To Whom It May Concern:
Please be advised that Anand Desai Law, PC represents Desiree Stephens in
connection with bodily injuries sustained as a result of a premises liability incident that
occurred on April 30, 2026. Kindly find enclosed a signed Designation of Attorney form
executed by our client. We respectfully request that all communication be directed to our
office. Please refrain from contacting our client directly without our express written consent.
This letter also serves as a formal demand that you preserve any and all evidence
relating to the above-referenced incident. Please immediately secure and preserve all
evidence related to this incident, including any video recording and data retrieved or
otherwise available from any data recording device(s) in operation at the subject premise on
the date of the incident. Failure to preserve such evidence may result in evidentiary penalties
(Evidence Code§ 413) and discovery sanctions (Cal. Code Civ. Pro.§ 2023.030). Please also
be reminded that California Penal Code§ 135 states: "A person who, knowing that any book,
paper, record, instrument in writing, digital image, video recording owned by another, or
other matter or thing, is about to be produced in evidence upon a trial, inquiry, or
investigation, authorized by law, willfully destroys, erases, or conceals the same, with the
intent to prevent it or its contents from being produced, is guilty of a misdemeanor." If there
are any costs or expenses, our office will cover them, provided we are promptly informed of
the nature and extent of the same.
Additionally, we request that you forward the following items for our review:
1. Any incident or accident reports prepared;
2. Any video surveillance footage or photographs related to the subject incident;
3. Copies of any statements obtained from witnesses or involved in parties (written or
recorded); and
4. Any documentation regarding maintenance, inspection, or repair logs relevant to the
premises where the in~ident occurred

Pursuant to Section 2695.2{c) of the California Code of Regulation, Title 10, Chapter 5, I authorize
Anand Desai Law, PC, its associates, agents, and/or representatives, to act as my counsel and
representative in communication, negotiation, litigation, and settlement(s) arising out of my
bodily injury and/or property damage claim arising from the above-referenced incident. Please
respond to this letter with a copy of your insured's declaration page.
This authorization shall remain in effect for an indefinite period, unless renewed or revoked in
writing by the undersigned as of the date of this authorization.
It is agreed that a photocopy of this authorization is to be considered valid as an original.
