Herewith we recommend that Part 7, Sections 6270, 6270.1 and
6270.2 of the Municipal Code be repealed. This ordinance re-
quires that all food handlers obtain chest x -rays every two
years and, if there is no evidence of tuberculosis, obtain
from the Health Department a permit 1�011work as a food handler.
This provision in the County law was recently repealed by the
Board of Supervisors at our request, the following reasons
having been advanced by us:
1. The requirement that all food handlers have chest x -rays
is out -dated and no longer considered an appropriate or
valid public health measure because this occupational
group does not have a tuberculosis incidence significant-
ly higher than that of the general population and because
tuberculosis is very rarely, if e ver, transmitted by way
of food.
We might just as reasonably require x -rays and permits for
all occupational groups in which there is any public contact.
2. The Health Department has never been able to adequately en-
force compliance because of the nature of restaurant work,
shortage of sanitarians, and diversion of sanitarians' atten-
tion to more pressing problems.
3. Considered as a case detection program it has been very in-
efficient. Analysis of our records indicates only four ac-
tive cases of tuberculosis detected in the last five years,
with an estimated 10,000 x -rays having been taken of food-
handlers in these five years.
This works out to a detection cost per case of $2,500, while
a cost study of the general tuberculosis control program of
the Health Department- involving approximately 10% of the
public health nursing staff time- reveals a case detection
cost of less than $250.
4. The State Health and Safety Code contains specific sections
relative to food handlers and communicable diseases,'and we
believe that these measures adequately cover the problem.

We will offer our communicable disease diagnostic services
.to all food establishments, continue to inspect restaurants,
and emphasize to the proprietors their basic responsibility
for good hygiene and sanitation.
However, if you decide that you wish to continue this Ordinance
in force, this service to your city will be additional to that
provided the unincorporated area and your existing sanitation
contract with the County must be revised to increase the sum an-
nually paid the County by $500.
This sum is based on an estimate of the number of food handlers
in San Luis Obispo, the employment turnover rate, the cost of
making and reading x -rays, and administration of the permit sys-
tem.
