Subject: REQUEST FOR RECORDS DESTRUCTION
On April 24, 2001, the City Council adopted a Records Retention Policy and Schedule. In
accordance with that policy and schedule, certain records have been identified as eligible for
destruction. A listing of those records and relevant sections from the records retention
schedule are attached.
The method of destruction will be to break the casing and shred the DVD as a means of
destroying the information. No records will be destroyed prior to the approved retention
period. We will begin destroying the records upon receiving your written authorization
below.
Request approval to destroy:
Server 3 DVD labeled ID3 beginning 4/20/2011 0910hrs — ending 3/26/13 0019hrs
Server 3 DVD labeled ID 10 beginning 3/26/13 0020hrs — ending 9/20/13 0136hrs
Server 2 DVD labeled ID 2013 beginning 02/15/2012 1603hrs — ending 10/04/2012 1301hrs
Server 2 DVD labeled ID 2014 5/2/11 — 3/16/13 corrupted DVD, bad data.
