and was led to the affected building room. E4 personnel observed the affected room and saw no fire or smoke, the sprinkler system was actively flowing into the affected room. E4 personnel pulled back and met up with the IC at the CP for information sharing on our observations and findings. E4 was assigned to Decon 1 and was instructed to provide a booster line for Decon operations. Additional Narrative (#4 of 4 ): Truck 1 arrived on scene established command and advised that there was nothing showing but we would be investigating. Truck 1 met with E4 on the B side of the structure. We were met there by two employees who had been working in the area of where the fire started. They told us the fire was out. I advised the BC who had assumed command on his arrival that the fire was out and I would be bringing two employees to the ICP to advise him on what had occurred. Truck 1 brought the employees over to the ICP and then was made available at scene by the IC. T1 stayed at scene until the incident was mitigated and then was released. On arrival BC3 assumed Sueldo IC and was in investigative mode. Bystanders from adjacent unit stated smelling smoke odor and light smoke from unit R. No RP on scene. E4 was asked by IC to conduct 360 and report back findings. All other units responding remained in standby staged. E4 personnel gained access to knox box and were unsuccessful finding a key to that unit. E4 forced entry on front door of unit with k-tool. E4 found a small smoldering fire in the garage area of the unit approximately 2-3 square feet in total. E4 relayed to Sueldo IC hazard could be mitigated by E4 personnel and all other units on scene were not needed. Sueldo IC cancelled all other units. BC3 turned the incident over and E4 assumed IC. E4 found a dust mop, battery charger, melted 5 gallon bucket, and three small sections of drywall that was damaged due to the fire. The fire did not extend from this area and a Thermal imaging camera was used to look for any extension in the walls. A small section of drywall 6''X6'' was removed to extinguish the smoldering fire. E4 utilized a electric fan throughout the unit to remove any remaining smoke and odor from the unit. The responsible ( arrived on scene and was made aware of the situation. On arrival BC3 assumed Sueldo IC and was in investigative mode. Bystanders from adjacent unit stated smelling smoke odor and light smoke from unit R. No RP on scene. E4 was asked by IC to conduct 360 and report back findings. All other units responding remained in standby staged. E4 personnel gained access to knox box and were unsuccessful finding a key to that unit. E4 forced entry on front door of unit with k-tool. E4 found a small smoldering fire in the garage area of the unit approximately 2-3 square feet in total. E4 relayed to Sueldo IC hazard could be mitigated by E4 personnel and all other units on scene were not needed. Sueldo IC cancelled all other units. BC3 turned the incident over and E4 assumed IC. E4 found a dust mop, battery charger, melted 5 gallon bucket, and three small sections of drywall that was damaged due to the fire. The fire did not extend from this area and a Thermal imaging camera was used to look for any extension in the walls. A small section of drywall 6''X6'' was removed to extinguish the smoldering fire. E4 utilized a electric fan throughout the unit to remove any remaining smoke and odor from the unit. The responsible ( arrived on scene and was made aware of the situation. He stated he believed the materials involved in the fire were detailing equipment for vehicles. E4 stated we were able to force the front door locking mechanism to gain entry and that there was not a key for his unit in the knox box. No other assistance was needed and E4 terminated Sueldo IC and cleared incident. Gated property was storage grounds for over 50-100 sea/land shipping containers. RP advises fire was extinguished. Upon investigation of Unit container, dark smoke emitting from container. Shipping container opened where smoke and fire found to rear of container. Prior to entering and extinguishing fire, I conducted an initial interview reporting party (RP and asked if there were any chemicals including the possibility of an illicit clandestine laboratory burning inside. RP "assured that fire was started from a hot weld above which hot metal caught fire to ordinary combustibles inside or below weld. Fire was emitting dark smoke and appeared to be incipient phase to rear of shipping container. A foam mattress bed found involved with fire and was major cause of smoke. Surrounding clothing and wood speakers were damaged and exposed to smoke/fire. Fire extinguished with reel-line of water and foam. BC Fox and Captain Gonzalez interviewed RP FM Maggio took control of investigation. All smoldering materials were removed from container. See investigation report for cause and origin. Gated property was storage grounds for over 50-100 sea/land shipping containers. RP advises fire was extinguished. Upon investigation of Unit container, dark smoke emitting from container. Shipping container opened where smoke and fire found to rear of container. Prior to entering and extinguishing fire, I conducted an initial interview reporting party (RP and asked if there were any chemicals including the possibility of an illicit clandestine laboratory burning inside. RP "assured that fire was started from a hot weld above which hot metal caught fire to ordinary combustibles inside or below weld. Fire was emitting dark smoke and appeared to be incipient phase to rear of shipping container. A foam mattress bed found involved with fire and was major cause of smoke. Surrounding clothing and wood speakers were damaged and exposed to smoke/fire. Fire extinguished with reel-line of water and foam. BC Fox and Captain Gonzalez interviewed RP FM Maggio took control of investigation. All smoldering materials were removed from container. See investigation report for cause and origin. Per policy, response reduced to one apparatus for a fire reported out, BC2 continued response. Upon arrival I found E4 at scene of storage container with light smoke showing initially, container began to produce dark smoke and small fire appeared to be burning in rear of container. E4 performed fire attack, BC2 took command and established Horizon IC, requested Truck 1 C3 and Fire Marshal Maggio to scn for investigation. Fire quickly knocked down by E4 crew, T1 arrived at scn and was assigned water supply and overhaul. Contact made with owner of contents int he storage unit denied and injury, declined medical assessment or treatment multiple times. I inquired if he was living in the storage unit since there was a bed that caught fire, he denied living in the unit, but could not provide an address at which he lives. stated that he was welding some holes on the roof and a fire began inside the unit, he reports that he initially had a "fire watch posted", but while welding he began to feel heat and see smoke, when he came down from roof there was a fire burning and the person doing fire watch was no longer in the area.advised that he had cut the holes in the storage unit to provide emergency exit if needed, and that the property owner would not let him access his belongings or the storage unit until the holes were repaired. FM Maggio called to the scene for investigation purposes, see his report for cause and origin / fire investigation information. Overhaul complete, one FF injured during overhaul and salvage, taken to local Emergency Department for evaluation and treatment, scene turned over to Maggio for investigation purposes, advised on the risks of being in side the storage unit or sleeping in it due to risk of CO, he again stated that he does not sleep there. All units released, Horizon command terminated. A Page 10 of 11 Additional Narrative (#2 of 2 ): Fire Origin and Cause Report: Rodger Maggio, Fire Marshal/Chief Building Official June 30, 2020 Property: 8x40 storage container Owner: Occupant: On Monday June 29, 2020 at approximately 8:50 PM I received a call from San Luis Obispo Fire dispatch requesting that I respond to investigate a fire at 4080 Horizon Lane, San Luis Obispo. I arrived, at what appeared to be a facility that stored Sea-Train storage containers, shortly after 9:00 PM and reported to Battalion Chief Sammy FOX. FOX explained that upon arrival the found smoke coming from an approximately 8 foot wide by 40 foot deep storage container, after the owner of the facility tried to cancel their response. FOX told me that the fire originated in a bedroom in the back of the storage container and that the occupant, was welding on top of the unit and started the bedding on fire. As was still on scene I took a few minutes to get his story. stated that he was an electrician and rented the storage unit for his shop. When asked who he rented from he would not answer but did state that he paid his rent to a place "over by AutoZone". told me that he had cut ventillation and emergency escape holes in his container because he thought it was unsafe not to have a second escape opening in the container, when the owner found out, he made repair the openings. told me that he was on top of the container welding the holes closed when he smelled the smoke then discovered that his bed was on fire. I initiated my scene examination outside of unit 94 where I observed a partially burned box spring and foam mattress and a stereo speaker piled in front of the container that the fire fighters had pulled out of the unit. Entering unit 94, which was in a east-west orientation, from the opening on the east side I observed heavy soot deposits through out with heat damage pronounced on the higher surfaces as I worked back to a partition wall in the back. Through the opening in the partition wall I observed an area the resembled a bedroom. On the back wall I saw a bedframe and a small dresser/nightstand in the corner. To the left of the entry into the bedroom there was a stereo speaker. The burn patterns in the room were consistent with a fire that started on the bed in accordance with statement. In the bedroom I observed what remained of electric overhead lighting and wiring, there was an outlet and switch just inside the doorway. The shop area was also hardwired and I observed an electric ceiling fan and overhead light fixtures. According to the space was powered by a small gasoline generator. While onsite I observed other containers with power cords going in to them and at least two other generators. I also observed photo-voltaic solar panels on top of an old delivery truck that was wired to the battery. It is my opinion, based on training and experience that this fire was accidental in nature, the ignition source was hot material from welding operations, the material first ignited was bedding. It is also my observation that the storage containers are not all used for storage, that many are used as shops, and at least one was used for habitation and several may have unpermitted and unsafe electrical connections. I will be forwarding this report to Code Enforcement for follow up. S. Fox BC2 Additional Narrative (#1 of 3 ): E4 arrived on scene and was assigned Div A with primary objective of direct fire attack on the left flank. T2 assigned to Div A to assist. E3 and T2 deployed a progressive hose lay on the left flank. A fence was cut to gain access. Div A progressed up the left flank until we tied in with Div Z. Both Div completed 100% mop up using hose and hand tools. E3 restored engine and was reassigned to a second fire off of Vachel st and were cancelled in route. Additional Narrative (#2 of 3 ): T2 arrived in the area of Suburban and Short while E4 and B2 investigated for best access. B2 advised T2 to report to 275 Tank Farm Rd. Upon arrival T2 was assigned to Division A and was directed to start a second progressive hoselay and take the left flank. T2 personnel began the progressive hoselay and opened a chainlink fence in one location south of the water tanks for access. T2 tied in with Div M and began 100% mopup. T2 completed 100% mopup and Div A assigned T2 to breakdown hoselines. T2 ensured all equipment was restored and was released from the scene without further incident. Additional Narrative (#3 of 3 ): TRN 1 arrived and assigned Div M. TRN 1 coordinated with Div A and Z to contain fire to under 1 acre. Mop up initiated with both hose lays that were on both sides of the fires. No obvious cause and origin noted upon initial view of fire, prior to suppression engagement.
