Correct the cited violation(s) by the "Reinspection Date" listed below to avoid further enforcement action including, but not limited to, chargeable re-inspection and/or Administrative Citation. You may request a review of the cited violations by the Department Director [Fire Chief ] within 5 days of report issuance if noted violations do not create an immediate danger to health and safety (SLO M.C. 1.24.090). If you have any questions, please contact our Inspection Line at 805/783-7775. Inspection Notes:Annual fire sprinkler inspection conducted 09/19/2023, License No. 761360. Title 19 inspection report reviewed during inspection with property management. Annual fire alarm inspection conducted 09/20/2023, License No. 761360. Annual fire extinguisher maintenance conducted 05/2023, License No. 20219. As a general reminder, annual maintenance is due by 05/2024. Note: Emergency exit signage currently installed inside movie theatres (multiple locations) did not appear to illuminate adequately and per installation listing has expired (09/2023). Exit signage will require replacement in locations throughout building as noted above. Note: Emergency exit lighting did not appear to function properly in the following locations: Exit Hallway No.3 & No.6. Unable to inspect / test all emergency exit lighting during field inspection due to access (height) limitations. Business owner is directed to provide an inspection log for all emergency lighting appliances as noted above. Note: Exterior covers for electrical wiring and motor assembly for the reclining chairs were found removed (multiple theatre locations). Electrical wiring was found exposed in several locations. This item has been referred over to the City Building Department for further follow-up and investigation. Business owner(s) are directed to address the following item(s) prior to requesting a follow-up inspection: 1) Repair (or replace) exterior cover(s) for reclining chairs in movie theatres as noted above. 2) Provide record of inspection for all emergency exit lighting as noted above. Property management is directed to address the following items prior to requesting a follow-up inspection: 3) Repair or replace exit signage and emergency lighting as noted above. Upon completion of item No.1, please contact the City Building Department (805) 781-7180 to request a follow-up inspection. Upon completion of items No.2-3, please contact the SLOFD Inspection Line (805) 783-7775 to request a follow-up inspection. *Please note: If a third inspection is required due to non-compliance, a fire-inspection fee of $271 may be charged for that inspection, and every inspection thereafter until compliance is obtained. Open junction boxes and open-wiring splices shall be prohibited. Approved covers shall be provided for all switch and electrical outlet boxes. 03/28/2024 1008. 2 Illumination required. The means of egress serving a room or space shall be illuminated at all times that the room or space is occupied.
