Remarks:
Arrived to find smoke and fire coming from the front door of the residence. T2 established California command and declared
a working incident. T2 FF pulled a crosslay and engaged in transitional attack. Pump 2 able to establish water supply. T2 made
confirmation with the residents of the involved unit that no one was inside. T2 knocked down fire and requested additional
engine and vent. BC1 took over command and assigned T2 DIVA and had E3 assigned to me. Div A coordinated salvage and
overhaul of residence. T2 confirmed no extension into walls or ceiling space. Fire Marshall arrived and conducted
investigation. Residents were interviewed by T1 Capt Lipson. Residences advised that two girls were sleep at 2200,
was with boyfriend in the room that was involved stated she lit a candle at
midnight, then went out. Came home at 230 and went to bed with candle still lit. woke up to smoke and fire in
room, no detector was activated in house, they had been removed and place on ground by occupants. were
able to get the other roommates out of the house. was treated by SLAS for minor burn injuries to arm. T2 confirmed
no extension and finished overhaul. T2 released by California IC. Scene turned over to rep,, by IC.

Additional Narrative (#1 of 3 ):
E-3 was assigned to assist Div A with fire attack and overhaul. E-3 performed overhaul and mop up in the room of origin, and
assisted with salvage and restoration of hose lines used by T-2 and was released.
Additional Narrative (#2 of 3 ):
BC1 arrived on scene and took over California Command and established the tac frequency as City Tac 1.
T2 was assigned Division Alpha and was ordered to do a transitional attack and given E3 and E3471 to work on the Division.
T1 was assigned ventilation group and established horizontal ventilation, cleared the second floor, and checked for extension
into the second floor.
E4 was assigned to back up Div A, assist with water supply, and to help secure the utilities.
BC3414 was assigned to Safety and was asked to do a walk around and he assisted with Utility control. These apartments only
had electrical service; there was no gas service to the apartments.
The fire was contained to the room of origin. E3471 and E21 (at FS#1) were released along with BC 3414.
PGE arrived on scene and was assigned with Captain E4 and the on site property manager, to
ensure the 5 apartments not affected could be re-inhabited. PGE locked out the electrical to the fire apartment (#1) and
the other apartments were re-energized. Captain Hale ensured that each apartment was below the 25ppm threshold for
CO.
Prev 20 was assigned to investigate the cause of the fire. See his report for specifics.
There were four occupants of the fire apartment. All occupants were able to escape the fire. One occupant had minor burns
on her forearm and was treated by SLAS M51. Patient refused to be transported to the hospital. T2 reported they fond
aworking smoke detector, but it was found on the floor in a basket.
T2 was assigned overhaul and worked with E3 and E4 to remove all the smoldering material from the building and utilized their
TIC to ensure no hidden fires in the walls and ceiling.
The property management company representative, arrived on scene and
the building was turned over to him. He was going to handle finding a new place for the displaced tenants.

Fire Origin and Cause Investigation
On Friday, May 10, 2019 at approximately 05:00 AM I received a phone call from the San Luis Obispo Fire Dispatch requesting
that I respond to 258 California Avenue to investigate the origin and cause of a structure fire. I arrived on scene at
approximately 5:20 AM and found a two-story apartment building with eight units and the first story unit closest to California
Avenue, identified as Unit A1 being overhauled.
Looking at the unit from the entry side I observed a large window opening onto the front porch that was broken out and
appeared to be the fire room. to the right was the entry door to the apartment that opened into the living room. This room
only exhibited smoke staining. On the ceiling of the living room I noted the melted smoke alarm base. It appeared that the
smoke alarm was not in place during the fire. Directly behind the living room was the kitchen. This room also only suffered
light smoke damage. From the living room was a short hallway that led to a bedroom and a bathroom. Both rooms only
showed light smoke damage. Only the smoke alarm base was observed on the wall in this bedroom. In the hallway I found an
electrical subpanel. Breakers seven and nine had been tripped and were identified as serving the heating unit.
The room with the most fire damage was the front bedroom that opened to the living room to the left of the main entry
door to the apartment. I entered this room and observed a post-flashover room. along the wall on my right I observed a
table and a bed, both had substantial damage. To the left of the bed was a wooden nightstand and next to that, another bed
that was along the wall to my left, and directly under the window that opened onto the front porch. As the window had
broken out during the fire ventilation patterns were evident throughout the room, especially directly across from the
window. In the area of the nightstand, between the beds there was an area of low burning and a defined 'V' pattern. As this
area was protected from the ventilation path I focused my attention to this area. Behind the nightstand was a duplex
electrical outlet. I examined the outlet and ruled it out as an ignition source, nothing was plugged in and it showed no internal
damage. On top of the nightstand was a salt lamp. The lamp showed only external damage and was not plugged in. Next to
the lamp I observed three glass containers. The glass container closest to the front edge of the nightstand was broken and I
observed the remnants of a candle at the bottom. Also on top of the nightstand was what appeared to be the remnants of a
paper booklet that was charred almost to ash. To the right side was a paper note book that was charred and on the left side
was a charred pillow. I examined the other electrical outlets in the room and ruled them out. I also examined the remains of
the thermostat on the wall and determined that the heat of the fire damaged the wiring causing a short, this would explain
the tripped breakers. I photographed the scene.
After my examination of the scene I met up with Captain Matt LIPSON who had interviewed the four residents who had self
evacuated. LIPSON explained that two of the residents had retired to their room for the night in the rear bedroom earlier in
the evening, while the two in the front bedroom had gone into the bedroom and lit a candle. The occupants in the front
bedroom then left the apartment with the candle still lit and returned at approximately 2:30 AM and went to bed, with the
candle still lit. At approximately 4:30 AM, the male occupant was awakened to a fire in the proximity of the nightstand and
went to find a fire extinguisher to no avail. He returned and assisted the female occupant to evacuate. the female occupant
suffered minor thermal injuries to her upper extremity and was treated at the scene. LIPSON also reported that the smoke
alarm was located by firefighters in a basket under a table.
It is my opinion that this fire's origin was the nightstand between the beds in the front bedroom. The material first ignited was
paper and the ignition source was an unattended candle. Cause is accidental.
