Correct the cited violation(s) by the "Reinspection Date" listed below to avoid further enforcement action including, but not limited to, chargeable re-inspection and/or Administrative Citation. Inspection Notes:Note: Follow-up (third) inspection was conducted by SLOFD on 12/24/2025 in conjunction with City permit No(s) BLDG-0312-2025 & BLDG-1099-2025. Previous inspections were conducted by SLOFD on 08/01/2025 and 12/22/2025. This inspection was requested by Daniel Leon (project manager) and Bruce Sandborn (business owner) and was attended by City Building Department staff. The following items were evaluated and were found to be corrected during this inspection: 1) Repair / replace manual pull stations for fire alarm system. ***Update: Manual pull station was found replaced and confirmed per prior phone conversation with Alpha Fire staff on 12/24/2025. Fire alarm system found to be operational during inspection. *** 2) Replace painted fire sprinklers inside the hallway corridor. *** Update: Painted fire sprinkler heads have been replaced. Visual inspection conducted for fire sprinkler(s) and associated piping inside theatre No.7 post installation of new drop panel ceiling. Unable to access fire sprinklers (x3) closest to theatre screen due to limited access. Additional inspection is required for fire sprinklers and piping in this location prior to final inspection for corresponding building permits. Commercial fire sprinkler riser was inspected and found to be operational. *** 3) Repair or cap exposed electrical wiring inside theatre No. 7. *Please note: If a third inspection is required due to non-compliance, a fire-inspection fee of $271 may be charged for that inspection, and every inspection thereafter until compliance is obtained. Failure to comply may result in a misdemeanor citation and an appearance by you in court.Page 1 of 13 *** Update: Electrical wiring has been covered and provided with an electrical outlet. Outlet operation will be tested by City Building Department staff prior to final inspection for corresponding building permits. *** 4)Repair emergency exit lighting inside theatre No. 7 and post occupant load signage. ***Update: Emergency exit signage has been repaired inside west exit. Unless exempt per stamped building plans additional exit lighting is required for the east exit prior to final inspection for corresponding building permits. *** The following items were evaluated and were found to require additional corrective action: 5) Repair the fire rated wall in the hallway corridor *** Update: Wallboard installation found to be in progress during inspection. Additional inspection of wallboard installation is required prior to final inspection for corresponding building permits. 6) Repair / replace exit door for theatre No. 7. *** Update: Door frame has been adjusted and will be subject to additional inspection by SLOFD / City Building Department staff. 7) Submit CSFM flame retardant listing for all decorative materials (i.e. curtains and wall upholstery). *** Update: Additional documentation has been submitted via email by Daniel Leon (project manager) on 12/24/2025. This documentation will be reviewed by SLOFD and City Building Department staff prior to conducting a final inspection for corresponding building permits. *** Note: Per direction of SLOFD Fire Prevention staff issuance of a temporary certificate of occupancy (TCO) is authorized pending approval of City Building Department staff. TCO issuance is conditional upon correction of pending code violations and not to exceed 30 business days. Note: Due to additional inspection a third inspection fee of $233.81 will be issued for this property. Upon receipt of invoice please coordinate payment with the City Finance Department. Open junction boxes and open-wiring splices shall be prohibited. Approved covers shall be provided for all switch and electrical outlet boxes. Electrical wiring for motor assembly for reclining chairs in auditoriums 1 and 2 observed exposed for several chairs. This is a repeated violation. Place cover on all chairs with exposed wiring by end of day. Send photo documentation this has been completed to inspector. 12/24/2025 603.6 Extension cords. Extension cords shall not be a substitute for permanent wiring and shall be listed and labeled in accordance with UL 817. Extension cords shall not be affixed to structures, extended through walls, ceilings or floors, or under doors or floor coverings, nor shall such cords be subject to environmental damage or physical impact. Extension cords shall be used only with portable appliances. Extension cords marked for indoor use shall not be used outdoors. Wine cooler in concession area observed plugged into extension cord. Plug appliance directly into permanent outlet. Send documentation to inspector by end of day. Discontinue use of temporary wiring in place of permanent wiring. Note: Electrical wiring was found exposed inside theatre No. 7 during inspection. Business management is directed to remove electrical wiring or install a listed cover plate (CFC 603.2). This item is required to be completed prior to occupancy of theatre No.7. 01/13/2026 12/24/2025 09:15:00 901.4 Fire protection and life safety systems. Fire protection and life safety systems shall be installed, repaired, operated and maintained in accordance with this code and the California Building Code. Auditorium 7 undergoing renovations. Observed ceiling tiles removed at screen end of auditorium. Ceiling tiles supported pendant sprinklers. Concrete ceiling observed above t-bar. Replace ceiling tiles or submit permit application through the SLO City Building Department to revise sprinkler protection in this auditorium. 12/24/2025 12/24/2025 09:15:00 5004. 3 Ventilation. Indoor storage areas and storage buildings shall be provided with mechanical exhaust ventilation or natural ventilation where natural ventilation can be shown to be acceptable for the materials as stored. Exceptions: 1.Storage areas for flammable solids complying with Chapter 59. 2.Storage areas for medical gases complying with Chapter 53. Auditorium 7, under active construction, observed accessible at the former emergency exit (adjacent to emergency exit from Auditorium 6). Secure access to auditorium 7 from the general public. Send photo documentation this has been completed to inspector by end of day. See section 11 of CFC 3303.1.1. 12/22/2025 12/22/2025 09:45:00 701.6 Owner’s responsibility. The owner shall maintain an inventory of all required fire-resistance-rated construction, construction installed to resist the passage of smoke and the construction included in Sections 703 through 707 and Sections 602.4.1 and 602.4.2 of the California Building Code. Such construction shall be visually inspected by the owner annually and properly repaired, restored or replaced where damaged, altered, breached or penetrated. Records of inspections and repairs shall be maintained. Where concealed, such elements shall not be required to be visually inspected by the owner unless the concealed space is accessible by the removal or movement of a panel, access door, ceiling tile or similar movable entry to the space. Note: New exit configuration completed for theatre No.7. Existing exit coordinator has been removed from service and covered with wallboard. Hallway corridor between theatre No. 6-7 will require additional wallboard installation to maintain the required fire rating for the corridor (CFC 701.6).
