Settle, Mayor Robert Griffin, Campaign asurer FROM: Lee Price, City Clerk SUBJECT: Reminder to file an Amended Campaign Statement As you will recall, I informed you by telephone on October 5h that Schedule A of your first pre-election campaign statement (Form 460) did not meet all the requirements for disclosing information about contributors of $100.00. The Political Reform Act, specifically Government Code Section 84211, requires that the name, address, occupation and employer of every contributor be listed on the form. The statement you filed on October 4th should be amended by listing the occupation and employer for every contributor. Using "private citizen" does not meet the State requirement. Copies of page 27 of the FPPC's Campaign Disclosure Manual and the applicable section of the Political Reform Act are attached for your reference. 11 Schedule A: Reporting Monetary Contributions Received Itemizing Contributors All monetary contributions (except loans) must be reported on Schedule A. The name, address, occupation, and employer of any contributor of $100 or more during the calendar year (January 1 - December 31) must be itemized. ' Records of all contributions of $25 or more must be kept and are used to determine whether an individual has contributed $100. Once a person's contribution has cumulated to $100 during the calendar year, enter the total amount contributed during the: • Statement period— in the "Amount Received This Period' column; and • Calendar year—in the "Cumulative to Date Calendar Year" column. Schedule A Form 490. Schedule A In addition, officeholders/candidates subject to contribution limits may be required to disclose the cumulative amounts of contributions received during the limitation period. Enter such amounts in the "Cumulative to Date —Other column. When a contribution from a recipient committee is itemized, enter that committee's identification number and full name and address. If an identifica- tion number has not yet been assigned, the name and address of the committee's treasurer must be listed. A lender who has forgiven a loan, or a third party that has made a loan payment, must be entered on Schedules A and B. Forgiveness of, or third party payment on,.a loan is a contribution.
