SUBJECT: REQUEST FOR RECORDS DESTRUCTION
On April 24, 2001, the City Council adopted a Records Retention Policy for preservation, protection, and
legal disposition of the City’s Records (Resolution No. 9174, 2001 Series), along with a Records Retention
policy and schedule update process. In accordance with the currently published policy and schedule,
certain records have been identified as eligible for destruction (Attachment A). Also attached are the
relevant sections from the currently published records retention schedule (Attachment B). The method of
destruction disposal will be via recycling or secure destruction.

Agreement between the City and County for Relocation Assistance Services
Associated with 148‐182 Higuera St. Detailed in Permanent Record ‐ Council
Agenda 10/07/1986 Item A‐1 & Council Resolution R6094 (1986 Series).
Agreement notes that it "... shall terminate June 30, 1987, upon completion of
services referred to herein or upon either party giving 30 days written notice
to the other party, whichever occurs first, unless the date of completion is
extended ..." The service was completed as represented physically on‐site but
the exact date is now unknown.

California Mens Colony Work Agreement. The term of the agreement was for
1 year, terminating June 30, 1992. No record exists to determine when this
was terminated but no current staff have knowledge of ever utilizing Mens
Colony inmates for unskilled laborers. Therefore it has been determined that
this agreement has been held for at least the minimum required time
following termination of the agreement.

Consultant Services Agreement with Allan D. Kotin & Associates for Financial
Consultation and Representation services on the impacts of the Dalidio
Project, Countywide voter Initiative Measure J. Term stated , "until acceptance
or completion of said services." Date of acceptance/completion is unknown.
Dalidio project was eventually superceded by the San Luis Ranch project,
adopted by Council Ordinance No 1649, 9/21/2018.

Agenda Report working files
Files created specifically as working or draft copies of eventual Council Agenda Reports should be considered as draft
or copies with the CAR document the final original. Council Agenda Packets have a Permanent retention period.
Therefore the draft and copy documents do not need to be retained.

From time to time, Carahsoft OBO City of San Luis Obispo (we, us or Company) may be
required by law to provide to you certain written notices or disclosures. Described below are the
terms and conditions for providing to you such notices and disclosures electronically through
your DocuSign, Inc. (DocuSign) Express user account. Please read the information below
carefully and thoroughly, and if you can access this information electronically to your

Getting paper copies
At any time, you may request from us a paper copy of any record provided or made available
electronically to you by us. For such copies, as long as you are an authorized user of the
DocuSign system you will have the ability to download and print any documents we send to you
through your DocuSign user account for a limited period of time (usually 30 days) after such
documents are first sent to you. After such time, if you wish for us to send you paper copies of
any such documents from our office to you, you will be charged a $0.00 per-page fee. You may
request delivery of such paper copies from us by following the procedure described below.
Withdrawing your consent
If you decide to receive notices and disclosures from us electronically, you may at any time
change your mind and tell us that thereafter you want to receive required notices and disclosures
only in paper format. How you must inform us of your decision to receive future notices and

Consequences of changing your mind

required notices and disclosures electronically from us and you will no longer be able to use your
DocuSign Express user account to receive required notices and consents electronically from us
or to sign electronically documents from us.

Unless you tell us otherwise in accordance with the procedures described herein, we will provide
