I request a document, spreadsheet or listing showing:    -- The name, regular pay, lump-sum pay, overtime pay, other pay, defined benefit plan cost, employee’s retirement cost covered, deferred compensation, health/dental/vision and total wages for all city of San Luis Obispo employees.     This is the same information that you reported to the state controller’s office, with the addition of each employee’s name.
