Correct the cited violation(s) by the "Reinspection Date" listed below to avoid further enforcement action including, but
not limited to, chargeable re-inspection and/or Administrative Citation. You may request a review of the cited violations by
the Department Director [Fire Chief ] within 5 days of report issuance if noted violations do not create an immediate danger
to health and safety (SLO M.C. 1.24.090).

Inspection Notes:Note: Construction inspection was completed with Mid-Coast Fire Protection, business owner and the
property management in conjunction with City permit No. Fire-1244-2025 & BLDG. 0274-2025.
Note: Five-year fire sprinkler system was last completed on 08/13/2021, License No. 761360. Annual-
year fire sprinkler system was last completed on 09/13/2024, License No. 761360. Title 19 inspection
report(s) have been uploaded to The Compliance Engine website. See attached records.
Note: Annual fire alarm inspection was last completed on ____, License No. ______. Alarm report is
pending from the contractor of record.
The following items were evaluated during acceptance testing for the hood extinguishing system
modification(s) per City permit No. Fire-1244-2025 and will require additional correction:
1) Activation of the hood extinguishing system via manual pull station initiated a supervisory
condition at the fire alarm control panel.
Per current NFPA 72 standards, hood extinguishing system actuation is required to activate an alarm
condition and is required to send an alarm signal to a UL listed central station. For further guidance
see NFPA 72 Section 17.14 (2022 Edition) & City of SLO Municipal Code 907.6.6 Monitoring.
Property management has been directed to coordinate correction and repair of this item with the
fire alarm monitoring service and the C-16 associated with City permit No. 1244-2025.
2) Make-up (supply) and exhaust air did not appear to function adequately during acceptance testing
for hood extinguishing system modification(s). Business owner has been requested to have a licensed
HVAC contractor and or CA licensed mechanical engineer evaluate the current hood exhaust system
and provide record of correction and or completion of repairs.

labeled in accordance with UL 1046 shall be in
