T-2 responded to an injury traffic accident with SLO PD on scene requesting. Upon arrival T-2 found two vehicles on the right
shoulder with one occupant still inside (driver) C/O back pain. Vehicle #1 was a silver Tesla with moderate damage to rear driver's
side door. Vehicle #2 was a black four door sedan with moderate damage to the front end. The Pt in the Tesla was assessed in
milddistress and refused any Tx/Ts signing out AMA.
Both vehicles were check for hazards/fluids down and none was noted. T-2 cleared W/O incident or change. See SLO PD accident

T-2 responded for reported 14X vehicle traffic collision with extrication required. Upon arrival, T-2 established Grand IC and
reported a two vehicle collision with one vehicle on its roof and an occupant inside retrieving personnel items. T-2 confirmed that
extrication was not required and reduced the assignment to T-2 and ambulance to handle. Driver and solo occupant of black Audi
sedan was self extricated and ambulatory PTA of EMS arrival without complaints of pain or injury. Driver and solo occupant of black
vehicle which had collided with Audi and came to rest on it's roof was also able to self extricate and ambulate PTA of EMS. Driver of
black vehicle was alert and oriented complaining of minor abrasions to left arm and left shoulder contusion possibly secondary to
seatbelt and airbag deployment. Post assessment, patient was offered ambulance transport to hospital which she declined. Patient
was advised of all assessment findings and recent vital signs. Patient was advised to seek further medical evaluation and treatment

on the front end by another sedan that was crossing the road. Driving going approx 35mph, see SLOPD report for details. Pt vehicle
had major front end damage, no passanger space intrusion and staring on windshield. Both vehicle secured and fluids controlled.
SLAS performed pt care on single pt that hit head on windshield, see SLAS PCR for details on pt. T2 assisted with debris removal

moderate damage. All occupants self extricated from vehicles and 1 driver requested medical attention. T2 paramedic and crew
rendered care. Upon arrival of San Luis Ambulance, T2 transferred care and the patient was transported. After transfer of care, T2
remained on scene providing scene clean up. Scene turned over to SLOPD, T2 cleared incident and returned to quarters.
