EDUCATION ON ALCOHOLISM, INCORPORATED
ARTICLE 1; PURPOSE
The purpose of the San Luis Obispo Committee for Education on Alcoholism, Incorporated
("Corporation") shall be to provide an organizational base to promote the recovery from the disease of
alcoholism and chemical dependency and to operate a residential facility to encourage and promote
sober living by recovering alcoholics in San Luis Obispo, CA.

2. Briefly describe your organization’s purpose, vision, primary activities, and operating structures *
Purpose, vision, primary activities, and operating structures:
Alcoholism and drug addiction is a complex disease, but a disease that can be treated.
After many years of experience, Middle House has developed a sober living plan which its residents are asked to follow. It includes: An
alcohol and drug free living environment, a clean place to live, a hot and nourishing meal every day, a sober living environment which
suggests individual initiative and the personal interaction of the residents.
It requires residents to be gainfully employed during their stay. The opportunity to attend Alcoholics Anonymous meetings is provided on a
daily basis. It encourages ongoing awareness of the disease and its effects through the interpersonal sharing of the residents. Recreational
activities are available.

In a broad sense, Middle House serves the greater San Luis Obispo community by removing actively alcoholic and drug dependent people
from the general public and returning to it healthy, sober, and productive citizens.
Specifically, while in treatment and by direction of the Executive Manager as Community Services, the residents of Middle House have
participated in voluntarily serving the following organizations and tasks; 1) Operation Surf, 2) as clean up crew for the SLO symphony
concert in Avila Beach, 3) partnered with the SLO Food Bank to serve free food to individuals classified as homeless, 4) joined other local
non-profits in distributing free food donated by the LDS church. The fourth event assists other non-profits such as The Salvation Army, B, a
local women's shelter and Resilient Souls.

All funding is provided by the clients through rent payments. A few random and unsolicited donations are made but they are not regular
and are not depended on as part of Middle House’s funding.
Part 3 Project/Program Information

The House Manager continues to evaluate the effect of the Client continues to implement a clean and sober lifestyle. This
program to influence future graduates.process occurs five times.

When clients of Middle House relocate to independent living after a satisfactory stay at Middle House, they find it difficult to find housing
due the extreme expense of pre-rental requirements (ie., security deposit, first & last months rent, etc). This grant will be a contribution to
the individual to supplement these expenses.

4. Please provide a detailed description of proposed project/program *
As a successful client completes their tenure at Middle House they will have to move to independent living. This means renting an
apartment or house in San Luis Obispo. Initial costs of this transition are extreme. Costs could include 1) a security deposit on the residence
and on each utility (water & sewer, gas & electric), 2) forward payment of first and last months rent, and 3) acquiring other necessities (some
furniture, renting a truck for moving, etc).
Often a client, although employed, does not have adequate capital for these large costs. Providing a person with a grant of $1250.00,
will favorably subsidize their own contribution to these expenses. Were this gift to be granted to an expected number of 5 exiting clients,
the total grant would be for $6,250.
5. Describe the community/population the project/program will support.*
The candidates to receive this grant have lived in the sober living environment at Middle House and have demonstrated their ability to live
responsibly, pay their bills and cooperate with others. They have stayed sober and clean for a significant length of time and have
demonstrated the ability to completely leave their habit in the past. They have worked in full or part time employment for the majority of
their residency and have practiced being a positive member of society.
6. The HRC has identified homeless prevention including affordable and alternative housing, supportive services, and
transitional housing as the main funding priority. Does your grant request meet this funding priority?*

As soon as the grant is funded, any clients who successfully leave Middle House will be gifted $1250 for moving expenses. This procedure
with repeat until 5 persons have been gifted, thereby exhausting the grant’s balance. The time this task is variable and completes within a
year.
9. Identify any partnerships/collaborations that are supporting this project/program and their roles *
Middle House initiated this program and is the only known participant.
10. Describe your plan for sustainability beyond the City’s one-year award funding *
The clients who receive this grant are expected to continue the practice of sober, independent living. They will have the skills and tools
acquired during their residency at Middle House. They are encouraged to continue to attend twelve step meetings and regularly associate
with the recovery community.
11. Describe the plan for promoting this project, program, or service within the City of San Luis Obispo *
This program will continue until funding exhausted. This is Middle House’s third application for this grant, having been previously awarded
two. This program has aided in securing housing in the city of San Luis Obispo for nine people and a tenth is pending. The Executive
Director of Middle House has continued to stay in communication with most of those former clients. Money granted to them though the
sober living facility grant has helped them become housed, employed, contributing members of the SLO community.
All of the prior recipients of this grant are currently heavily involved in service positions in the SLO recovery community where they assist
people in, and new, to recovery.
They serve to demonstrate to current clients the benefit of completing their participation in Middle House and of choosing a clean and
sober life style.

Detailed Budget for the Program
The Executive Director of Middle House selects a candidate-client that qualifies to
receive one of five divisions of the grant money. That candidate must have completed all
of the obligations and agreements committed to upon their entry into Middle House.
The candidate-client prepares to leave,
The Executive director gives the candidate a check for $1250.00 (funded by the grant).
The candidate-client spends the money as they determine supports the cost of moving.
The candidate-client occupies their new residence.
This process repeats four more times, when all of the grant monies, $6250, are
distributed.
Statement of Diversity, Equality and Inclusion
As reported in our mission statement, the purpose of Middlehouse is to provide and
promote the recovery from the disease of alcoholism and chemical dependency, serving
the community of San Luis Obispo. The disease does not discriminate. Middlehouse offers
its services and opportunities to those who choose to recover. There are no other
qualifications.
The Board of Directors has required that all board members, staff and residents each
read and sign a Code of Ethics which places great value on diversity and inclusion.
Throughout Middlehouse’s entire organization and offerings, conduct which may lead
to discrimination, harassment, bullying, abusive conduct or retaliation is not and will not be
tolerated and will be reported to the appropriate authority,
The grant will be rewarded to anyone who completes the recovery program at
Middlehouse. Characteristics of age, ethnicity, creed, marital status, medical condition,
race, religion or sexual orientation are not considered when awarding this grant.
This grant will enable a sober and productive citizen to thrive and serve others within
the general community of San Luis Obispo.
