Location : Near Intersection of Madonna St and Los Osos
Valley Road in San Luis Obispo, California.
Dear Gentleperson:
Please be advised that our office has been retained by Ana Hernandez to represent the
above captioned client in recovering any and all damages arising from the accident which
occurred on the above captioned date
It is our understanding your city's video cameras at the intersection mentioned above
may have footage that captured the accident. This footage may be crucial in the resolution of our
client's i.rjury claim.
Kindly forward to our office a copy of anylall written incident reports and video footage you may
have on file regarding our client's accident.
1- Please provide us with a copy of all Outdoor Surveillance Video footage for
ALL cameras, including any and all video surveillance from outside cameras,
indoor and outdoor camera footage, for the period of L2:00am on December
14,2025 throueh 12:00am on December 16.2025.
We are happy to send someone from our office to obtain a copy as well please call us as
soon as possible to set up a time and date.
If you are unable to produce any of the above requested information, please provide us
with a written explanation as to what the reason is.
Furthermore, this letter is to put you on formal notice to maintain said written reports and
video footage during the pendency of this claim.

well as criminal penalties pursuant to Califurnia Evidence Code $ 4l 3, California Code of Civil
Procedure S 2023, andCalifornia Penal Code $ 135.
Once again, this writing shall serve as our formal notice and demand to preserve anv
