review and signature. Please provide the name(s), title(s), and contact information of the person(s)
authorized to sign the agreement on behalf of your firm. Note: for corporate entities, the City
requires signatures from two individuals authorized to sign
Additionally, within ten business days, please provide the City with the following documents:
1. Insurance Certificates and Endorsements
2. Copy of your City Business Tax License
3. W-9 Form
