While the new fixtures look fine in terms of historic/architectural consistency, you should check with the
Building staff on permit requirements. Unless the wiring already exists to the rear porch mounting
location, the project will need a building permit for the electrical work.
The fixture looks fine. I have approved this as "architecturally insignificant." Go ahead with the
installation.

Mary Beth Schroeder, 2085 Wilding Lane, presented a letter in support of a new senior citizen's
center at I34l Nipomo Street, and also supported the Mills Act Historic Preservation Program.
She voiced concerns that the La Loma de la Nopalera Adobe on Lizzie Street is getting more
dilapidated since the City acquired the property several years ago and asked the Committee to
look into what's being done to preserve and rehabilitate the historic, city-owned adobe.

Adriance Apartments. Aids Support Network, applicant.
Edie Kahn, Executive Director of the Aids Support Network, P.O. Box 12158, San Luis Obispo,
displayed a sample replacement window that ASN was considering using to replace dilapidated
windows at the historic Adriance Apartments. She described rehabilitation work that ASN had
already completed at the apartments, and noted that the windows are "drafty" and in need of
repair or replacement. ASN hopes to replace the existing wood windows with vinyl-clad metal
windows to improve energy efficiency, reduce interior noise, reduce maintenance and stay within
their budget. She introduced Joe Schauerrnan, a window installation contractor retained by ASN,
who would be doing the window improvements. Mr. Schauerman explained the advantages of
replacement over repair and provided performance specifications for the new window. He also
explained the architectural features of the replacement window.
Seeing no further public comment, Chairperson Schrage closed the public hearing

ln response to a request by the applicant, Committee members also supported use of City

The decision of the CHC is final unless appealed to the City Council within 10 days of the
action. Any person aggrieved by a decision of the CHC may file an appeal. After the
appeal period, this determination is final and shall run with the land.

Preseruation Program. She voiced concerns that the La Loma de la Nopalera Adobe
on Lizzie Street is getting more dilapidated since the City acquired the property several
years ago and asked the Committee to look into what's being done to preserue and
rehabilitate the historic, city-owned adobe.

Ihe following matters are expressly excluded from the coverage of this policy
and the Company will not pay loss or damage, costs, attorneys' fees 0r
expenses which arise by reason of:
1. (a) Any law, ordinance or governmental regulation (including but not limited' ' to building and zoning laws, ordinances, or regulations) restricting,
regulating, prohibiting or relating to (i) the occupancy, use, or enioyment
of
- the land; (ii) the character, dimensions or location 0f any
improvement now or hereafter erected on the land; (iii) a separation in
ownership or a change in the dimensions or area of the land or any
parcel of which the land is or was a part; or (iv) environmental
protection, or the effect of any violation of these laws, ordinances or
governmental regulations, except to the extent that a notice of the
enforcement thereof or a notice of a defect, lien or encumbrance
resulting from a violation or alleged violation affecting the land has been
recorded in the public records at Date of Policy.
(b) Any governmental police power not excluded by (a) above, except t0' '
the extent that a notice of the exercise thereof or a notice of a defect,
lien or encumbrance resulting from a violation or alleged violation
affecting the land has been recorded in the public records at Date of
PolicY.
2. Rights of eminent domain unless notice of the exercise thereof has been
recorded in the public records at Date of Policy, but not excluding from
coverage any taking which has occurred prior to Date of Policy which would
be binding on the rights of a purchaser for value without knowledge.
3. Defects, liens, encumbrances, adverse claims or other matters:
(a) created, suffered, assumed or agreed to by the insured claimant;
(b) not known to the Company, not recorded in the public records at Date
of Policy, but known to the insured claimant and not disclosed in writing
to the Company by the insured claimant prior to the date the insured
claimant became an insured under this policy;
resulting in no loss or damage to the insured claimant;
attaching or created subsequent to Date of Policy (except to the extent
that this policy insures the priority of the lien of the insured mortgage
over any statutory lien for services, labor or material or the extent

5, PftOOF OF LOSS ()R DAMAGE.
In additicn to and after the notices required under Section 3 of these
C0nd;ti0ns and Stipulations have been provided the C0mpany, a pr00f 0l
loss or damage signed and sworn t0 by the insured claimant shall be
furnished to the Company within 90 days after the insured claimant shall
ascertain the facts giving rise to the loss or damage. The proof of loss 0r
damage shall describe the defect in, or lien or encumbrance 0n the title,
or other matter insured against by this policy which constitutes the basis
ol loss or damage and shall state, to the extent possible, the basis of
calculating the arnount 0f the Ioss or dama0e. lt the Company is prejudiced
by the failure 0f the insured claimant io provide the required proof ol loss
0r damage, the Company's obligations to the insured under the policy shall
terminate, including any liability or obligati0n to defend, prosecute, or
continue any litioation, with regard to the matter or matters requiring such
proof of loss or damage.
ln additi0n, the insured claimant may reasonably be required t0
submit to examination under oath by any authorized representative of the
Company and shall produce for examination, inspection and copying, at
such reasonable tjmes and places as may be designated by any authorized
rcpresentative 0f the Company, all records, books, Iedgers, checks,
correspondence and memoranda, whether bearing a date before or after
Date of Policy, which reasonably pertain to the loss or damage. Further,
if requested by any authorized representative of the Company, the insured
claimant shall grant its permission, in writing, for any authorized
representative 0f the Company t0 examine, inspect and copy all records,
b00ks, ledgers, checks, conespondence and memoranda in the custody
0r contr0l of a third palry, which reasonably pertain t0 the loss or damage.
All informati0n designated as confidential by the insured claimant provided
t0 the Company pursuant to this Section shall not be disclosed to others
unless, in the reasonable iudgment 0f the Company, it is necessary in the
administration of the claim. Failure 0f the insured claimant to submit for
examination under oath, produce other reasonably requested information
or grant permission t0 secure reasonably necessary lnformati0n flom third
parties as required in this paragraph, unless prohibited by law or
governmental regulation, shall terminate any liability of the Company under
this policy as to that claim.
6. OPIIONS TO PAY OR OTHERWISE SETRE CLAIMS;
TERMINATIOII OF LIABItIW.

(e) resulting in loss or damage which would not have been sustained if the
insured claimant had paid value lor the insured mortgage.
Unenforceability of the lien of the insured mortgage because ol the inability
or failure r,f the insured at Date of Policy, or the inability or failure of any
subsequent owner of the indebtedness, t0 comply with applicable doing
business laws of the state in which the land is situated.
lnvalidity or uneniorceability of the lien of the insured mortgage, or claim
thereof, which arises out of the transaction evidenced by the insured
mortgage and is based upon usury oi any consumer credii protection or
truth in lending law.
Any statutory lien Jor services, labor or materials (or the claim of priority
of any statutory lien for services, labor or materials over the lien of the
insured mortgage) arising from an improvement or work related to the land
which is contracted for and commenced subsequent t0 Date of Policy and
is not financed in whole or in part by proceeds of the indebtedness secured
by the insured mortgage which at Date of Policy the insured has advanced
or is obligated to advance.
Any claim, which arises out of the transaction creating the interest of the
mortgagee insured by this policy, by reason of the operation of federal
bankruptcy, state insolvency, or similar creditors' rights laws, that is based
0n:
(i) the transaction creating the interest of the insured mortgagee being
deemed a fraudulent conveyance or fraudulent transfer; or
(ii) the subordination of the interest of the insured mortgagee as a result
of the application of the doctrine of equitable subordination; or
(iii) the transaction creating the interest of the insured mortgagee being
deemed a preferential transfer except where the preferential transfer
results from the failure:
(a) to timely record the instrument of transfer; or
(b) of such recordation to impart notice to a purchaser for value or a
iudgment or lien creditor.

1. DEFINITIONS OF TERMS.
The following terms when used in this policy mean:
(a) "insured": the insured named in Schedule A. The term
' insured" also includes:
(i) the ov/ner 0f the indebtedness secured by the insured
mortgage and each successor in ownership of the indebtedness except
a successor who is an obligor under the provisions of Section 12(c) of
these Conditions and Stipulations (reserving, however, all rights and
detenses as to any successor that the Company would have had against
any predecessor insured, unless the suicessor acquired the indebtedness
as a purchaser lor value without knowledge 0l the asserted defect, lien,
eflcumbrance, adverse claim 0r other matter insured against by this p0licy
as affecting title to the estate 0r interest ifl the land);
(ii) any governmental agency or governmental instrumentality
which is an insurer or guarantor under an insurance c0ntract 0r guaranty
insuring or guaranteeing the indebtedness secured by the insured
m0rtgage, or any part thereof, whether named as an insured. herein 0r not;
(iii) the padiesdesignated in Section 2(a) 0f theseConditi0nsand
Stipulations.
(b) "insured claimant": an insured claiming l0ss or damage.
(c) "knowledge" or "known": actual knowledge, not constructive
knowledge or notice which may be imputed to an insured by reason of
the public records as defined in this policy 0r any other records which
impart constructive notice of matters affecting the land.
(d) "land": the land described or retened to in Schedule A, and
improvements af{ixed thereto which by law constitute real property. The
term "land" does not include any property beyond the lines of the area
described or refened to in Schedule A, nor any right, title, interest, estate
0r easement in abutting streets, roads, avenues, alleys, Ianes, ways 0r
waterways, but nothing herein shall modify oI limit the extent t0 wllich a
right 0f access t0 and from the land is insuted by this policy.
(e) "mortgage": mortgage, deed 0f trust, trust deed, or other
security instrument.
(f) "public records": records established under state statutes at
Date of Policy for the purpose of imparting constructive notice 0l matters
relating to real property to purchasers for value and without knowledge.
With respect to Section 1 (a)(iv) of the Exclusions From Coverage, "public
rec0rds" shall also include environmental protection liens filed in the
records of the clerk of the United States district court for the district in
which the land is located.
(g) "unmarketability of the title": an alleged or apparent matter
aftecting the title to the land, not excluded or excepted from coverage,
which would entitle a purchaser 0f the estare 0r inrerest described in
Schedule A"0r the irsur'ed mortgage i0 0e released fron the ob igation
i0 ourchase bv virtue of a contractual condition re0uirin0 the deliverv oi
9. RE0UCT|OI{ 0F INSURANCE; REDUCTI0Ii 0RIIRMI}|AT|0N
OF LIABILITY.
(a) All payments under this policy, except payments made for
costs, attorneys' lees and expenses, shall reduce the amount of the
insurance pro tanto. However, any payments made prior t0 the acquisition
ol title t0 the estate 0r interest as provided in Section 2(a) 0f these
C0nditions and Stipulations shall not reduce pro tanto the amount of the
insurance afforded under this policy except to the extent that the payments
reduce the amount 0f the indebtedness secured by the insured mortgage.
(b) Payment ifl part by any person 0f the principal 0f the
indebtedness, or any other obligation secured by the insured mortgage,
or any voluntary partial satisfactlon or release of the insured mortgage,
to the extent ol the paymeflt, satisfacti0n 0r release, shall reduce the
amount of insurance pro tanto. The amount of insurance may therealter
be increased by accruing interest and advances made t0 protect the lien
0f the insured mortgage and secured thereby, with interest thereon'
provided in no event shall the am0unt of insurance be greater than the
amount of insurance stated in Schedule A.
(c) hyment in full by any person 0r the voluntary satisfaction 0r
release ot the insured m0rtgage shall terminate all liability of the Company
except as provided in Section 2(a) of these Conditions and Stipulations.
10. LIABILITY NONCUMULANVE.
lf the insured acquires title t0 the estate 0r interest in satisfaction
0f the indebtedness secured by the insured mortgage, or any part theleoi,
it is expressly understcod that the amount of insurance under this policy
shall be reduced by any amount the Company may pay under any policy
insuring a mortgagi to ivhich exception is taken in Schedule B or t0 which
the insured has agreed, assumed, or hken sublect, 0r which is hereafter
executed by an insured and which is a charge or lien 0n the estate 0r
interest described or refened to in Schedule A, and the amount s0 pald
shall be.deemed a payment under this policy.
11. PAYMEIIT OF LOSS.
(a) No payment shall be made without producing this policy for
end0rsement 0f the payment unless the p0licy has been lost or destroyeo,
in which case prodf'of toss or destructioi shall be furnished t0 the
satisfaction ol the Company.
(b) When liability and the extent 0l loss or damage has been
definitdli tixed in accordance with these Conditions and Stipulations' the
loss or damage shall be payable within 30 days thereatter.

3, N()TICE OF CLAIM TO BE GIVEN BY INSURED
CLAIMANT.
The insured shall notify the Company promptly in writing (i) in case
of any litjoation as set forth in Section 4(a) belnw, (ii) in case knowledge
shall come to an insured hereunder ot any claim of title or interest which
is adverse to the title to the estate or interest 0r the lien of the insured
mortgage, as insured, and which might cause loss 0r damage for which
lhe Company may be liable by virtue of this policy, 0r (iii) if title t0 the
estate 0r interest or the lien of the insured mortgage, as insured, is rejected
as unmarketable. lf prompt notice shall not be given to tfle C0mpany, then
as to the insured all liability of the Company shall terminate r,yith regard
lo the matter 0r maners l0r which pr0mpt notice is required; provided,
however, that failure to notify the Company shall in n0 case prejudice the
rights ol any insured under this policy unless the Company shall be
prejudjced by the tailure and then only t0 the extent 0f the prejudjce,
4. DEFENSE AND PROSECTJT1OII OF ACTIONS; DUTY OF
II{SURED CLAIMANT T() C()OPEMII.
(a) Upon written request by the insured and subiecl t0 the options
contained in Section 6 ol these Conditions and Stipulations, the Company,
at its own cost and without unreasonable delay, shall provide for the
defense of an insured in litigation in which any third party asserts a claim
adverse t0 the title 0r inlerest as insured, but 0nly as to those stated causes
of action alleging a defect, lien or encumbrance 0r other matter insured
against by this policy. The Company shall have the right t0 select counsel
of its choice (subject to the right 0f the insured to object for reasonable
cause) lo represent the insured as to those slated causes of action and
shall not be liable for and will not pay the fees of any other c0unsel. The
Company will not pay any iees, costs or expenses incurred by the insured
in the delense of those causes of action which allege maners not insured
against by this policy.
(b) The Company shall have the righl at its own cost, t0 institute
and prosecute any action or proceeding or to d0 any other act which in
its opinion may be necessary or desirable to establish the title t0 the estate
or interest 0r the lien of the insured mortgage, as insured, or t0 prevent
0r reduce loss or dimage to the insured. The Company may take any
appropriale action under the terms 0f this policy, whether or not it shall
be liable hereunder, and shall not thereby concede liability or waive any
provision 0f this policy. lf the C0mpany shall exercise its riOhts under this
paragraph, it shall do s0 diligenlly.
(c) Whenever the Company shall have brought an action 0r
inlerposed a defense as required or permitled by the provisions of this
policy, ttre Company may pursue any litigation t0 final determination by
a court 0f competent jurisdiction and expressly reserves the right, in its
sole discretion, t0 appeai from any adverse judgment or order.
(d) ln all cases where this policy permits 0r requires the Company
t0 prosecute or provide for the defense of any action 0r proceeding, the
insured shall secure to the Company the right t0 s0 prosecule or provide
delense in the action 0r proceeding, and all appeals therein, and permit
lhe C0mpany to use, at its option, the name of the insured for this purpose.
Whenever requested by the Company, the insured, at the Company's
expense, shall 0ive the Company all reasonable aid (i) in anyection 0r

Purchase the lndebtedness.
(i) to pay or tender payment 0f the amount of insurance under this
policy toOether with any costs, attorneys'lees and expenses incurred by
the insured claimant, which were authorized by the Company, up to the
time of payment or tender of payment and which the Company is obligated
to pay; or
(ii) to purchase the indebtedness secured by the insuied
m0rtgage f0r lhe amount owing thereon together with any costs, ailorneys'
fees and expenses incurred by the insured claimant which were authorized
by the Company up to the time of purchase and which the Company is
obligated to pay.
lf the C0mpany offers to purchase the indebtedness as herein
provided, the owner 0l the indebtedness shall transfer, assign, and convey
the indebtedness and the insured mortga0e, together with any c0llateral
security, t0 the Company upon payment therefor.
Upon the exercise by the Company of either 0f lhe options provided
for in paragraphs a(i) or (ii), all liability and obligations to the insured undei
this policy, other than to make the payment required in those paragraphs,
shall terminate, including any liability or obligation to defend, prosecute,
or continue any litigation, and the policy shall be surrendered to the
Company for cancellation.
(b) T0 Pay or 0therwise Settle With hrties other than the Insured
or With the lnsured Claimant.
(i) to pay or otheMise settle with other parties for or in the name
o{ an insured claimant any claim insured against under this policy, together
with any costs, attorneys' fees and expenses incuned by the insured
claimant which were authorized by the C0mpany up t0 the time of payment
and which the Company is obligated to pay: 0r
(ii) t0 pay or otheMise settle with the insured claimant the loss
or damage provided for under this policy, together with any costs,
attorneys' fees and expenses incuned by the insured clalmant which were
authorized by the Company up t0 the time 0f payment and which the
Company is obligated to pay.
Upon the exercise by the Company 0f either of the options provided
l0r in paragraphs b(i) or (ii), the Company's obligations t0 the insured
under this p0licy l0r the claimed loss 0r damaoe, other than the payments
required to be made, shall terminate, including any liability or obligation
t0 defend, prosecute or continue any Iitigation.
7. DETERMII{ATION AND EXTENT OF LIABILITY.
This pollcy is a c0ntract 0f indemnity against actual monetary loss
or damage suslained or incurred by the insured claimanl who has sutlered
loss or damage by reason ol maners insured against by this policy and
only to the extent herein described.
(a) The liability of the Company under this policy shall not exceed
the least of:
(i) the amouflt ot insurance stated in Schedule A, or, if
applicable, the amount of insurance as defirred in Section 2(c) 0f these
Conditions and Stipulations;
(ii) the amount of unpaid principal indebledness secured by the
insured mortgage as limited or provided under Section 8 ol these
Conditions and Stipulations or as reduced under Sectjon 9 of these
Conditions and Stipulations, at the time the loss or damage insured against
by this policy occurs, together with interest thereon; or
(iii) the ditlerence between the value of the insured estate or
interest as insured and the value of the insured estate 0r inlerest suhject
to the defect, lien or encumbrance insured against by this policy.
(b) ln the event the insured has acquired the estate 0r interest in
the manner described in Section 2(a) 0f these C0nditions and Stipulations
0r has conveyed the title, then the liability 0f the Company shall continue
as set forth in Section 7(a) 0l these Conditions and Stipulations.
(c) The Company will pay only those costs, attorneys' fees and
expenses incurred in accordance with Secti0n 4 0f rhese Conditions and
Stipulations.
8. LIMITATION OF IIABIIITY.
(a) lf the Company establishes the title, 0r removes the alleged
defect, lien 0r encumbrance, or cures the lack of a right of access to 0r
from the land, or cures the claim 0l unmarketability of title, or otherwise
establishes the lien of the insured mortgage, all as insured, in a reasonably
dili0ent manner by any method, including litigati0n and the completion of
any appeals therekom, it shall have fully performed its obligations with
respect t0 that matter and shall not be liable lor any loss or damage caused
thereby.
(b) ln the eventof any litigation, including litigation bythe Company
or with the Company's consent, the C0mpany shall have no liability for loss
or damage until there has been a final determination by a c0urt ol
compelent jurisdicti0n, and disposition of all appeals therefrom, adverse
to the title or to the lien ol the insured mortgage, as insured.
(c) The Company shall not be liable for loss 0r damage to any
insured for liability voluntarily assumed by the insured in setiling any claim
0r suit-without the prior written consent 0f the Company.
(d) The Company shail not be tiabte for:
(i) any indebtedness created subsequent to Date ol policy except
for advances made to protect the lien 0f the insured mortgage andiecured
thereby and reasonable amounts expended to preveni diterioration of
improvements; or
(ii) construction loan advances made subsequent to Date ol
folicy, except construction loan advances made subsequent to Date ol
Policy lor the purpose ol linancing in whole or in part the construction of
an improvemenl l0 the land which at Date of policv were secured bv the
'nsured m0rlgage and which the insured was ano coirirreo ioie oiri,jaieot0 advance al and afier Date 0i policy.
The Company shall be subrogated to and be entitled t0 all rights
and remedies v/hich {he insxred, claimant w0uld have had against any
person or property in respect t0 the claim had this p0licy not been issued.
lf requested by the Company, the insured claimaot shall transler tQ the
Company all rights and remedies against any person or property
necessary in order to pertect this right ol subrogation. lhe insured
claimant shall permit the Company to sue, compromise or setlle in the
name of the insured claimant and t0 use the name of the insured claimanl
in any transaction or liligatjon involving these rights or remedies.
lf a payment on account of a claim does not tiilly cover the loss
0l the insured claimant, the Company shall be subrogated to all rights and
remedies 0f the insured claimant after the insured claimant shall have
recovered its principal, interest, and costs of collection.
(b) The lnsured's Ri0ftts and Limitations.
Notwithstanding the foregoing, the owner ot the indebtedness
secured by the insured mortgage, provided the priority ol the lien ol the
insured mortoage or its enforceability is not affected, may release or
substitute the personal liability ol any debtor or guarantor, or extend or
otherwise modify the terms ol payment, or release a portion ol the estate
0r interest from the lien ol the insured mortgage, or release any collaterai
security for the indebtedness.
When the permitted acts ol the insured claimant occur and the
insured has knowledge of any claim 0l title or interest adverse to the title
to the estate or interest 0r the priority or entorceability of the lien ol the
insured morigage, as insured, the Company shall be required to pay 0nly
that part of any losses insured against by lhis poiicy which shall exceed
the amount, if any, lost t0 the Company by reason ol the impairment by
the insured claimant of the Company's right 0f subrogation.
(c) Ihe Company's Righls Againsl l{on-insured obligors.
The Company's right 0f subrogation against n0n-insured obligors
shall exist and shall include, without limitation, the rights of the insured
t0 indemnities, guaranties, other policies of insurance 0r b0nds,
notwithstanding any lerms or conditions c0ntained in those instruments
which provide for subrogation rights by reason 0f this policy.
The Company's riqht ol subrogation shall not be avoided by
acquisition ol the insured mortgage by an obligor (except an obligor
described in Secti0n 1 (aXii) of these Conditions and Stipulations) who
acquires the insured mortgage as a result ol an indemnity, guarantee, other
policy ol insurance, 0r bond and the obligor will not be an insured under
this policy, notwithstandlng Section 1(axi) 0l these Conditions and
Stipulations.
13. ARBIMTION.
Unless prohibited by applicable law, either the Company or the
insured may demand arbitrati0n pursuant to the Title lnsurance Arbitration
Rules of the American Arbitration Association. Arbitrable matters may
include, but are not limited to, any controversy 0r claim between the
Company and the insured arising out o{ or relating to this policy, any
service of the Company in conneclion with its issuance or the breach ol
a policy provision 0r olher obligation. All arbitrable matlers when the
Amount 0f lnsurance is $1 ,000,000 or less shall be arbitrated at the opli0n
0l either the Company or the insured. All arbitrable matters when the
Amount of lnsurance is in excess 0f $1,000,000 shall be arbitrated only
when agreed to by both the Company and the insured. Arbitration pursuant
t0 this policy and under the Rules in effect on the date the demand {0r
arbitration is made 0r, at the option of the insured, the Rules in eltect at
Date 0f hlicy shall be bindino upon the parties. The award may include
atlorneys' fees only if the laws of the state in which the land is located
permit a court to award attorneys'fees t0 a prevailing party. Judgmenl
upon the award rendered by the Arbitrator(s) may be entered in any court
having jurisdiction thereof.
The laws of the situs of the land shall apply to an arbitration unde
the Title Insurance Arbitrati0n Rules.
A copy of the Rules may be obtained from the Company upon
request.
14. LIABIL]W LIMITED TO THIS POLICY;
POTICY EMIRE COI{TRACT.
(a) Ihis policy together with all endorsements, il any, attached
hereto by the Company is the entire policy and contract between the
insured and the Company. ln inierpreting any provision ofthis policy, this
policy shall be construed as a whole.
(b) Any claim of loss or damage, whether or not based on
negligence, and v/hich arises out of the status of the lien ol the insured
mortqage or of the title to the estate or interest covered hereby or by any
action asserting such claim, shall be restricted to this policy.
(c) No amendment o{ or endorsement lo this policy can be made
except by a writing endorsed hereon or atlached hereto signed by either
the President, a Vice President, the Secretary, an Assistant Secretary, 0r
validatino ofiicer or authorized sigflatory 0f the Company-
15. SEVERABITITY.
ln the event any provision 0t this policy is held invalid 0r
unenforceable under applicable laril, the policy shall be deemed not to
include that provision and all other provisions shall remain in full force
and effect.
16, NONCES, WHERE SENT.
All notices required lo be given the Company and any statement
in writing required to be furnished the Company shall include the number
0f this policy and shall be addressed t0 the Company at 114 East Fifth
Street, Santa Ana, California 92701, or to the otfice which issued this
