SUBJECT: City of San Luis Obispo Injury and Illness Prevention Program
CAO RECOMMENDATION: Adopt a resolution establishing an Injury and Illness
Prevention Program
DISCUSSION:
BACKGROUND
The legislature adopted Senate Bill 198 (SB 198) which requires that effective July 1, 1991, all
California employers, regardless of size, adopt an Injury and Illness Prevention Program.
Employers failing to do so can suffer stiff financial penalties. The City of San Luis Obispo has
had a long-standing safety program which encompassed many of the requirements of SB 198.
However, the new legislation required development of new inspection and training forms and
Codes of Safe Practice for City operations. The City has been meeting the requirements of SB
198 since July, with the exception of having written Codes of Safe Practice. These Codes have
now been completed and allow the full Injury and Illness Prevention Program to be presented
for adoption.
PROGRAM OVERVIEW
SB 198 provides specific guidelines as to the required content of the City's Injury and Illness
Prevention Program. The program must identify who is responsible for implementing and
monitoring the program; provide for identifying and evaluating workplace hazards;, identify
methods and procedures for correcting unsafe practices and conditions; implement a safety
training program for all employees specific to each job assignment; provide a method for
communicating with employees on safety matters; and create a system for insuring employees
comply with safety practices. The Injury and Illness Prevention Program presented for adoption
meets all the requirements of the law.
In addition to creating a written Program, the City has worked to revitalize the employee Safety
Committee so that it will able to support the Program and assure that it is effective.
CONCURRENCES:
The Management Team is aware of the requirements of SB 198 and has seen that the
components of the program are already in place. The City's Safety Committee has reviewed
the Program and found it satisfactory.
FISCAL IMPACT:
The Written Injury and Illness Program should not cause any significant increase in the City's
safety program expense, since it mainly formalizes existing safety practices. However, failure
to adopt the Program could put the City at risk for substantial fines.

ALTERNAITM:.
1. Rewrite the proposed Program. The Safety Committee feels that the program as
presented meets all the requirements of SB 198 and incorporates our existing safety
practices.
2. Fail to adopt an Injury and Illness Prevention Program. If the City does not adopt a
program it is in violation of the law and subject to substantial fines.

RESOLUTION OF THE COUNCIL OF THE CITY OF SAN LUIS OBISPO
ESTABLISHING AN INJURY AND ]ILLNESS PREVENTION PROGRAM
WHEREAS, the Legislature of the State of California passed Senate Bill 198 which
requires all employers to have a written Injury and Illness Prevention Program, and
WHEREAS, the City of San Luis Obispo recognizes the need to provide its employees
with a safe working environment, and
WHEREAS, the City of San Luis Obispo also recognizes its responsibility to ensure that
all its employees receive training in safe working practices, and
WHEREAS, the City of San Luis Obispo acknowledges its responsibility to comply with
the requirements of Senate Bill 198
NOW, THEREFORE, BE IT RESOLVED, the City Council of the City of San Luis
Obispo does hereby establish an Injury and Illness Prevention Program.

Completion of this form indicates that it is the policy of the employer.to fully comply
with Labor Code Section 6401.7 (SB 198) and General Industry Safety Order Section
3202, Injury and Illness Prevention Program.

III. (A) THE CITY'S SYSTEM FOR " IDENTIFYING, EVALUATING AND
PREVENTING OCCUPATIONAL SAFETY AND HEALTH HAZARDS
INCLUDES THE FOLLOWING:
• Review of applicable General Industry Safety Orders and other Safety
Orders that apply to the operation.
• Review of industry and general information (including Material Safety
Data Sheets for chemicals used) on potential occupational safety and
heath hazards.
• Investigation of all accidents, injuries, illnesses and unusual events that
have occurred at all City locations.
• Evaluation of information provided by employees.
(B) SAFE WORKING CONDITIONS, WORK PRACTICES AND PROTECTIVE
EQUIPMENT REQUIREMENTS ARE DOCUMENTED AND
COMMUNICATED IN THE FOLLOWING MANNER:
• Codes of Safe Practices have been developed for general and/or specific
job safety classes or workstations. (Attachment A). They are
maintained at each work location and in the Personnel Department.
• All employees are provided with a Safety Manual which covers safe
working conditions, work practices and protective equipment
requirements.

IV. INSPECTIONS ARE CONDUCTED TO VERIFY COMPLIANCE WITH CODES OF
SAFE PRACTICES AND OTHER SAFETY REQUIREMENTS TO IDENTIFY ANY
ADDITIONAL HAZARDS AND TO INVESTIGATE ACCIDENTS, INJURY AND
ILLNESS CASES AND UNUSUAL OCCURRENCES:
Frequency and Responsibility for Inspections
• All work locations are inspected monthly. Assigned inspectors are
indicated on Attachment B.
Documentation of Inspections
• Monthly inspections are documented on the appropriate inspection form
as shown in Attachment C.
• These forms are maintained in the Personnel Department.
Accident and Injury/Illness Investigation
• Investigations are conducted as soon as possible after an accident,
occupational injury or illness, or hazardous condition is reported.
• Investigations of occupational injury or illness are documented on the
Supervisor's Accident Investigation Report form in the Workers'
Compensation packet. (Attachment D).
• Investigations of non-injury accidents are documented on the
Supervisor's Report of an Accident. (Attachment E).
• Investigations of hazardous conditions are documented on the Hazard
Report Form. (Attachment F).
• All completed forms are maintained in the Personnel Department.
V. EMPLOYEE SAFETY TRAINING IS PROVIDED INITIALLY OR IN THE
FOLLOWING CIRCUMSTANCES:
• Initial training for all current employees upon establishment of this
program or prior to July 1, 1991.
• New employees are provided initial training upon hiring prior to
assignment.
• Employees are provided training when assigned to a new task for which
training has not been received.
• Supervisors are trained on hazards and safe practices in their area of
responsibility.
• Training includes general area safety and specific assignment or job
safety class training, and the potential occupational safety and health

hazards and the Code of Safe Practices for the area.
• Documentation of training is maintained on the Employee Safety
Orientation Program form. (Attachment G). This documentation is
maintained in each employee's department.
• Refresher training is provided annually or as needed.
VI. EFFECTIVE COMMUNICATIONS WITH EMPLOYEES HAVE BEEN ESTABLISHED
WHICH INCLUDE THE FOLLOWING METHODS TO MEET THE STANDARD'S
REQUIREMENTS:
• Communication of safe working conditions, work practices, and required
personal protection equipment is included in initial and all subsequent
training.
• Safety Tailgate Sessions are held every two weeks by safety
representatives for employees who work in potentially hazardous jobs.
Tailgate Sessions are held for clerical/administrative employees monthly.
Records of Tailgate Sessions are documented on the Safety Training
form (Attachment H) and maintained in the Personnel Department.
• Quarterly Safety Feeds are open to all employees. A safety program is
presented as part of the Feed.
• Employees are advised through the City's Safety Manual that safe work
conditions, safe work practices, and required personal protective
equipment are mandatory and will be enforced by the following:
• A recognition program for compliance/good safety record as
determined by the City's Safety Committee.
• Discipline for non-compliance as outlined in the City's Safety
Manual.
• Employees may provide information on unsafe conditions to their
supervisors or the risk manager by using the Hazard Report form.
(Attachment F). Other safety issues or concerns may be forwarded to
the City's Safety Committee by means of the employee's safety
representative.
• Employees have been advised there will be no reprisals or other job
discrimination for expressing any concern, comment, suggestion or
complaint about a safety-related matter.
VII. RECORDKEEPING REQUIREMENTS OF 8 CCR SECTION 3202(d) WILL BE
ADHERED TO, INCLUDING:
• Maintenance of all written records for three years unless required by law
to maintain longer.
