The California Public Records Act (Government Code 7921. et set.) was enacted to ensure public
records are available for inspection by members of the public. Completion of this form will assist staff
in identifying related records to accurately complete your request.
Requested records will be distributed to the email address that is listed on this form, unless directed
otherwise by City staff. Requests for printed records will require payment subject to the City’s
Comprehensive Fee Schedule. Payment must be rendered prior to production of printed materials.

Release Forms
Requests for certain public records legally require release forms to be submitted for records to be
distributed to the requestor. To help expedite your request, please read below and ensure additional
information is submitted along with this public records request form.
•Personal health information
Records containing personal health information require a HIP AA Release Form. Examples
include fire incident reports, worker’s compensation claims, etc.

•Printed residential and/or commercial building plans
The Public Records Act does not allow the release of printed copies of this material without the
permission of the architect/engineer copyright owner. The public records requestor is
responsible for obtaining said authorization by completing all three release forms listed
below. You may call the Community Development Department at (805) 781 - 7170 to find out
the name of the copyright owner. In-person viewing of plans do not require release forms.

YOUR REQUEST WILL BE PROCESSED IN COMPLIANCE WITH THE PUBLIC RECORDS ACT (PRA). California
Government Code Section 7921. An Agency shall notify the requestor within 10 days from receipt of request with a
Determination which states if the Agency is in possession, in whole or in part, of the requested documents, and possible lega l
exemptions which prohibit the release of non-disclosable documents, as outlined per the PRA. In some instances, an Agency
may require an extension of up to 14 days to provide a Determination, as authorized by the PRA. A notice will be provided to the
requestor setting forth the reasons for the extension and the date on which a Determination is expected to be supplied.

To Whom It May Concern:
This is a request for government records made under the right of access granted by
the Constitution of the State of California as amended by the passage of Prop 59 on
Nov. 3, 2004, as well as the provisions of the California Public Records Act, California
Government Code §6251, with emphasis on the requirements of 6253.1. Please
provide the requested information in an electronic format.
I am seeking records showing the following information related to the following state-
awarded Homekey housing project (the “Project”): Beacon Studios (Calle Joaquin
Homekey), which City of San Luis Obispo applied for in San Luis Obispo, San Luis
Obispo County and received a Homekey award on 12/19/23:
1. Copies of City of San Luis Obispo’s application for Project Homekey funding,
and all related documents/materials that were a part of the application,
whether supplied at the time of the application or later.
2. Copies of all signed covenants and contracts related to the development and
operation of the Project.
3. The total amount of federal, state, local, and other funding received and spent
on the Project.
4. Copies of all evidence of payment or disbursement to City of San Luis Obispo
relating to funding for the Project. This request includes any correspondence
(letters, memos, emails) relating to the payment or disbursement at issue.
5. The total amount of federal, state, local, and other funding that City of San
Luis Obispo returned to the relevant government entity as a form of
reimbursement if the Project was either incomplete or late, or failed to
materialize altogether. This request includes any correspondence (letters,
memos, emails) relating to the funding returned or reimbursed at issue.
6. Any audits or compliance reviews relating to the Project. This request includes
any correspondence (letters, memos, emails) relating to the audits or
compliance reviews at issue.
7. The total number of units completed and permitted for occupancy in the
Project.
8. Monthly occupancy rates for the Project.
9. The number of evictions or forced tenant exits at the Project.

10. Correspondence (letters, memos, emails) related to the Project beyond the
correspondence requested above.
11. Meeting minutes and agendas relating to the Project.
12. Reports and studies pertaining to the Project.
13. Policies, procedures, and guidelines relevant to the Project.
14. Notes and drafts associated with the Project’s planning or execution.
15. Bids, proposals and requests for proposals relating to the Project.
16. Name of the owner of the property or properties that were the focus of the
Project. If the owner is a limited liability company (LLC), current evidence of
each general partner and the applicable operating agreement, if available.
17. All deeds related to the property or properties that were the focus of the
Project.
18. The purchase price and date of the property or properties that were the focus
of the Project; the property or properties that were the focus of the Project’s
prior owner; and the prior owner purchase price and date.
19. The required Annual Fiscal Year Expenditure Report applicable to the Project,
related expense ledgers submitted therewith, and all related correspondence
related to expenditures for the Project, whether before or after submission of
the Annual Report.
20. All requests for extensions to performance milestone deadlines, and all related
correspondence and materials.
I am requesting these records based on the understanding that San Luis Obispo
County was awarded funds by the California Department of Housing and Community
Development for the Homekey program. I further understand that this program
requires reporting on development and operational outcomes, and that local
governments routinely track homeless housing outcomes, as well as vital information
including deaths and potential crimes.
I realize, however, that you are not required to create a record if one does not exist.
Therefore, if you cannot provide a document or spreadsheet containing the requested
data, please provide whatever records do exist that might allow me to ascertain the
requested information, including but not limited to all state Homekey reporting
documents and staff memos or correspondence relating to Homekey properties / the
Project.
If you contend that any portion of the records requested is exempt from disclosure by
express provisions of law, Government Code §6253(a) requires segregation and
redaction of that material in order that the remainder of the records may be released.
If you contend that any express provision of law exists to exempt from disclosure all
or a portion of the records I have requested, Government Code §6253(c) requires that

you notify me of the reasons for the determination not later than 10 days from your
receipt of this request. Government Code §§6253(d) & 6255(b) require that any
response to this request that includes a determination that the request is denied, in
whole or in part, must be in writing and include the name and title of the person(s)
responsible for this response.
Additionally, Government Code §6253(d) prohibits the use of the 10-day period, or
any provisions of the CPRA or any other law, “to delay access for purposes of
inspecting public records.” In responding to this request, please keep in mind that
Article 1, §3(b)(2) of the California Constitution expressly requires you to broadly
construe all provisions that further the public’s right of access, and to apply any
limitations on access as narrowly as possible.
