E4 arrived on scene, established Madonna IC, to find a two story multi family dwelling with smoke and fire coming from a
second-floor apartment (apt # 88).
IC declared an offensive strategy, working incident, and conducted a 360 of the involved building.
BC3 arrived on scene and assumed Madonna IC.
E4 communicated condition report, one unit involved, moderate victim profile, with possible fire extension to attic.
IC set incident objectives: confine fire to unit of origin and conduct a primary search.
IC assigned City Tac 1 for all tactical radio traffic.
E4 assigned to report to Div 2 and initiate fire attack and primary search.
E3 assigned to establish 2 out with crew and assume Div 2, with tactical objective of fire attack and primary search.

exception of E4, who remained on scene to assist with investigation.
Red Cross was contacted and worked with the property manager to provide assistance with housing and food for the 11
occupants that were displaced.
Two SAVE cards were issued to displaced occupants.
So Cal Gas company advised that gas utilities have been locked out and secured to units 84 and 88.
PG&E worked with a facility electrician to restore power to the seven unaffected units.
Fire scene was surveyed, and photos were taken by SLOPD investigator Jeff Koznek nd SLOFD
Inspector/Investigator Joey Nau .
Onsite portion of investigation was completed, scene was secured and released to property manager by E4, Madonna IC
terminated.

Additional Narrative (#1 of 5 ):
E-4 arrived on scene to establish Madonna IC. E4 completed a 360 walk around the building to find fire and smoke from corner
of building. PD told to evacuate residence in floor below fire room.
E-4 secured a water supply and stretched a 1 3/4 line for transitional attack. Someone from crowd of people stated residents
were not in the unit involved in fire. E-4 gave condition report with moderate victim profile given and possible extension into
attic.
B3 arrived on scene and assumed command, E-4 to report to division 2. E4 made entry once 2 out in place and knocked down
seat of fire. E-4 completed a primary search with an all clear. E-4 checked for extension into attic with no findings. E-4 noted
electrical panel had several breakers tripped.
E-4 remained on scene throughout incident to assist with overhaul and fire investigators with moving debris. E-4 was
transferred command from B3 and told to finish investigation before turning scene over to property manager. E-4 ensured no
heat present with TIC throughout residence with no temp reading above 65 degrees present.
E-4 secured building by locking from door and turned scene over to property manager without incident.
Additional Narrative (#2 of 5 ):
T1 arrived at scene and was assigned Roof Division. T1 personnel deployed a 35' extension ladder and tools to the A/B corner
of the involved area of the building and accessed the roof. T1 found a stable roof deck but large volumes of smoke
consistent across the entire attic coming from numerous vents. T1 personnel cut a heat hole over the north end of the
building and cleared the ceiling below. T1 asked Division 2 if the ventilation was adequate. Division 2 requested more
ventilation. T1 personnel proceeded to the south end of the roof and cut another heat hole and cleared the ceiling below.
T1 confirmed with Division 2 that no further ventilation was needed. T2 confirmed with IC that nothing further was needed on
the roof and descended to rehab and check-in with Division 2 for overhaul needs. Division 2 had no needs while waiting for
investigation so T1 personnel assisted with cleaning up tools and equipment. T1 became available and cleared the scene
without further incident.
Additional Narrative (#3 of 5 ):
Truck 2 arrived on scene and was given the assignment to control utilities and report to Div 2. Truck 2 shutdown gas and
electric to the apartment and faced to face with Div 2. Truck 2 was then assigned to assist E21 with 2nd search and pull
ceiling to check for further fire extension. T2 performed mild overhaul but was instructed to wait for investigators. T2 was
then released to cover the city.
Additional Narrative (#4 of 5 ):
E-3 arrived on scene and was assigned Div 2 and two-out by Madonna IC.
Pump 3 and Firefighter 3 deployed a second 1 3/4" hose line to the second floor unit involved in heavy fire, and assisted E-4
personnel with primary search and fire attack.
Division 2 was assigned T-2, E-3, E-4 and Cal Fire E-21 for resources.
Division 2 assigned T-2 to conduct secondary search, and assist in overhaul efforts. Cal Fire E-21 was assigned to check for fire
extension in the unit below on the first floor and finding none, transitioned to assisting with overhaul on fire unit.
During the course of the secondary search and overhaul, E-3 and Cal Fire E-21 were successful in locating and rescue of two
pet kittens that were missing in the fire unit, they were alive and appeared uninjured and were reunited with the occupants.
Division 2 continuied to assst IC with relocation efforts for the displaced family and assigned E-4 personnel to

On Friday, June 21, 2024, I was paged by SLO Emergency Communications Center (SLOECC) as requested by San Luis Obispo
Fire Department (SLOFD) Battalion Chief, Shayne SKOVE, to investigate a residential structure fire at 1578 Madonna Road,
Unit #88.
I responded to Station One to retrieve my radio, gear and assigned work vehicle, and I updated SLOECC while en route via
radio, that I was responding to the incident. I arrived on scene at approximately 23:30 hours.
Upon my arrival, I checked in at the incident command post to receive direction from BC SKOVE. SKOVE provided me with a
brief update and directed me to the south-facing side of the property, where I met with SLOPD Detective, Jeff KOZNEK.
KOZNEK had taken preliminary photos of the fire damage, a sketch of the scene, capturing the apartment’s layout, and
conducted witness interviews, gathering contact information of witnesses.
We met with SLOFD Captain Armando GUTIERREZ and crew, who led us upstairs into apartment #88 to view the property’s
interior damage, and they provided me with a verbal update regarding their fire suppression operations for both of their
visits.
GUTIERREZ stated that SLOFD was initially dispatched earlier in the evening (approximately 17:30 hours) on a first call, due to
report of problematic electrical outlet. Fire Engine 4 responded to the incident and observed smoke in the general area of
the bedrooms. The crew was directed to a child’s bedroom where the reporting resident, , stated an outlet
behind a crib was observed sparking and that he shut the associated breaker off as a precaution (no visible damage to this
outlet was observed by the crew upon their arrival).
Shortly after turning off the breaker to the room with the crib, reported to the crew that he heard a noise coming
from the rearmost children’s bedroom and then entered to observe a piece of paper on fire on the floor in front of an outlet
near that bedroom’s window. The paper was extinguished by the residents prior to the engine company’s arrival.
The crew photographed visible damage to the drywall near the outlet and removed this section to verify the absence of
additional heat, smoke or fire within the wall. GUTIERREZ notified property manager, , of their findings, and
directed her to leave the breakers off until a PG & E representative could follow up.
Per KOZNEK, The PG & E rep referred the property manager to a licensed electrical contractor, who conducted a service visit,
notified the residents that the breakers could be turned back on, and stated that they should not use any outlets in the
children’s bedrooms, pending a follow-up service visit from another electrical contractor during the week. See Detective

SLOFD was dispatched to the property a second time that night and arrived at approximately 21:30 hours to smoke showing,
and the rearmost children’s upstairs bedroom in flames. SLO City Fire suppression crews extinguished the fire and our
investigation took place at approximately 23:50 hours, once premises were confirmed to no longer be considered an
immediate safety hazard.
Detective KOZNEK and I observed smoke damage throughout the property and noted significant fire damage to the
rearmost children’s bedroom. I took photographs to capture the scene and surrounding areas, prior to having the
suppression crew excavate debris from the most burned portions of the room. There were no visibly protected areas around
the outlets or on their surrounding walls in the children’s rooms to indicate that any devices were plugged in, immediately
prior to or during the fire. A damaged electrical cord from a dresser desk under the window was found unplugged and
collected, along with what appeared to be a damaged electronic charger, also found unplugged. A damaged junction box
pulled from the ceiling was also collected and showed no signs of internal fire damage.
All outlets and walls were inspected in the apartment, with damage noted only to two, both of which were in the rearmost
children’s bedroom.
An outlet behind the bed showed signs of fire damage externally, but was largely visibly protected behind the drywall and
insulation once removed by suppression crew members.
The outlet near the children’s dresser desk had shown signs of damage on the first call of the evening, and GUTIERREZ’S
photos capture visible partial damage adjacent to the outlet once the drywall portion was cut away.
After the second call (reporting the fire), additional wall layers were cut away to show partially protected insulation with a
portion displaying a burn pattern, and visible damage to a wooden stud running vertically to the right of the outlet.
The dresser desk, containing clothing, toys and a backpack on top, were all nearly consumed in the fire and the wall directly
adjacent to this area displayed strong calcination.
The same wall, near the closet and behind the bed, showed strong burn patterns just above where the mattress and more
clothing and toys were present, indicating high volumes of combustible fuel.
Smoke detectors were found functioning in the unit at the time of the investigation. There were no signs of defective
appliances in the room that had caught fire, no visible accelerants found on scene, nor indicators of an intentionally started
fire.
Investigation discovered the following, indicating that the fire most likely started at or near the outlet under the rearmost
children’s bedroom window as a result of an electrical system malfunction.
• Reported arcing of an outlet near the crib in the anterior, adjacent bedroom.
• Clear visible burns to the exterior drywall from the outlet nearest the window and dresser desk in the rearmost
children’s bedroom (prior to the fire).
• Witnessed burning of a piece of paper directly in front of this outlet.
• Visible damage adjacent to this outlet from within the walls, behind an otherwise protected area of insulation and
wooden 2x4 studs.
