Remarks:
E4 responded to above location with a first alarm assignment for a reported structure fire. On arrival E4 assumed Gathe IC
and reported smoke coming from A-side of structure and was in a offensive mode. E4 operator pulled a crosslay and worked
on water supply while E4 FF and Captain performed 360. On arrival, a local citizen was using a garden hose through the window
of the bedroom on the A-D corner of the structure. E4 captain met with RP in front of residence and she stated that the fire
started in bedroom and all occupants were out the residence but her animals were unaccounted for and possibly still inside.
The bedroom she mentioned was the A-D corner of the residence.
E4 completed 360 and determined fire was still contained to the room of origin and that we have a low victim profile. Still be
working in offensive mode and entry will be made on delta side of structure. Entry was not made till 2 out conditions were
met. All units were to switch to city tac 1.
E4 made entry and found a closed bedroom door to the fire room. E4 FF extinguished remaining fire and heat sources within
minutes.
Additional crews were assigned to Division Alpha and helped assist with ventilation, extinguishment, and salvage/overhaul. A
primary and secondary of the residence was performed. The animals that were unaccounted for had all been found and were
with the residents. For more information on each apparatus, see each additional narratives.

BC3 (Gathe IC) released all units on the incident except for E4 to assist with fire investigation and securing the residence. E4
also assisted the residents with gathering some prersonal belongings before leaving the incident.
E4 personnel assisted Investigator Koznek with his investigation.
Servpro arrived on scene and E4 personnel assisted with boarding up window to secure the residence. Both the front and
back doors were locked as well. At the time E4 left the incident everyone had left the incident.
Additional Narrative (#1 of 4 ):
On arrival, I took over Gathe IC from E4. I then assigned E4 fire attack, and primary search, T1 ventilation, and utilities, E2 DIV.
Alph, assist with water supply, and two out, E3 backup fire attack, and E21 to assist occupants (who were out safely) and check
for medical needs. I then requested a working incident.
Once the fire was knocked down, I requested an investigator, and fire investigator Koznick responded.
The crews continued with fully extinguishing the fire and completing a secondary search. The house was all clear.
A board up company was requested and responded to board up one window in the A/D corner
Additional Narrative (#2 of 4 ):
On arrival E-2 was assigned Div. A and assist with water supply. Pump 2 assisted Pump 4 with water supply. Div. A made contact
with E-4 and advised E-4 they were working for Div. A and had FF2 for a crew strength of 3. Div. A completed a 360 noting no
additional hazards. Div. A coordinated ventilation, primary and secondary all clears, and gathered information for the
investigation. E-2 completed the assignment and was released.
Additional Narrative (#3 of 4 ):
Upon arrival T1 was assigned ventilation and utilities, therefore Truck 1 personnel set up mechanical fans for horizontal ppv,
and shut off the utilities to the structure.

Upon arrival E3 was assigned to assist E4 with Fire Attack. E3 face to face with E4, no further FA needed, Given assignment of
2secondary search of structure for animals. All animals accounted for.
