Tasks include coordinating monthly inspections, completing annual reports, completing post-construction requirement reports, reviewing trash assessment requirements, providing training as needed, inspecting post-construction storm water devices, representing the City of Pismo Beach in communications with the Water Board, and assisting in other storm water services as needed by City staff. Client: City of Pismo Beach Contact: Eric Eldridge, PE, Senior Engineer (805-773-7041) Contract Amount: $14,500 Project Timeline: April 2023 - present Emergency Services Temporary Facilities The City of Pismo Beach awarded CLAD a contract to provide project management, coordination, construction management, and inspection services for the City’s emergency services temporary facilities. CLAD is overseeing the demolition of existing buildings at 330 Main Street, replacement with modular buildings, preparation of Suite A-4 at City Hall, and the implementation of a Fire day use room at the Veterans Hall. CLAD performs routine site inspections and reporting for all locations under this contract. Client: City of Pismo Beach Contact: Eric Eldridge, PE, Senior Engineer (805-773-7041) Contract Amount: $63,250 Completion Date: Ongoing VSFB Facility Fire Hydrant Code Upgrades CLAD works with C.D. Lyon, Inc. to provide civil engineering services for a fire hydrant upgrade project at Vandenberg Space Force Base. The goal of the project is to install water pipe, valves, and fire hydrants at 10 American Water locations on base to establish regulatory fire codes and industry standards. CLAD’s scope of work includes topographic survey, civil engineering design services (30/60/90/100% drawings), construction staking, GIS/as-builts, traffic control plans, and client meetings. We also oversee fire flow testing and pump replacement design. Client: C.D. Lyon, Inc. Contact: Ben Arbogast Contract Amount: $81,700 Project Timeline: December 2023 - present 580 Bell Street CLAD provides design and permitting services to a private client for a commercial development located on Bell Street in Los Alamos. Our principals have worked with this client for several years. CLAD’s most recent services have been preliminary grading and drainage design, preliminary hydrology reporting, and public improvement plans including street improvements, utilities, and erosion control. Client: B&I Ascend, LLC Contact: Ian Meyer, ian@mco.ltd.uk Contract Amount: $16,710 Project Timeline: July 2023 - present On-Call Civil Engineering Design Services 7 VSFB Building 9334 Sewer Lateral American Water issued an RFP for the design-build of a sewer lateral near Building 9334 at Vandenberg Space Force Base. The project includes abandoning the existing sewer lateral and installing approximately 230 linear feet of 6-inch sewer pipe as well as one (1) dual sweep-cleanout. CLAD’s scope of work involves engineering design in accordance with all local, state, and federal regulations as needed for work on the base. Client: Joseph Engineering Contact: Steve Behnke, 805-928-3703 Contract Amount: $20,000 Project Timeline: February 2024 - present A Street & Betteravia CLAD worked with Cole Enterprises to provide civil engineering services for a new commercial development at the corner of A Street and West Betteravia Road in Santa Maria. We provided a topographic survey per the client-approved sewer and water routing, then provided a plan for the tie-in to the existing water and sewer mains on A Street. The new lines were extended to an office building facing West Betteravia Road and all design was completed in accordance with the City of Santa Maria’s standards for approval. Client: Cole Enterprises Contact: Steve Chambers, 805-314-2578 Contract Amount: $5,800 Project Timeline: April 2023 - July 2023 Judkins Middle School Accessibility Improvements The Lucia Mar Unified School District (LMUSD) refurbished and demolished various portable classrooms on their campus. As part of this project, portions of the existing asphalt and concrete required removal and replacement, which needed ADA accessible paths upon replacement. CLAD provided civil engineering design services for the accessible paths, and also conducted site inspections to ensure the paths of travel were installed in accordance with the plans. Client: Lucia Mar Unified School District Contact: Andy Stenson, 805-474-3000 x1032 Contract Amount: $12,550 Project Timeline: October 2022 - August 2023 On-Call Civil Engineering Design Services Paso Robles Curb Ramps CLAD assisted John Madonna Construction with public improvement plans to upgrade curb returns on Theater Drive in Paso Robles per ADA standards. We performed a topographic survey and full public improvement plans for the two (2) new ADA-compliant returns. Client: John Madonna Construction Contact: John Madonna, 805-543-7751 Contract Amount: $8,500 Project Timeline: July 2023 - September 2023 In the course of his 24 years working in the civil engineering field, Alberto has designed, inspected and managed the construction of a variety of projects including water, sewer, streets, drainage improvements, flood structures, and traffic control. He is a qualified Stormwater Pollution Prevention Plan (SWPPP) developer and practitioner. Throughout his career, he has routinely led teams of 10 – 20 people, assigning each person to tasks and projects that align with their individual strengths. 8 License Registered Civil Engineer CA - #67602 Education Bachelor of Science Civil Engineering Cal Poly San Luis Obispo L. Alberto Lopez, RCE, QSD/P MS4 permit under the Regional Water Quality Control Board. Jack Ready Imagination Park Alberto has performed various pro bono services for Jack’s Helping Hand, a local nonprofit organization. Alberto has lent design and project management services to the organization for Jack Ready Imagination Park, which will be the first universally accessible park for physically challenged children in SLO County. The park will include an accessible playground and equestrian therapy area. Alberto aided in the completion of the maintenance building, public restroom, and caretaker’s quarters. Emergency Services Temporary Facilities The City of Pismo Beach awarded CLAD a contract to provide project management, coordination, construction management, and inspection services for the City’s emergency services temporary facilities. As project and construction manager, Alberto oversees the demolition of existing buildings at 330 Main Street, replacement with modular buildings, preparation of Suite A-4 at City Hall, and the implementation of a Fire day use room at the Veterans Hall. Alberto coordinates the various activities and conducts routine construction inspections at all sites. Safe Routes to School Sidewalk Infill Alberto was the Principal-in-Charge and QA/QC manager for the engineering services, which included design, plans, and specifications for the construction of ADA compliant curb ramps and sidewalks, curb and gutter, and driveway approaches surrounding the school. The goal of the project was to increase pedestrian and bike safety near Grover Beach Elementary School. Spanos Stadium Turf Improvements Alberto teamed up with Harris Architecture & Design and Maino Construction for a turf replacement project at Cal Poly San Luis Obispo. His civil engineering department began working on grading plans for the stadium improvements. These plans included raising the field elevations, adjusting stadium walkways for ADA compliance, and adjusting the direction of flow for all surface drainage. Alberto served as the project manager and oversaw all internal coordination between surveyors and engineers as well as communication between the Owner and the project team. On-Call Civil Engineering Design Services Pismo Beach Storm Water Compliance Alberto assists the City of Pismo Beach with compliance with the 9 Upper Field Turf Improvements Alberto once again provided civil engineering services for the turf replacement project at Cal Poly’s upper practice fields as part of the school’s efforts to upgrade their outdoor sports facilities. The project is currently under construction. Alberto coordinated the multidiscipline efforts for this project and oversaw the creation of the grading plan set, handling QA/QC plan checks as needed throughout the project’s duration. Cal Poly Solar Farm and Solar Lab Alberto was the lead civil engineer for a design project for the 20 acre solar farm at Cal Poly, San Luis Obispo. Alberto provided a grading plan for the solar array, grading for new classroom and parking lot, access roads, pre and post drainage analysis, and storm water pollution prevention plan (SWPPP). For the lab, Alberto provided a grading and drainage plan and a retaining wall design for an ADA-compliant solar academic lab at Cal Poly near Building 20. Oceano Community Services District - District Engineer Alberto served as the District Engineer for Oceano Community Services District for nearly a decade. He provided consulting and engineering services to OCSD on an ongoing and as- needed basis. Services included management of OCSD’s water resources equipment, design of and assistance in the management of dozens of capital improvement projects, and other general engineering services. In his 20 years working in the civil engineering field, David has designed a variety of projects ranging from residential and mixed-use projects to grading design for oilfield facility roads and well pads. He is recognized for his expertise in storm water control practices, particularly methodologies used throughout the Central Coast. David routinely oversees the preparation of plans and specifications as well as hydrology and hydraulic studies. 10 License Registered Civil Engineer CA - #70849 QSD/P No. 23867 (CA) Education Bachelor of Science Civil Engineering Cal Poly San Luis Obispo David Chanley, RCE, QSD/P, QiSP valves, and fire hydrants at 10 American Water locations on base to establish regulatory fire codes and industry standards. David manages CLAD’s scope of work, including topographic survey, civil engineering design services (30/60/90/100% drawings), construction staking, GIS/as-builts, traffic control plans, and client meetings. He also coordinates with our subcontractor providing fire flow testing and fire pump replacement design. VSFB Fire Hydrant Code Upgrades CLAD works with Joseph Engineering to provide civil engineering services for a fire hydrant upgrade project at Vandenberg Space Force Base. The goal of the project is to install water pipe, 520 Bell Street David was the project manager in charge of improvement plans for the relocation of a private building at 520 Bell Street in Los Alamos. The redesign involved updates to the on-site and off-site civil improvement plans. David oversaw the grading and drainage design, erosion control plan, utility plan, notes, details, hydrology report, and storm water control plan. The off-site public improvements included relocation of an under-sidewalk drain. People’s Self-Help Housing Buellton David worked with Ravatt, Albrecht and Associates to provide civil engineering services for a PSHH project located on McMurray Road in Buellton. This new construction features over 75 new units on a 10-acre mixed-use site. David was the project manager for this work, overseeing the conceptual and schematic design, entitlements, construction documents, bid support, and construction administration . David also serves as the Qualified SWPPP Developer (QSD) for this project and oversees all SWPPP inspections. 580 Bell Street CLAD is contracted with a private developer to provide preliminary grading, drainage, and hydrology services for a property at 580 Bell Street in Los Alamos. Our scope of work was recently expanded to include public improvement plans per Caltrans standards, including street improvements, utilities, and erosion control. David oversees the engineering for the base scope as well as the public improvement plans. David is CLAD’s project manager for this work, and oversees engineering design in accordance with all local, state, and federal regulations as needed for work on the base. On-Call Civil Engineering Design Services Jenna has over 10 years of experience in project coordination and administration, as well as 7 years of experience as a civil engineering designer. She is well-versed in providing improvement plan design for public and private projects alike, including plans for street, sewer, water, and utility upgrades. Her experience spans a variety of markets, such as public works, schools, and residential and commercial development. In addition to her design work, Jenna is trained in site inspection and SWPPP inspections. 11 Emergency Services Temporary Facilities CLAD has a contract with the City of Pismo Beach to provide project management, coordination, construction management, and inspections services for the City’s emergency services temporary facilities. Jenna has provided project coordination services for the contract. She coordinated the mobile office site plan, and floor plans for the Main Street location, floor plans for Dispatch, layouts at the Veterans Hall, containers for the Corporation Yard, and fences and alarms throughout all facility locations. Entrada de Paso Robles CLAD is contracted to perform SWPPP inspections for the Entrada de Paso Robles project site in northern San Luis Obispo County. Currently the site of the Field of Lights art installation, the remainder of the 400 acres are under construction to be turned into a destination park and resort off of Highway 46. Jenna assists with SWPPP inspections and documentation for the site, and has trained multiple other SWPPP inspectors for this large, complex site. Arroyo Grande High School Practice Gym CLAD provided surveying and civil engineering services for the construction of a practice gym at Arroyo Grande High School. Once topographic survey was completed, Jenna completed the conceptual design plan for the new 13,000+ square foot gymnasium which will primarily be used for physical education classes and youth sports. In addition to the building, the design also included ADA compliant parking. Judkins Middle School Access Improvements Jenna provided design and inspection services for access improvements at Judkins Middle School. The Lucia Mar Unified School District (LMUSD) refurbished and demolished various portable classrooms on their campus. As part of this project, portions of the existing asphalt and concrete required removal and replacement, which needed ADA accessible paths upon replacement. Jenna contributed to the civil improvement plans for this project, and also provided site visits and inspections for the new paths of travel. Stanford LDS Institute CLAD was contracted by Harris Architecture and Design to provide updated grading and drainage plans for the parking lot at the LDS Institute at Stanford. Jenna provided civil engineering design for the ADA improvements to the lot, and consistently communicated with pertinent governing bodies through the plan check and approval process. Education Bachelor of Arts English Cal Poly San Luis Obispo Jenna Wagner On-Call Civil Engineering Design Services Anthony has over 10 years of experience in the civil engineering field. He specializes in producing detailed designs for private and public infrastructure projects, report writing, and calculations for various projects spanning from entitlement and development stages through the inspection process of construction. In addition to design, he provides plan review, RFI review, and contractor submittal review for public and private projects. 12 People’s Self-Help Housing Buellton CLAD is under contract with Ravatt, Albrecht & Associates to provide engineering services for People’s Self-Help Housing’s Buellton Garden Apartments. This project involves the construction of a new, two-story, 89-unit affordable multi-housing complex on three acres of land. Anthony assists with reviewing RFIs and submittals as part of CLAD’s bid support and construction administration scope of work. He previously completed civil engineering design for the project. Anthony has been involved in the design process since the beginning of the project and has provided engineering design in accordance with all local, state, and federal regulations as needed for work on base. VSFB Facility Fire Hydrant Code Upgrades CLAD works with Joseph Engineering to provide civil engineering services for a fire hydrant upgrade project at Vandenberg Space Force Base. The goal of the project is to install water pipe, valves, and fire hydrants at 10 American Water locations on base to establish regulatory fire codes and industry standards. Anthony works on the engineering portion of our scope of work, which includes the submission of 30/60/90% design drawings to the client as well as coordinating as-builts and traffic control plans. 580 Bell Street Los Alamos CLAD currently provides design services for on- and off-site improvements at a private property located at 580 Bell Street in Los Alamos. Anthony served as the lead engineer on this project, which involved completion of public improvement plans and submittal review. Education Bachelor of Science Civil Engineering Cal Poly San Luis Obispo Anthony Guerrero Lompoc Water Line Replacement Anthony provided engineering services for a water line replacement project for the City of Lompoc. His role included creating and preparing public improvement plans and preparing cost estimates. Anthony also worked with clients, contractors, and municipal staff in the planning, design, plan checking and construction processes. On-Call Civil Engineering Design Services Work Program CLAD understands that the City of San Luis Obispo (City) intends to compile a list of qualified consultants to assist the city with civil engineering services for Capital Improvement Plan work. We acknowledge that the services may include but are not limited to studies, investigations, design, plan review, surveying, scheduling, cost estimating, drainage analysis, or other engineering analysis or reports to support development of project plans and specifications. L. Alberto Lopez will serve as the primary point of contact for this contract. David Chanley, CLAD’s Vice President of Engineering, will oversee all design services completed for City projects. We have multiple senior designers and CAD technicians on staff available to deliver research and design deliverables in a timely manner. Our design team has experience with both public and private improvement design projects, and our principals are well-versed in completing projects in accordance with local municipal design standards. CLAD’s principals have served as client representatives & liaisons for a variety of projects, primarily for public clients. We are familiar and comfortable with attending scope meetings and project kick-off meetings as needed, and we will provide proposals including cost estimates and tentative schedules for each project brought forth by the City. CLAD’s team of professional engineers will also conduct any necessary research prior to commencing design, including visits to the site to better understand project constraints and conditions. David Chanley will lead our design staff to submit plans and complete revisions based on City comments in accordance with a 50/90/100% design schedule for each project. This schedule will be accounted for in each proposal submitted to the City as required. David is also a storm water expert and will effectively assist the City with any storm water control needs that arise in its projects. All plans completed under this contract will utilize the City’s title sheet and additional layout sheets per the RFP. City Responsibilities Based on our experience with public works contracts, when our services are called upon, CLAD expects the City to: provide copies of available reports, studies, plans, and other documentation through the Public Works Department for our reference; designate a City staff member as a contact for each project; act as a liaison with the appropriate decision- making bodies (City Council, Planning Commission, etc.); obtain and make available any project-specific environmental documents and/or studies; cover all permitting fees; obtain permits for creek-related work from relevant regulatory agencies; complete staff reports for approval; advertise and award construction contracts; and arrange and provide construction management services for each project. 13 Attachment A Submittal Forms On-Call Civil Engineering Design Services Page 9 of 21 SECTION D: PROPOSAL SUBMITTAL FORMS ACKNOWLEDGEMENT The undersigned declares that she or he: Has carefully examined Specification No. 5009.2024.CE.
