Upon arrival BC3 established Legacy IC to find 3 two-story single-family dwellings under construction heavily involved in fire. IC declared a defensive strategy, a working incident, and requested a second alarm. E4 assigned Div Alpha (A) T1 assigned Div Charlie (C) Incident objectives were to confine fire to buildings of origin, flow large water streams to extinguish fire, and protect exposures. E4 attempted to hook up to nearest hydrant and found it to be inoperable. E2 assigned to Div C and water supply ICP established at Heirloom and Froom Ranch Staging established at Dalidio and Froom Ranch Training 1 arrived and was assigned to assume Div A with E4 working for Div A. E3471 assigned to assist with water supply and then report to Div C. E3 assigned to assist with water supply and the to report to Div A. Utility company representatives from PG&E and So Cal were on scene and confirmed there was no utility service to any of the addresses involved. Second alarm units were assigned as follows: E15, E64, and E5392 assigned to Div A. T7545 assigned to Div C and then later reassigned to Div A. All units utilized multiple 1.75", 2.5" hand lines, as well as deck gun and elevated master streams to protect exposures and extinguish fire. Fire scene was surveyed, and photos were taken by SLOPD investigator Jeff Koznek () and SLOFD Inspector/Investigator Joey Nau (. Fire scene was tapped off and was secured by SLOPD overnight. State OES was notified due to fire suppression water runoff, Control # 24-3381. At 0800 on 06/12/2024 SLOFIST investigation team convened and initiated investigation. At approximately 1500 on 06/12/2024, SLOFIST investigation team completed the onsite portion of investigation at all addresses involved. Construction site representative Chris Harris was advised that all fire resources were clear of incident location and scene was released. Upon arrival, E-3 assisted T-1 and E-4 with establishing a water supply to hydrant on San Luis Ranch road by supplying an additional 50 feet of 4" hose to finish the hydrant connection. After securing water supply, E-3 proceeded to engage in fire attack on the side C/B exposures utilizing 2 1/2" and 1 3/4" hose lines pulled off of E-4. Following the successful knockdown of the fire, E-3 assisted in breaking down and rolling up hose utilized off of E-2- and was released by IC to perform code 11 City coverage. Additional Narrative (#2 of 10): Truck 1 was assigned Div C and had E2 assigned to them for fire attack and exposure protection. Truck 1 pulled a cross lay for fire attack and exposure protection on the C side. Pump 1 and ladder 1 attempted to get water from the hydrant E4 was next to but the hydrant was inoperable. Pump 1 and ladder 1 with assistance from PD and 3471 established a water supply with a 650-foot LDH hose pull. Truck 1 put up their aerial waterway for a defensive attack while supplying E4 with water. E2 requested additional personnel and FF1 was assigned to E2 for their operation. 3471 pulled a hard line from their Engine and provided fire suppression in the vegetation on the "Charlie" side. Truck 7545 personnel was assigned to Div C and replaced E2's crew while they were in rehab. Once E2's crew was out of rehab, 7545 was reassigned to Div A. Div C was also made safety on the C side while T1 and 7545 used their aerial waterways to fully extinguish the fire. T1 and 3471 assisted E4 in rolling hose and breaking down the LDH that was connected to the hydrant. Page 12 of 18 Additional Narrative (#3 of 10): Training 1 arrived on scene and was assigned Div Alpha. Alpha was assigned E-3, E-4 initially; Alpha gave verbal direction to both crews to continue to make a transitional attack on the unit to the north, to prevent further fire extension. Div Alpha completed a 360 and relayed findings to IC. Div Alpha asked for 3 additional units and was given E5392, E64, E12. Div Alpha had unit swap with E3 and E4 who were transitioned to rehab. Units continued to knock down active fire on the roof of the furthest north unit, crews advised to stay out of the collapse zone. Div Alpha made face to face communication with Div Charlie to establish division boundaries and responsibilities. Div alpha was assigned all overhaul and operation on interior of block wall, Div Charlie was everything on the Froom Ranch street side of the incident. Div Alpha, Div Charlie and IC met to devise new plan of attack on remaining fire on roof of unit. T7545 was repositioned to Terra Way to assist T1 with aerial master stream operation. All units cleared from surrounding buildings as safety precautions as aerial water was flown. Fire fully extinguished with aerial operations. All units released without incident. Additional Narrative (#4 of 10): E2 arrived at scene behind T1. T2 was assigned to find a working water supply on Froom ranch. E2 made access to the hydrant and secured the water supply. E2 was assigned to work for Div C (Truck 1) with the objective of suppression and exposure control. E2 FF placed a deck gun in service, stopping the progress of the roof fire on the west exposure house. E2 placed 200' of 2.5" handline in service. T1 FF assigned to E2. After 360 by E2, the 2.5" handline transitioned to the trunk line in the cul-de- sac. FF1 utilized a hotel pack for extinguishment. FF2 deck gun became inefficient and shut down the line to place an additional hotel bundle in service in the cul-de-sac. FF2 and E2 made entry into the western exposure house to complete the extinguishment of the roof. The E2 crew remained at the scene, assisting with the overhaul and monitoring of adjacent structures. E2 release from incident for equipment re-hab. Page 13 of 18 Additional Narrative (#5 of 10): SAN LUIS OBISPO CITY FIRE DEPARTMENT FIRE PREVENTION BUREAU **SUPPLEMENTAL REPORT** Incident Number: 24-03290 Incident Date: 06/11/2024 Incident Address: 1057 Terra Way Incident Type: Residential Structure Fire INCIDENT ACTION: SUPPLEMENTAL FIRE INVESTIGATION REPORT Violations: See Investigation Report Name: Matheson Bliss Agency: San Luis Obispo City Fire Department Rank: Hazardous Materials Coordinator Assignment: Fire Station One Date: 06/19/2024 Narrative: I met with SLO City Fire Department Fire Inspectors Joey NAU, Julianna HUTCHINSON, and Arlen DIEPENBROCK, SLO City Police Department Detective Jeff KOZNEK and members of SLO Fire Investigation Strike Team (SLO FIST) at Terra Way at 0800hrs on 6/12/24. I surveyed the exterior of building numbers 128, 129, and 130 Terra Way in a clockwise direction. All buildings were of wood frame construction with oriented strand board (OSB) in place; however, no windows, doors, Tyvek house wrap, nor siding had yet been installed. I observed minimal to no fire or smoke damage on the western portion of building 128 (Side A, Side B) including the garage and secondary story bedroom above the garage. Additionally, the northeast facing back wall (Side C) exterior OSB showed no signs of smoke or heat damage. Significant damage was observed on the eastern portion of the building (Side D) adjacent to building 129. Fire damage including complete destruction of OSB, charring of wall studs, and sooting and was greater on the second floor and exterior of building 128 than the interior at this side. Detective KOZNEK stated that SLO City Police Department received a report on the morning of Monday June 10, 2024, that a fire was set in building 128 at the stairwell at an unknown date and time over the previous weekend. The stairwell is centrally located in the building. I observed an approximately 2sq ft rectangular stain on the concrete adjacent to the lower steps of the stairway. Charring and sooting were observed in the structural supports of the exposed side of the stairwell/railing but did not extend to other interior portions of the building. Additionally, sawdust accumulation and containers of flammable adhesives were observed on the interior eastern wall (Side D) of building 128 with some staining observed on the container labels but in relatively undamaged condition and closed. I observed the second floor of building 129 had collapsed as well as the majority of the first floor with debris and heavily charred structural supports piled inward on the concrete slab. The front/western (Side A) portion of the building included the garage and an attached additional dwelling unit (ADU) which remained the most intact. OSB with relatively minimal exterior sooting was observed at the western most point of the ADU (Corner A/B) as well as the southwest portion of the exterior garage (Corner D/A). Some OSB was observed without smoke/soot damage on the exterior north corner (Corner B/C), above wall studs which had failed and collapsed inward on the slab. Framing in the eastern corner and sidewall (Corner C/D, Side D) of the building remained upright; however appeared heavily charred, leaning and structurally insecure. Similarly, the second floor of building 130 had collapsed. A significant portion of the first floor had collapsed with the exception of the garage framing at the southwestern side (Side D, Corner D/A) and several studs on the interior/ northeastern wall of the garage. Studs and beams were observed still intact with less soot and char damage observed exterior on the beam across the upper front of the garage (Side A). Building 130 was the closest in proximity to a brick wall extending on the southwest and southeast sides of the structure. Dark staining was observed on the back/southeastern portion of the wall, with less staining observed on the southwestern wall (Side D). The brick wall and an adjacent walkway served as a separation between buildings 130 and 131. Relatively minimal damage was observed to the adjacent siding on the northeastern portion of building 131 (Side B). Windows had been installed in this building and some smoke damage was observed around the window frames. Charring was observed on the northeastern facing portion of the roof of building 131. Groups were formed to assess different aspects of the investigation. I assisted in the determination of potential electrical sources in the area. A temporary electrical main was stationed approximately 200 ft northeast of the investigation area on the opposite side of Heirloom Place. Electrical (6AWG) was run southwest underneath Heirloom Place to an in-ground electrical box adjacent to the street. Next to the in-ground box was a 50 Amp power distribution box (Box A). Three additional power distribution boxes were observed in the investigation area (B,C,D). Box B was located behind/north of building 127. Box C was located on the sidewalk in front of building 129. Box D was located in between buildings 131 and 132. The cord running from Box A to Box B had been unplugged at both ends. A cord was observed plugged in from Box B to Box C and another plugged in from Box C to Box D. Per Coastal Community Builders Inc. Superintendent Chris HARRIS and employee Gregory KOHLER, the power distribution boxes are normally left with power on overnight. During response actions on 6/11/24 power had been on to these distribution boxes until a PG&E employee had disconnected one of the boxes. Mr. KOLHER reported he then unplugged Box A and switched off the main sometime between the hours of 1945-2030 on 6/11/24. An orange extension cord was observed in the immediate area of Box C; however, it was not plugged in to the power distribution box at the time of the investigation; it terminated in the garage of building 128 and was not plugged into any equipment in that structure. The three structures 128, 129 and 130 did not yet have electrical panels installed and were not tied into the electrical main. I assisted in removal of debris from the slab of 129 and 130. Hand shovels and heavy equipment including a skid steer and excavator were utilized to remove remnants of wood construction and place debris in between buildings. The concrete slabs were washed down. Patches of heavy spalling of concrete and staining was observed throughout both slabs. I departed the scene and returned to Fire Station 1 at approximately 1530hrs. E4 personnel pulled 1 3/4" and 2 1/2' lines for exposure protection in a defensive position outside the collapse zone. E4 operator was unable to secure the closest hydrant which was on the corner of Froom and Heirloom due to the hydrant being inoperable. Div A notified IC of the hydrant being inoperable. Operator of E4 and T1 secured a water supply by hand jacking supply line east on Froom towards Dalidio. Div A was taken over by Training 1 and E4 was assigned to Div A to continue defensive suppression ops.
