The San Luis Obispo County Housing Trust Fund strongly supports HASLO's request for CDBG funds to
purchase homes in the City of San Luis Obispo for special needs households and individuals.
Increasing the supply of affordable rental housing for lower income households is rightfully the City's top
housing priority. Your proposal addresses this priority and it does far more.
Safe, supportive and affordable housing for our neighbors with special needs is even more critical than
affordable apartments. It is a matter of life and death for many. It is also more difficult and expensive
than simply building new apartment units. It can be j ust as difficult and time consuming to get the needed
approvals and financing for just one or two units of special needs housing as it is for 30 or 40 apartments.
In addition to the housing structure, you need to secure, coordinate and fund various services for the
clients who live in these homes.
HASLO has a long track record of partnering with local governments, funders and service providers to
make special needs housing successful. The Housing Trust Fund is proud to have worked with you on a
number of these projects. Incidentally, our most recent loan was for Hope House in Los Osos. We
partnered with you and Restorative Partners to provide a housing environment in which women released
from incarceration can re-enter society and reunite with their children. Kudos for your great work.
The Housing Trust Fund has always prioritized financing housing for those with special needs. We look
forward to partnering with you again to create more safe, supportive and affordable housing for those
with special needs using your new CDBG grant.

The County of San Luis Obispo is pleased to announce the availability of funds for the Community
Development Block Grant (CDBG) program. Applications MUST address one of the three national objectives
set by the U.S. Department of Housing and Urban Development (HUD), or they will NOT be considered for
CDBG funding (see the section on Qualifying Criteria for detailed information on the objectives).
Furthermore, completed applications should provide the necessary exhibits, budgets, or requested

Special Needs Households that are either homeless, or at risk of being homeless. These households with special needs
that are also homeless or at risk or homelessness face the greatest landlord barriers and are the most difficult to house.
We currently operate19 similar properties through the County (10 in SLO), and yet there remains a severe shortage. City
CDBG funds would leverage funds from HASLO, the Housing Trust Fund and private bank financing.

(3-2) Agency Mission Statement:
The mission of San Luis Obispo Non -Profit Housing Corporation is to provide affordable housing to lower income
households in San Luis Obispo County. SLONP is a 501c3 of HASLO, the Housing Authority of San Luis Obispo. They
maintain a special commitment to the poorest and most difficult to house special needs populations.
(3-3) Please describe your organization's capacity to implement the proposed project/program. Who will
be involved in the project/program? (In-house employees, contractors, other agency partners, etc.) List
projects of similar size and type that your organization has completed.
SLONP/HASLO has a long history of operating all types of housing, but in particular the group home and special needs
housing described in this application. SLONP/HASLO currently owns and operates 19 homes throughout San Luis Obispo
County. 10 are located in the City of San Luis Obispo. Referrals and wrap around services are provided by multiple
community partners, including Dept of Social Services, CAPSLO, SLOCODA, Transitions Mental Health, Restorative
Partners, Dignity Health and others. Each agency partner has indicated a severe shortage of housing for these special
needs populations. This CDBG project seeks to add to the supply of permanent supportive housing for special needs
homeless/at-risk of homeless individuals and families.
We are asking to retain flexibility in the subpopulation of Special Needs group that could be housed overtime, as
community needs change. However, the housing itself would permanently be reserved for Special Needs Homeless or At
Risk of Homeless. Some of the Special Needs populations currently residing in our 19 homes/group homes include mental
health patients in recovery, substance disorder residents in recovery, family reunification residents working with DSS,
women exiting incarceration seeking sober living with structured re-entry support services, and medically fragile homeless.
Each of the 19 homes is paired with a service provider MOU highly experienced with the needs of the particular population.
SLONP and HASLO will implement this activity through in-house employees. We maintain staff highly trained and
experienced in housing acquisition, financing, property management and CDBG compliance.

We gather housing applicant eligibility information at intake, including third party verifications. Staff are third party trained,
tested and certified in HUD and State of California compliance rules. Client files are maintained in locked, fire resistant
storage. Data is updated annually. Reports are provided regularly to all funders. We maintain two Certified Public
Accountants on staff. Annual third party audits are conducted and provided to our oversight board as well as funders.

Yes. New Special Needs Housing for Homeless/At Risk will be created. Additionally wrap around services that support
the personal development and self-sufficiency of residents will be provided.

Programs will be expanded through the addition of new Permanent Supportive Housing. This would represent a 10%
increase in the City of SLO (we currently operate 10 Special Needs housing facilities in SLO).

QUALIFYING CRITERIA (10 points)
The Community Development Block Grant program was established by Congress in 1974 with
passage of the Housing and Community Development Act and is administered by the United
States Department of Housing and Urban Development (HUD). This program provides funds to
municipalities and other units of government around the country to develop viable urban
communities. This is accomplished by providing affordable, decent housing, a suitable living
environment and by expanding economic opportunities principally for low and moderate income
persons. Although local units of government develop their own programs and funding priorities,
all activities must be consistent with one or more of the following HUD national objectives:
• Principally benefits low- and moderate -income persons
® Prevents or eliminates slum or blight
• Addresses an urgent need or problem in the community (e.g., natural disaster)
As an entitlement Urban County under the CDBG program, the County of San Luis Obispo receives
annual funding allocations from the federal government to fund activities to address these
national objectives.
As a funding recipient, San Luis Obispo County is required to submit an Annual Action Plan that
describes how the Urban County will utilize federal funds to address the national objectives in a
manner that will produce the greatest measurable impact on the Urban County communities. The
lead agency responsible for submission of this Plan to HUD is the Planning and Building
Department of the County of San Luis Obispo.

The target clientele is proposed to serve Special Needs populations that are homeless or at risk. The Special Needs
categories will include but not be limited to: mental health, substance recovery, re-entry, and family unification.
We anticipate that 95% will have a disability.
We anticipate that 95% will be Extremely Low Income (at or below 30% AMI).
Regarding age we anticipate the majority to be adults but also children in the case of family unification.

Homelessness and the lack of affordable housing that will accept homeless households is an extreme problem. Add a
Special Need such as the various type of circumstances or disabilities previously described - mental health, substance
recovery, family unification, or re-entry, and the barriers become almost insurmountable. The project specifically addresses
this hardest to house need. It also addresses these households need for wrap around services to be successful.
HASLO/SLONP has service partnerships with many agencies including CAPSLO, Restorative Partners, Transitions
Mental Health, County Drug and Alcohol, Rise, Stand Strong and others that we will expand upon.
HASLO/SLONP is a highly experienced developer and operator of housing in our community, and in particular very
experienced with Special Needs households. This project will succeed in being completed on schedule, within budget,
and operating well over time due to our experience and 50 year track record.
If the project is not funded, it will mean at least one additional year of these households living on the street or at high
homeless risk. In other cases it could mean an additional year before homeless parents are reunited with their children.
Due to the drawn out nature of obtaining CDBG funding (estimated 8.5 months from application deadline to HUD June
approval of County Action Plan, and then an additional 3-4 months until funding is available), it is most practical to enter
into escrow on the subject house once funding is committed by the City. At this writing, 10/18/18, there are 14 vacant 3+
bedroom homes for sale in the City within our $700,000 budget. This is typical, and we are confident as an experienced real
estate developer that given the nature of the project this is the best way to structure activity delivery/the acquistion.

BENEFICIARY DATA (15 points)
Organizations will be asked to provide detailed beneficiary data regarding race, ethnicity, gender,
income, etc. If they cannot provide data, they may not be eligible for funding.
(6-1) How do you document and maintain income status of each client in compliance with
HUD regulations? (Example: very low (<_50% AMI) and low (<_80% AMI) Area Median Income (AMI). Please
provide a sample of your intake process as an attachment if possible.
We certify for AMI income eligibility as well as other eligibility criteria for over 3,000 households annually. Our staff are third
party trained, tested and certified in HUD and State of California compliance rules for affordable housing programs including
CDBG.
Each applicant completes a housing application with relevent information for determining eligibility.
Each applicant then attends an in-person interview and staff go through a standardized questionnaire with them exploring in
detail eligiblity information.
Third party written documentation is obtained to verify all information.
Applications and supporting materials are maintained in individual client files and stored in locked filing cabinets.
(6-2) How do you collect demographic data on the beneficiaries of the proposed project or

FINANCIAL INFORMATION (20 points)
For CDBG applications to the County of San Luis Obispo involving acquisition, construction, or
rehabilitation projects, the County will require additional information on financial source and use
of funds and other budget details prior to the draft Action Plan funding recommendations.

This project will be self-supporting and will not require ongoing City/County funding for operations or maintenance Rent
receipts from clients will be utilized to cover operations, maintenance and debt service. However, because clients are
anticipated to have little to no income initially, HASLO will issue each client a Section 8 voucher, which will then be used to
cover rental costs on behalf of that client. Yes, these funds are available now.
This project is not required to pay prevailing wage CDBG rules require that projects designed for 8 or more units require
Davis Bacon wages. This project is designed for 5 residents, below the "8 unit" threshold of CDBG.

HMIS Reporting
All homeless service providers applying for CDBG funds to assist,
house, or shelter the homeless must identify and demonstrate the
capacity to participate in the County of San Luis Obispo Homeless
Management Information System (HMIS) to provide: personnel for
data entry, user licensing, and hardware and software necessary for
compatibility with HMIS. HMIS is an electronic data collection system
that stores client level information about persons who access the

QUALIFYING CRITERIA (10 points)
The Community Development Block Grant program was established by Congress in 1974 with
passage of the Housing and Community Development Act and is administered by the United
States Department of Housing and Urban Development (HUD). This program provides funds to
municipalities and other units of government around the country to develop viable urban
communities. This is accomplished by providing affordable, decent housing, a suitable living
environment and by expanding economic opportunities principally for low and moderate income
persons. Although local units of government develop their own programs and funding priorities,
all activities must be consistent with one or more of the following HUD national objectives:
• Principally benefits low- and moderate -income persons
• Prevents or eliminates slum or blight
• Addresses an urgent need or problem in the community (e.g., natural disaster)
As an entitlement Urban County under the CDBG program, the County of San Luis Obispo receives
annual funding allocations from the federal government to fund activities to address these
national objectives.
As a funding recipient, San Luis Obispo County is required to submit an Annual Action Plan that
describes how the Urban County will utilize federal funds to address the national objectives in a
manner that will produce the greatest measurable impact on the Urban County communities. The
lead agency responsible for submission of this Plan to HUD is the Planning and Building
Department of the County of San Luis Obispo.

HMIS Reporting All homeless service providers applying for CDBG funds to assist,
house, or shelter the homeless must identify and demonstrate the
capacity to participate in the County of San Luis Obispo Homeless
Management Information System (HMIS) to provide: personnel for
