1. Purpose/Mission Statement *
Corazon Latino, a newly established non-profit organization, is proud to announce its mission of educating, providing emotional well-being,
sharing resources, promoting civic engagement, and empowering the Latinx community on the Central Coast. With a vision of equitable
access to services for the Latinx and most vulnerable communities, Corazon Latino aims to promote family well-being and create a healthier
future.
2. Briefly describe your organization’s purpose, vision, primary activities, and operating structures *
Our vision is that the Latinx and/or most vulnerable communities of the Central Coast can have equitable access to the services necessary
to achieve a healthy life by promoting family well-being.
Our primary activities include going directly to the community by providing recurring Community Support Groups in accessible locations
that eliminate transportation barriers and ensure that individuals have access to the resources and support they need during evenings and
on some occasions, weekends. Within these support groups, Corazon Latino aims to provide an atmosphere of community
bonding/engagement, safe spaces, free childcare, free food/snacks, and equitable access to resources.
Corazon Latino's ultimate goal is to create recurring Community Support Groups where multiple resources and services are available,
tailored to the needs and schedules of the community. By bringing together agencies, organizations, and community members, Corazon
Latino aims to create a comprehensive support network that addresses the diverse needs of the Latinx population. Corazon Latino is
planning to collaborate with organizations such as Community Health Centers (CHC), whose mission aligns with Corazon Latino's vision,
which is to educate and provide accessible, affordable, comprehensive and quality healthcare services to the community.

7. Describe the community(ies) your organization supports *
We support many of the communities on the central coast, ranging from both SLO and Santa Barbara counties. We make sure to support all
communities and especially focus on the rural and hard to reach places around these counties, such as San Miguel and Guadalupe. We
specifically support the following populations: Latinx, low-income, Mixtec-speaking, Spanish-speaking, etc. With our innovative approach to
blending partner agencies and the community into recurring support groups around the county, we will be able to reach San Luis Obispo
County residents without needing them to provide lengthy transportation rides to reach services/support. We know that the residents
seeking homeless prevention support, hunger/malnutrition prevention, and mental/physical health needs may also struggle with
transportation needs so we have made it a priority to schedule these support groups in equitable and easy to access places around the
county.

As a new non-profit within the Central Coast, Corazon Latino currently has two major sources of funding: CenCal Health and Uplift Central
Coast. We will continue to strive towards more funds in order for these support groups to continue.

1. Please provide an executive summary of proposed project/program *
Corazon Latino proposes to create recurring focus groups where marginalized communities will have access to multiple agencies providing
information and support regarding homeless prevention, hunger and malnutrition prevention, language services, and physical/mental
health services. Corazon Latino will provide coordination for these support groups along with Community Health Workers/Promotores
(CHW’s/P) in order to facilitate a safe space for individuals to encourage and support each other through their challenges. The partner
agencies will also be available after their presentations to facilitate members through the “next steps” for their resources, whether that be
signing them up for services or scheduling their first appointment. Corazon Latino also aims to provide childcare, food, and ongoing
rotation of partner agencies at these support groups.

Corazon Latino emphasizes that individuals do not have to face life's burdens alone. By joining a support group, individuals facing
homelessness, medical challenges, mental illness, or personal struggles find solace and understanding among peers who share similar
experiences. Support groups provide a platform for sharing personal stories, learning coping mechanisms, and receiving emotional support
that may not be readily available elsewhere.
Corazon Latino aims to have recurring focus groups where marginalized communities and partner agencies can come together to expand
their number of clients and directly provide services to the residents. With these focus groups, we will work with partner agencies that can
provide resources and services surrounding homeless prevention, hunger and malnutrition prevention, physical/mental health, and
language services in Mixteco and Spanish. We will conduct these focus groups with Promotores, in order to establish trust between the
community and agencies. We will be conducting these monthly focus groups in San Miguel, Paso Robles, San Luis Obispo, and Oceano.
What makes our focus groups unique to other focus groups? Our specific focus group format is recurring and allows for community
members to directly engage with enrolling for specific services with partner agencies while at the focus groups, as opposed to sending them
with information to enroll on their own. Our goal with this is to allow residents the ability to have support when learning about resources
and throughout the enrollment process for any services. It is important for us that our support group participants will be able to engage
directly with any questions they have in-person with the partner agencies. This allows for the community members to leave the focus
groups with a definitive plan, such as: having their next appointment scheduled, having their enrollment process completed, etc.
Corazon Latino's ultimate goal is to consolidate multiple resources in one location, at times that work best for the community. The
organization is committed to providing childcare services and information in the participants' native language. By introducing Community
Support Groups, Corazon Latino aims to break down barriers and empower the Latinx community to access crucial services. These groups
offer a sense of belonging, impart valuable skills, and facilitate access to a variety of resources.
5. Describe the community/population the project/program will support.*
We will support marginalized communities, immigrant communities, Mixtec-speaking communities, and Spanish speaking communities. In
addition, we will support populations needing assistance in homelessness prevention, hunger and malnutrition prevention, mental/physical
health and in Spanish and Mixteco language services.

8. Provide timeline for project *
Monthly focus groups in the following areas: San Miguel, Paso Robles, San Luis Obispo, Oceano.
We plan to begin our Support Groups approach within March, 2024. We will begin this process through the support of other grants. As we
begin these support groups, we will already have recurring dates, locations, and methods of communication to engage with the Latinx
communities and marginalized communities. We will be able to rotate agencies and services at each support group, allowing the
community the opportunity to learn and engage with agencies that provide a various range of services. We will also establish the Corazon
Latino Coalition, which will consist of partner agencies who will work together to provide presentations, services, resources, and more at
our recurring focus groups.
July 2024 - This grant will provide us the opportunity to engage with partner agencies who specialize in homelessness prevention,
mental/physical health, basic need agencies such as food banks, financial institutions, and in Spanish and Mixteco language services. After
receiving this grant, we will follow the same format that we have established with the enhanced services provided by these new partner
agencies.

All of the organizations will assist with encouraging DEI throughout our focus groups. Corazon Latino will also strengthen DEI efforts by
providing Promotores at each focus group and by providing language services in Spanish and Mixteco.
10. Describe your plan for sustainability beyond the City’s one-year award funding *
We will begin funding these support groups through CenCal and Uplift Central Coast. We will continue the recurring focus group format
with the assistance of this grant and will continue applying for grants to continue this amazing work.
11. Describe the plan for promoting this project, program, or service within the City of San Luis Obispo *
We plan on promoting this project in the ways that will best reach the people who need the most resources in our county. We will advertise
through social media, whatsapp, radio, and fliers, etc.

At Corazon Latino,we understand how diversity,equity,and inclusion are integral
parts of maintaining a healthy community within San Luis Obispo County.Our mission
revolves around our core beliefs,which strive for education,mental/physical health,
civic engagement,and empowerment for all residents of San Luis Obispo County.We
celebrate the diversity within our county through our values,which include:integrity,
service,excellence,solidarity,and empathy.Corazon Latino celebrates our diversity
every day,including in our practices,policies,and organizational governance.Another
way that we celebrate inclusion everyday is through language access as we strive to
have as much content as possible in Spanish,Mixteco,and English as a way to create
a place of belonging for all residents of San Luis Obispo County.
We are committed to continuing to support diversity,equity,and inclusion in the
City of San Luis Obispo and throughout the county within our daily processes through
our staff members,volunteers,grants,and overall structure of our focus groups.Within
our vision we continue to work towards advancing the equitable access of services
necessary to achieve a healthy life for all San Luis Obispo County residents.With the
Human Services Grant,Corazon Latino will be able to reflect our equitable mission,
vision,and values throughout San Luis Obispo County through recurring focus groups.
This grant will allow us to continue the work of serving our community through reflecting
and amplifying the voices of our diverse community.We will be able to provide safe
spaces for diverse agencies and communities to come together in creating a
sustainable resource environment for all.
