Inspection Notes:SLO Fire Department performed an Annual Fire and Life Safety Inspection at 280 California
Boulevard on 10/21/2025 at 11:00 AM with fraternity house manager Spencer Levin.
The following items shall be addressed by property management:
1. Replace missing or non-functioning smoke alarms and/or CO alarms (see attached photo).
Note: The building owner shall be responsible for maintaining the fire and life safety systems in an
operable condition at all times per (CFC 907.8.4).
2. Emergency lighting was found not working in the living room and hallway near the kitchen area.
Please contact a California licensed C-10 contractor to perform maintenance and repair of all
emergency lighting per 1008.1 Means of Egress Illumination (see attached photo).
3. Electrical wire exposed due to the removal of the old smoke/CO alarm located in the hallway near
the kitchen area. Please install an approved cover over the exposed electrical wiring (see attached
photo).
Note: Open junction boxes and open-wiring splices shall be prohibited. Approved covers shall be
provided for all switch and electrical outlet boxes per (CFC 603.2.2 Open Electrical Terminations).
4. Please remove all the accumulation of combustible waste and rubbish found at the front entry of
the parking lot (see attached photos).
Note: Accumulations of wastepaper, wood, hay, straw, weeds, litter or combustible or flammable
waste or rubbish of any type shall not be permitted to remain on a roof or in any court, yard, vacant
lot, alley, parking lot, open space, or beneath a grandstand, bleacher, pier, wharf, manufactured
home, recreational vehicle or other similar structure per CFC 304.1.1 Waste Material.

Note: The portable fire extinguisher will be due for an annual service before 11/21/2025. Please have
all expired fire extinguishers on site serviced by a California state-licensed extinguisher contractor,
and continue to have them serviced annually.
Note: Please continue to check laundry dryer vents regularly
to avoid the accumulation of combustible debris.

The following items require corrective action:
(1) Please replace all unit number markings on the front doors of each dwelling unit for clear
identification.
(2) Please replace/repair any missing/not working smoke alarms found in each bedroom/hallway

Note: A liquefied petroleum gas (LPG) tank was found stored inside the living room. The occupant
was directed to remove it immediately, and the tank was promptly relocated outdoors. LPG
containers shall be stored outdoors in a well ventilated area, kept upright, protected from heat and
physical damage, and not stored inside buildings or near ignition sources.
The following items require corrective action:
(1) The fire alarm control panel is displaying a trouble signal related to the kitchen duct. Please
contact a licensed fire alarm service contractor to inspect and maintain the system as required.
(2) Multiple exposed wires were observed hanging from the living room ceiling. Additionally, a light
fixture in the carport was found broken with exposed wires. Please have a qualified electrician secure
and properly terminate all wiring to prevent electrical hazards.
(3) Trash and debris were observed in the carport and along the fence line (including a mattress and
overflowing trash cans). Please dispose of trash regularly to avoid accumulation and remove any
discarded furniture or materials not in use.

During my inspection of 1304 East Foothill Ave. I found the following deficiencies:
The fire extinguisher located outside of apartment one has lost its Charge. Please have a licensed fire
extinguisher technician come out and service the unit prior to the next inspection.
Please ensure that all water heater closets are free of combustable storage.
Apt 1 was missing 3 smoke alarms.
Apt 2 was missing the smoke alarm in the living room and in the hallway. Additionally, combustible
storage needs to be cleared out of the water heater closet.
Apt 3 was missing a smoke alarm in the bedroom.
Apt 4 was missing the smoke alarms in the hallway & living area.
Apt 5 was missing a smoke alarm in the bedroom above the garage.
The carbon monoxide detector in the living room of the main house needs new batteries.
Additionally, I found a buildup of mold in the ceiling of the laundry room. Together with the tenants, I
took apart the dryer vent, shortened the dryer vent run, straightened the tube and reattached it. It
still requires cleaning of the buildup in the tube as well as annual service.

Fire alarm control panel was found in an impaired condition (i.e. communication failure) during
inspection and unable to determine if remote annunciation panel (main entrance) is currently
operational. Junction box with low-voltage electrical wiring was found uncovered at the front gate
and will require repair (CFC 907.8).
Property management is directed to coordinate repair of the items noted above and provide a
current fire alarm monitoring agreement for this property.
Any modification(s) to the fire detection system (i.e. cellular communication or panel upgrade) will
require a construction permit from the City Building Department prior to installation (CFC 105.6.6).
Note: Unable to inspect dwelling units onsite due to prior occupancy. Property management has
provided a smoke alarm inspection log for all dwelling units onsite and has been included in this
report.
Note: Wallboard behind water heater appliance inside the laundry room was found damaged during
inspection. See attached records. Property management is requested to repair (or replace) wallboard
in this location.
Note: Combustible materials (i.e. plastic tarp and clothing) were found in direct contact with the
dryer motor and the exhaust vent during inspection. Tenants are requested to remove storage in this
location (CFC 610.1.2).

metal storage cabinet in an exterior location (CFC 315.3.3 Equipment Rooms).
Note: Dead grass noted on the north facing side of the property will be required to be mowed. As a
general reminder the City of SLO weed abatement deadline was 06/10/25. Property management is
requested to coordinate the removal of excess storage in this location and coordinate mowing of
vegetation (CFC 304.1.2).
Note: Temporary electrical wiring (i.e. extension cord) was found plugged into an electrical power
strip on the back patio of Unit No.11. Portable lighting was found plugged into the power strip and
the extension cord was found with damaged electrical wiring.
Extension cord was unplugged by tenants during inspection. The tenants have been directed to
discontinue use of this extension cord and plug all temporary lighting into an electrical outlet (CFC
603.2).
Note: Portable (green) extension cord located at the main entrance was determined to be non-
operational during inspection. See attached records.
Note: Recommend the installation of an exterior enclosure or cabinet on the west facing side of the
building to protect cable line(s) and associated equipment from mechanical damage and or adverse
weather conditions.
Note: Emergency contact information and gate access code was updated for this property during
inspection and secured inside the existing Knox-box.
Note: Notice of Violation has been issued by the City Code Enforcement Division on 03/19/2025 for

Note: Property management is directed to address the following items for this property:

Vegetation.
Weeds, grass, vines or other growth that is
capable of being ignited and endangering
property, shall be cut down and removed by
the owner or occupant of the premises.

soil and prevent erosion.
(3)Remove that portion of any tree which

The following items were evaluated and found to be corrected during this inspection:
1) Electrical plug for the water heater appliance was found partially unplugged during inspection.
Tenant has properly plugged the appliance into an existing wall outlet and this appliance appears to
be operational.
Yellow "caution" tag was found affixed to the water heater appliance cord; property management
was notified following inspection at approximately 14:08 hours and requested to follow-up on this
item prior to the close of business.
*** Update: Water heater appliance was found plugged into a wall outlet and property management
has conducted additional inspection of this appliance to further address the conditions noted above.
2) Electrical wiring found exposed inside carport (space No.1) during inspection. Property
management is requested to provide an electrical cover plate in this location

The following items were evaluated and will require additional correction for this property:
3) Excess storage (i.e. mattress, trash, etc.) were found along south facing side of the property.
Tenants have been requested to clean up and properly dispose of storage with the local sanitation
district.
***Update: Trash and combustible storage has been cleaned up by tenants since previous inspection
(06/25/25). Additional cleaning has been requested for the northwest corner of the property.
In addition, a portable gas container was found stored under the northeast stairs. Tenants have been
directed to remove the gas can from the property or store inside a metal storage cabinet outside of
the building.
4) Panic hardware on the east facing exit (i.e. parking lot) was found secured with plastic zip tie(s)
during inspection. Per tenants, this action has been taken to prevent unauthorized access into the
property.
Tenants have been directed to remove zip-ties from exit hardware by the close of business on
07/09/25 and coordinate repair (or replacement) of existing panic hardware with the property
management (CFC 1032.2 Reliability).
Property management has been requested to provide additional photographic documentation upon
the completion of this item.
5) Coordinate repair of the fire alarm system and provide a current record of fire alarm monitoring
agreement.
***Update: Fire alarm control panel was found to be in an operable condition during inspection.
Updated fire alarm monitoring is pending from the fire alarm monitoring service.
Any modification(s) to the fire detection system (i.e. cellular communication or panel upgrade) will
require a construction permit from the City Building Department prior to installation (CFC 105.6.6).

The leaking roof has been repaired with sandbags and tarps. While this is not a Fire code violation, I
found it noteworthy enough to include in the report.
There is an open electrical conduit box that needs a cover. It is located between the gate and the
wash rack area.
The smoke alarms are missing from the following rooms.

Inspection Notes:SLO Fire Department performed an Annual Fire and Life Safety Inspection at 654, 658, and 660
Graves (Zeta Beta Tau) on 10/16/2025 at 1:00 PM with House Representative Russell Williams.
The following items require corrective action:
(1) Please ensure that each unit is equipped with a minimum 2A:10BC rated fire extinguisher that is
mounted, visible in the kitchen area, and serviced annually by a certified fire extinguisher contractor
in accordance with California Fire Code (CFC) 906.2. Please note, if a new fire extinguisher(s) is
purchased to meet the inspection requirements, please provide a copy of the purchase invoice for
verification and recordkeeping.
654 Graves: Fire extinguisher was found not tagged and not mounted and is currently stored in the
garage. Please have the extinguisher serviced, tagged, and properly mounted in a visible location
within the kitchen area.
658 Graves: No fire extinguisher was found on site. Please provide a 2A:10BC fire extinguisher in the
kitchen area, mounted and tagged per code.
660 Graves: A 1A-rated extinguisher was found on site; however, the minimum required rating is
2A:10BC. Please replace the existing extinguisher with the appropriate model and ensure it is
mounted and tagged.
(2) The fire sprinkler system has not been serviced within the past 12 months. Please schedule an
annual inspection, testing, and maintenance of the sprinkler system by a licensed C-16 contractor in
accordance with CFC 901.6.
(3) Dryer vents were found in need of cleaning. Please clean all dryer vents to remove accumulated
lint and debris and maintain them regularly to prevent fire hazards per CFC 603.6.

906.2 General requirements.
Portable fire extinguishers shall be selected,
installed and maintained in accordance with
this section and California Code of
Regulations, Title 19, Division 1, Chapter 3.
Exceptions:
1.The distance of travel to reach an
extinguisher shall not apply to the spectator
seating portions of Group A-5 occupancies.
2.Thirty-day inspections shall not be required
for portable fire extinguishers that are
supervised by a listed and approved
electronic monitoring device, provided that
all of the following conditions are met:
2.1.Electronic monitoring shall confirm that
extinguishers are properly positioned,
properly charged and unobstructed.
2.2.Loss of power or circuit continuity to the
electronic monitoring device shall initiate a
trouble signal.
2.3.The extinguishers shall be installed inside
of a building or cabinet in a noncorrosive
environment.
2.4.Electronic monitoring devices and
supervisory circuits shall be tested when
extinguisher maintenance is performed.
2.5.A written log of required hydrostatic test
dates for extinguishers shall be maintained by
the owner to verify that hydrostatic tests are
conducted at the frequency required by
California Code of Regulations, Title 19,
Division 1, Chapter 3.
3.In Group I-3, and in mental health areas of
Group I-2, portable fire extinguishers shall be
permitted to be located at staff locations.
[California Code of Regulations, Title 19,
Division 1, §565.1(a) through (c)] Classification
of Hazards.
(a) Light (Low) Hazard. Locations where the
total amounts of Class A combustible
materials, including furnishings, decorations
and contents, is of minor quantity. These shall
include buildings or rooms occupied as
offices, classrooms, churches, assembly halls,
etc. This classification anticipates that the
majority of the contents are either
noncombustible or so arranged that a fire is
not likely to spread rapidly. Small amounts of
Class B flammables used for duplicating
machines, art departments, etc., are included
provided that they are kept in closed
containers and safely stored.
(b) Ordinary (Moderate) Hazard. Locations
where the total amounts of Class A
combustibles and Class B flammables are
present in greater amounts than expected
under Light (Low) Hazard occupancies. These
occupancies could consist of offices,
classrooms, mercantile shops and allied
storage, light manufacturing, research
operations, auto showrooms, parking
garages, workshop or support service areas of
Light (Low) Hazard occupancies, and
warehouses containing Class I or Class II
commodities.

extinguishers equipped with metal horns are
not considered safe for use on fires in
energized electrical equipment and,
therefore, are not classified for use on Class C
hazards.
(e) Extinguishers and extinguishing agents for
the protection of Class D hazards shall be of
types approved for use on the specific
combustible metal hazard.
[California Code of Regulations, Title 19,
Division 1, §566(a) through (f )] Application for
Specific Hazards.
(a) Class B Fire Extinguishers for Pressurized
Flammable Liquids and Pressurized Gas Fires.
Fires of this nature are considered to be a
special hazard. Class B fire extinguishers
containing agents other than dry chemical are
relatively ineffective on this type of hazard
due to stream and agent characteristics.
Selection of extinguishers for this type of
hazard shall be made on the basis of
recommendations by manufacturers of this
specialized equipment. The system used to
rate extinguishers on Class B fires (flammable
liquids in depth) is not applicable to these
types of hazards. It has been determined that
special nozzle design and rates of agent
application are required to cope with such

to extinguish this type of fire unless there is
reasonable assurance that the source of fuel
can be promptly shut off.
(b) Fire extinguishers provided for the
protection of cooking appliances that use
combustible cooking media (vegetable or
animal oils and fats) shall be listed and
labeled Class K fires.
(c) Three-dimensional Class B Fires. A three-
dimensional Class B fire involves Class B
materials in motion such as pouring, running
or dripping flammable liquids and generally
includes vertical as well as one or more
horizontal surfaces. Fires of this nature are
considered to be a special hazard. Selection of
extinguishers for this type of hazard shall be
made on the basis of recommendations by
manufacturers of this specialized equipment.
The system used to rate extinguishers on
Class B fires (flammable liquids in depth) is
not directly applicable to this type of hazard.
(d) Water Soluble Flammable Liquid Fires
(Polar Solvents). Extinguishers used for the
protection of water soluble flammable
liquids, such as alcohols, acetones, esters,
ketones, etc., shall be selected in accordance
with Section 565.2. AFFF-type and FFFP-type
fire extinguishers shall not be used for the
protection of water soluble flammable
liquids, such as alcohols, acetone, esters,
ketones, etc., unless specifically referenced
on the extinguisher nameplate.
(e) Electronic Equipment Fires. Extinguishers
for the protection of delicate electronic
equipment shall be selected from the
following: carbon dioxide type or a
halogenated agent type, or a distilled water
mist type with a minimum Class A:C Rating.
(f ) In patient care areas and sleeping rooms of
health care facilities, fire extinguishers,
including the agents and expelling means,
should be selected and utilized that would
not be detrimental to patients and are
appropriate for the type of fire expected,
such as distilled water mist type fire
extinguishers with a minimum 2-A:C rating.
[California Code of Regulations, Title 19,
Division 1, §567.8] Installation Temperatures.
Water-type (water, AFFF, FFFP) extinguishers
shall not be installed in areas where
temperatures are outside the range of 40°F to
120°F (4°C to 49°C). Other types shall not be
installed in areas where temperatures are
outside the range of -40°F to 120°F (-40°C to
49°C). Fire extinguishers shall not be exposed
to temperatures outside of the range shown
on the fire extinguisher label.
Exceptions:
1. Where fire extinguishers are installed in
locations subject to temperatures outside
these ranges, they shall be of a type approved
and listed for the temperature to which they
are exposed, or they shall be placed in an
enclosure capable of maintaining the
stipulated temperature range.

only can be protected to temperatures as low
as -40°F (-40°C) by the addition of an
antifreeze stipulated on the extinguisher
nameplate. Calcium chloride solutions shall
not be used in stainless steel fire
extinguishers.

listed for use at temperatures as low as -65°F
(-54°C).

(2) Extra (high) hazard areas.
(3) Location that makes fire extinguishers
susceptible to mechanical or physical damage.
(4) Exposure to abnormal temperatures or
corrosive atmospheres.
(b) Manual inspection of extinguishers shall
include a check of at least the following
items:
(1) Located in designated place.
(2) No obstruction to access or visibility.
(3) Operating instructions on nameplate
legible and facing outward.
(4) Safety seals and tamper indicators not
broken or missing.
(5) Examine for obvious physical damage,
corrosion, leakage or clogged nozzle.
(6) Pressure gauge reading or indicator in the
operable range or position.
(7) Fullness determined by weighing or
hefting.
(8) For wheeled units, the condition of tires,
wheels, carriage, hose and nozzle shall also be
checked.
(9) For nonrechargeable extinguishers using
push-to-test pressure indicators, test the
indicator.
(c) Electronic monitoring shall include
monitoring of the following items:
(1) Located in the designated place.
(2) No obstruction to access or visibility.
(3) Pressure gauge reading or indicator in the
operable range or position.

Nonrechargeable Extinguishers. When an
inspection of any nonrechargeable fire
extinguisher reveals a deficiency in any of the
conditions listed in (3), (4), (5), (6), (7) or (9) of
Title 19, Division 1, Section 574.2(b), it shall
be discharged and removed from service.
Exception: Nonrechargeable extinguishers
containing a halon agent shall be removed
from service, not discharged, and returned to
the manufacturer or local fire extinguisher
distributor having the capability of recovering
the halon agent.
[California Code of Regulations, Title 19,
Division 1, §575.10] Out of Service. Fire
extinguishers removed from service for
maintenance or recharge shall be replaced by
fire extinguishers suitable for the type of
hazard protected and shall be of at least
equal rating. [California Code of Regulations,

Extinguishers. Portable fire extinguishers
shall not be removed from the premises for
hydrostatic testing or any other purpose,
without first replacing the extinguisher with a

(b)] Removal of Tag.
(a) No person shall remove any tag, collar or
label required by Title 19, Division 1, Chapter
3, Article 9 from a portable fire extinguisher
except when service is performed.
(b) No person shall deface, modify or alter any
tag collar or label required by California Code
of Regulations, Title 19, Division 1, Chapter 3,
Article 9 to be attached to any portable fire
extinguisher.
[California Code of Regulations, Title 19,
Division 1, §574.5(a) through (c)] Inspection
Record Keeping.
(a) The fire extinguisher owner shall maintain
records of all fire extinguishers inspected,
including those extinguishers that were found
to require corrective actions. Records shall be
maintained until next required maintenance.
(b) At least monthly, the date the manual
inspection was performed and the initials of
the person performing the inspection shall be
recorded on a tag or label attached to the fire
extinguisher, or an inspection checklist
maintained on file, or an electronic system
(e.g., bar coding) that provides a permanent

Fire protection and life safety systems shall
be maintained in an operative condition at all
times, and shall be replaced or repaired
where defective. Nonrequired fire protection
and life safety systems and equipment shall
be inspected, tested and maintained or
removed in accordance with Section 901.8. All
fire alarm systems, fire detection systems,
automatic sprinkler or extinguishing systems,
communication systems, and all other
equipment, material or systems required by
these regulations shall be maintained in an

an operable condition at all times in
accordance with California Code of
Regulations, Title 19, Division 1 regulations

Maintenance of Equipment. All fire alarm
systems, fire detection systems, automatic
sprinkler or extinguishing systems,
communication systems, and all other
equipment, material or systems required by
California Code of Regulations, Title 19,

diminishment of the fire protective qualities
of such equipment, material or systems,
immediate action shall be instituted to affect

(3) Testing and maintenance on automatic fire
extinguishing systems exempted in writing by
the State Fire Marshal, when the building
owner or occupant has the staff and
equipment to conduct testing and
maintenance.
[California Code of Regulations, Title 19,
Division 1, §904.2(b)] Testing and
Maintenance Requirements.
(b) Any testing and maintenance of automatic
fire extinguishing systems shall be performed
in accordance with these requirements.
Exceptions:
(1) The State Fire Marshal may waive, in
writing, the requirement that testing and
maintenance be performed in accordance
with these requirements when a licensee can
demonstrate that a system cannot
functionally be tested and maintained in
accordance with the California Code of
Regulations, Title 19, Division 1, Chapter 5.
(2) If at any time a licensee encounters a
specialized or modified system which cannot
be tested and maintained in accordance with
California Code of Regulations, Title 19,
Division 1, Chapter 5, the licensee shall
contact the State Fire Marshal and test and
maintain the system as directed.
(A) The intent of this section is to cover
automatic fire extinguishing systems as
originally designed, installed and approved by
the Authority Having Jurisdiction. It is not,
however, intended to require that such
systems be upgraded to current adopted
standards.
