During this project an underground cable was damaged. The location of the property where this occurred is: Monterey Street near Andrews Street in San Luis Obispo, CA I am requesting copies of the following issued for the above location: 1.) Right-of-Way, Encroachment or Utility permits or contracts. 2.) Certification of General Liability Insurance, if required for above permits. Please search for the above for the time period of: 9/2018 to the end of 9/2019 Please provide copies of any permits or contracts, and insurance documents found via e-mail, fax or post at your discretion.
