Hello Andrea,
My name is Jamie Moothart. I work with Access Central Coast as the Community Integration Specialist. I
have had the wonderful opportunity of providing direct support services to our disability and senior
community for over 3 years. My role is to help people who have been in an institutionalized setting, like
hospitals, and skilled nursing facilities, with transitioning back to community based living.
During my time with Access Central Coast, I have been able to support close to 100 individuals who need
assistance with navigating community resources so that they can become and remain safe and
independent in the community. Many individuals I assist need help with finding and applying for
affordable housing. This can be a complicated system, and many clients do not have as much
experience with using online resources. I typically go to meet with my clients in person at the
facilities they are staying and provide hands-on assistance with applying for housing programs. When
someone is housed, they then need connection to other services. I will assist in making sure they have
caregiving, access to food programs, transportation, durable medical equipment, and more. I
communicate with the facilities staff, the clients' families and other community organizations to make
sure the clients get all the support they need.
I think it is important to have someone that can physically go to meet with people, or for them to have a
physical location like our office, so that they can get the high level of support that we frequently provide.
