Examples o HIPAA Release Form Printed residential and/or commercial building plans The Public Records Act does not allow the release of printed copies of this material without the permission of the architect/engineer copyright owner.The public records requestor is responsible for obtaining said authorization by completing all three release forms listed below. You may call the Community Development Department at (805) 781- 7170 to find out the name of the copyright owner. California Government Code Section 7921.An Agency shall notify the requestor within 10 days from receipt of request with a Determination which states if the Agency is in possession, in whole or in part, of the requested documents, and possible legal exemptions which prohibit the release of non-disclosable documents, as outlined per the PRA. In some instances, an Agency may require an extension of up to 14 days to provide a Determination, as authorized by the PRA. A notice will be provided to the requestor setting forth the reasons for the extension and the date on which a Determination is expected to be supplied. Code § 6250 et seq.) regarding the matters set forth below. For the purpose of this request, “public record” and “writing” have the meanings defined in Gov. Code §§ 6252(e) and (g), respectively. Any reference to the City of San Luis Obispo (“the City”) includes all employees, officers, officials, agents and contractors of the City. I am hereby requesting that the City promptly make available for inspection the following public records: Any and all documents created, recorded and/ or maintained by the City of San Luis Obispo Department of Building & Safety (“LADBS”) related to the residential property located at APN 003-553-013, which has a street address of 1626 Osos Street, San Luis Obispo CA 93401, from January 1, 2013 to present, including, but not limited to, Certificate(s) of Occupancy, building permits, electrical permits, mechanical permits, plumbing permits, plot plans, site plans, complaint records, Complaint Notification Letters, Notice(s) and Order(s) to Comply, Substandard Building Declaration(s), Notice(s) and Order(s) of Abatement, Inspection Reports, Reinspection Reports, Punch Lists, Code Enforcement Work Logs, Tenant Habitability Plans, correspondence, telephone records, memos, inspector’s notes and/ or hearing records. Please provide the records in electronic format to courtney@elderspencer.com. We understand that individuals’ names and confidential information will be redacted from the records. However, unit numbers should not be redacted off any correspondence or inspection City of San Luis Obispo September 5, 2025 Page 2 reports that were sent to the property owner; the unit numbers have already been shared and are therefore not confidential information. If any public record requested above is to be withheld and not made available for inspection, please state in writing the specific ground(s) for withholding the record. As to each record withheld, if any, please also provide the name and title of the official who made the decision to withhold the record and the name, title and address of the person or body, if any, to whom the decision to withhold the record may be appealed. In addition, please provide information sufficient to identify the record(s), if any, withheld, including the title of the record, the nature of the record (e.g., letter, memo, e-mail, etc.), the number of pages in the record (original or copy), to whom the record was addressed, shown or circulated, the location of the record, and a statement of the subject matter sufficient to enable me and, if necessary, a court, to evaluate the basis for the decision to withhold the record from public inspection.
