Dear Mr. Jose Quintana:
The City of San Luis Obispo is pleased to award your company the contract for the
“Mission Plaza Enhancements, Specification No. 91439-01”, in the amount not to
exceed $4,230,551.76 approved by City Council on November 12, 2024. You will
receive an email from the Public Works Department, requesting signatures for the
contract via DocuSign. The fully executed contract will be provided when complete.
In the meantime, we are requesting the payment and performance bonds as well as
insurance certificates and endorsements; the City does not maintain standard bond
forms so whichever your Surety provides is acceptable. I would like to call your attention
to the City insurance requirements. Note: Insurance policies must be endorsed to
name as "Additional Insured Parties: The City of San Luis Obispo, its elective
boards, officers, agents and any subcontractors in the performance of work for
the City of San Luis Obispo". An acceptable form is the CG 20 10 11 85 or both
forms of the CG 20 10 07 04 and CG 20 37 07 04. Copies of such endorsements
must be sent with insurance certificates showing the required levels of coverage.
The insurance company must carry a rating of A VII or better, in accordance with the
A.M. Best Rating Guide. Please provide the City at least 30 days prior notice of any
reduction or cancellation of insurance coverage.
The City of San Luis Obispo requires (contractors or companies) doing work within the
City limits to have a business tax certificate. If you do not have a tax certificate, please
contact the Finance Department at (805) 781-7134.
The Finance Department will also need your Federal Tax Identification Number before
payments can be processed to your company. This is standard City practice to allow
compliance with Federal I.R.S. regulations. You can contact the Finance Department
at (805) 781-7135 to provide your tax information.

Please return the required insurance certificates and endorsements, and payment and
performance bonds within ten (10) days, not including Saturdays, Sundays, or legal
holidays, to the address below. The City looks forward to working with you. If you
