Correct the cited violation(s) by the "Reinspection Date" listed below to avoid further enforcement action including, but not limited to, chargeable re-inspection and/or Administrative Citation. Inspection Notes:Follow-up (second) inspection conducted by SLOFD for initial inspection previously conducted on 12/11/2024. See supplemental records. Note: Annual fire sprinkler inspection was conducted on 06/27/2024, License No. 579499. Title 19 report was available for inspection and fire sprinkler riser room access was provided by onsite security due to currently inoperable Knox-box. See comment No.2 below. Note: Existing roof vent found covered with a plastic tarp cover during inspection. Recommend repair (or replacement) of this item by the property management. See comment No.1 below. During prior inspection the property management was directed to address the following items: 1) HVAC / Duct smoke detection units: Conduct an inspection for the fire detection system and provide an updated inspection report. Inspection is required to be conducted by a licensed fire alarm contractor and a record of inspection is required to be provided within 30 business days of this notice. ***This item has not been completed as of 01/09/25. 2) Fire detection system: Conduct an inspection for the fire detection system and provide an updated inspection report. Inspection is required to be conducted by a licensed fire alarm contractor and a record of inspection is required to be provided within 30 business days of this notice. ***This item has not been completed as of 01/09/25. 3) Knox-box:  Knox-box: The Knox-box onsite did not properly open during inspection. Consequently, SLOFD Fire Prevention staff were unable to access the fire sprinkler riser during inspection. Property management is directed to coordinate repair (or replacement) of the Knox-box onsite to provide emergency access to the fire sprinkler riser for this building. *Please note: If a third inspection is required due to non-compliance, a fire-inspection fee of $271 may be charged for that inspection, and every inspection thereafter until compliance is obtained. Violations Code Description Comments Reinspection Date 907.8 Inspection, testing and maintenance. The maintenance and testing schedules and procedures for fire alarm and fire detection systems shall be in accordance with Sections 907.8.1 through 907.8.4 and NFPA 72. Records of inspection, testing and maintenance shall be maintained. Note: To address recent call history for improperly functioning duct smoke detection and the fire detection system the property management is directed to coordinate field inspection(s) for the following items: 1) HVAC duct smoke detection units: Conduct an inspection of all units currently associated with the HVAC and the fire detection system onsite. Inspection is required to be conducted by a licensed HVAC contractor of record of inspection is required to be provided within 30 business days of this notice. 2) Fire detection system: Conduct an inspection for the fire detection system and provide an updated inspection report.
